This visit was conducted with Tonya Carpenter, lead teacher, and Lindsey Simmons, teacher. Your program currently operates with a Five-star license. Restrictions include first shift only, meets all enhanced requirements, reduced staff/child ratios minus one and school age equipment cannot be used by preschoolers. The license was observed, and the restrictions were found in compliance. The program is operated by Surry County Schools. The program’s compliance history was 99% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the classroom was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children engaged in sand, blocks, home living, manipulatives, and reading. I observed children participating in outdoor play. I observed materials and activities that were available to the children in the classrooms. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum. The last annual compliance visit was conducted on August 22, 2024. I received a copy of the sanitation inspection that was completed January 21, 2025, with superior classification. The last fire inspection was conducted on September 7, 2024. Program records and required postings were monitored. A fire drill was conducted on April 30, 2025. The last shelter in place drill was documented on January 23, 2025. An outdoor inspection was documented on 04/30/2025. Due to tropical storm Helene coming at the end of September 2024, a fire drill was not documented, and two fire drills were documented for October 2024. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Children were served lunch with a choice of chicken filet sandwich/corndog, nachos and cheese Doritos, carrots/ranch, peach cup/applesauce/mini rice Krispy Treat. Storage of hazardous items was monitored today. Hazardous items were stored in locked file cabinets. Storage and administration of medication were monitored. No medications were observed nor reported. Surfacing was not monitored due to the recent heavy rains. The program does provide transportation through the public school. Required DPI Transportation forms were current and on file verifying all requirements are monitored. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance on the annual compliance visit conducted on August 22, 2024. The following violations were cited during today’s visit: Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. There was not a fired drill documented for the month of November 2024. .0604(t); .0302(d)(5) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last shelter in place was conducted on 1/23/2025. .0604(u);.0302(d)(8) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Technical assistance discussed during today’s visit related to violations • There was not a fired drill documented for the month of November 2024. Ms. Carpenter stated he had been conducted and she received the actual date and time for November 6, 2024 and documented on the fire drill log. Fire drills are essential for ensuring the safety of children and staff in case of an emergency. These drills must be conducted monthly and documented and should be unannounced to mimic real-life scenarios. They involve evacuating the building, taking attendance, and returning to the facility once it's deemed safe. • The last shelter in place was conducted on 1/23/2025. Childcare facilities should conduct shelter-in-place and lockdown drills at least every three months. These drills, which involve staying inside the building to avoid external threats or securing individuals within a specific area due to immediate danger, are crucial for ensuring the safety and well-being of children and staff. Additional Information/Consultation: Prior to the beginning of the 2025-2026 school year, surfacing must be placed surrounding equipment more than 18 inches and must meet the requirement of 6 inches. North Carolina child care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. Provider Access to ABCMS allows administrators to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. To get started, complete the ABCMS Child Care Provider Portal Training in Moodle—a video tutorial followed by a few questions. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The visit was conducted by Audrey Davis, child care consultant with Tonya Carpenter and Lindsey Simmons, classroom teachers. This facility is operated by Surry County Schools. Your program currently operates one classroom with a five star license effective 1/7/20. Restrictions include daytime care, meets enhanced space, enhanced ratios, enhanced ratios minus one, and school-age play equipment off-limits to preschool children. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 93% prior to today’s visit. All programs are required to maintain at least 75% compliance. A playground inspection was last documented October 19, 2023. A fire drill and a shelter-in-place emergency drill were last documented 12/20/23. You agreed to scan and email me a copy of your most recent fire inspection. I observed a sanitation inspection last conducted on 8/17/23 earning a Superior classification with no demerits. A walk-through of the facility was completed. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. Children are just beginning a new study on buildings and are comparing and contrasting different versions of the Three Little Pigs along with a variety of other books and activities. Staff reported that children play outside daily when weather conditions permit. Today, children are enjoying brand new blocks for the outdoors. They also got a new sandbox and see-saw with recent NC Pre-K funds. I observed transition from classroom to bathroom to lunch. Lunch was a choice between yogurt pack, hoagie sandwich, potato wedges and chicken and rice with carrots or broccoli, fruit cup, and milk. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are lead teacher Tonya Carpenter, and teacher Lindsey Simmons. A selection of children's files was monitored for completed health assessments and developmental screenings. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses The Creative Curriculum. The classroom uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress and documentation was viewed today. The checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The classroom operates from 8 a.m. to 3 p.m. Parent conferences are held one time per year in the late fall. You stated that you use the Remind App, phone calls, email and face to face communications with parents. You verified that you incorporate family engagement via your Pre-K website and Ready Rosie. Storage of hazardous items was monitored today and are stored in a locked classroom closet. No new staff were reported at this program since your annual compliance visit on August 31, 2023. Limited monitoring of continuing staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, Special Training including Health and Safety Training and Criminal Background Checks. Seven children enrolled at this site ride the public school bus to and/ or from school on some days. I observed the required information on file for transportation. You stated each bus driver has been provided with an emergency and identifying information sheet with photo for each child transported. I observed a copy of parent- signed permission to transport including when and where the child is to be transported, approximate times of arrival and departure, and signed authorization that buses are insured, inspected, and in good, safe condition. One violation was observed during today’s visit.
