Restrictions include daytime care only, meets ratios minus one, meets enhanced ratios, meets enhanced space, space 1 and 5 not to be used for child care. The license was posted and the restrictions were being followed. The NC Secretary of State website was reviewed on 11-15-2024. The program’s compliance history was 94% prior to today’s visit. Programs are required to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. I have observed indoor and outdoor areas today. I monitored staff/child ratios and space capacity today, as is indicated on the attached worksheet. I observed proper supervision of children. During the visit I observed children playing indoors and outdoors and interacting with the teachers. The last sanitation inspection was conducted on 01-05-2024. The last fire inspection was conducted on 11-14-2024. A fire drill was documented on 10-22-2024. A shelter in place drill was documented on 11-07-2024. An outdoor inspection was documented on 10-16-2024. I observed program records and posted items today. I observed staff and children’s records today according to DCDEE procedures. I observed the storage of hazardous items and cleaning supplies. I observed both the storage of medications and the administration of medication paperwork. This program does not provide transportation. I documented the following violations during today’s visit: Violation Number Comment Rule 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. Two children were not signed in this morning in space 2. 10A NCAC 09 .0302(d)(4) 434 The materials and equipment indoors and outdoors were not sufficient to provide a variety of play experiences to promote the children's emotional and social development, health and physical development, approaches to play and learning, language and communication development, and cognitive development. There was one item missing in the art center in space 2. 10A NCAC 09 .0509(2) 1879 Prescribed medicines, that are pharmaceutical samples, was not stored in the manufacturers original packaging, was not labeled with the child's name, and/or written instructions did not include the required information. Comments: I have emailed the Head Start office and asked that they send me the Staff Worksheet for this site. Consultation: If Mr. Allen puts the menu in the hallway instead of in each classroom, then if lunch arrives and is not what is listed, there is only one place that staff must change the menu. Name of Operation: GRANITE CITY HEAD START Facility ID: 86000430 Consultant: TAMMY CHILDRESS Operation Type: Center Case Number: Visit Date: 3/18/2024 Number Present: 47 Completed Date: 3/18/2024 Age: From 3 To 5 Total Minutes: 185 Time In: 08:35 AM Time Out: 11:40 AM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance with all applicable child care requirements during the second temporary time visit following a change of location. The visit was conducted with Karl Allen, Site Director, by Tammy Childress, Lead Child Care Consultant. Currently this center operates with a Temporary License, issued on November 28, 2023, to operate first shift. The facility is licensed to provide care to children ages zero to five-years-old and is approved to operate three classrooms within the building with an occupancy of seventy-six. Restrictions are daytime care only, meets enhanced ratios minus one, meets enhanced space, and Spaces 1 and 5 may not be used. The Secretary of State website was checked today, and the owner, Yadkin Valley Economic Development District, Incorporated, is current on all required documents and active on the NC Business Registry. If any changes to the corporation need to be made or you decide to sale your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. All designated indoor and outdoor spaces for child care and staff records not previously monitored during the first temporary time period visit on January 17, 2024,were monitored today. All program records and documents required to be posted were monitored. The facility does not provide transportation. During today’s visit children present were observed to be engaged in large group activities, breakfast, free choice activities, personal care routines and outdoor play. The Creative Curriculum is used. Two classrooms are designated as NC Pre-K and NC Pre-Kindergarten Services were monitored. The following violations were cited during today’s visit: Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menus posted in Classroom 1 and 3 were for the prior week. 10A NCAC 09 .0901(b) 528 Food substitution was not of comparable food value or recorded on the menu prior to the meal or snack being served. The menu in Classroom 3 was for the current week, but listed that eh fruit served to children for breakfast would be sliced strawberries. The children in all classrooms were observed to be served oranges rather that strawberries with breakfast. 10A NCAC 09 .0901(b) 1914 The person identified as the person or alternate person responsible for carrying out the emergency medical care plan was not on the premises at all times and/or did not accompany children for off premise activities. The alternate person named in Classroom 3, is no longer employed in the classroom or on-site. .0802(b)(1-2) All violations must be corrected immediately. You must submit documentation on how each violation was corrected to Ms. Childress by April 1, 2024. The documentation should be submitted by email to Ms. Childress at tammy.childress@dhhs.nc.gov. Based upon the violations documented during today’s visit, the following rules were reviewed with Mr. Allen: 10A NCAC 09 .0901 GENERAL NUTRITION REQUIREMENTS (b) When food is prepared by or provided by the center, menus for nutritious meals and snacks shall be planned at least one week in advance. At least one dated copy of the current week's menu shall be posted where it can be seen by parents and food preparation staff when food is prepared or provided by the center. A variety of food shall be included in meals and snacks. Any substitution shall be of comparable food value and shall be recorded on the menu prior to the meal or snack being served. 15A NCAC 18A .2820 STORAGE (d) Medications including prescription and non-prescription items shall be stored in a locked cabinet or other locked container and shall not be stored above food. Designated emergency medications shall be kept inaccessible to children but are not required to be in locked storage. Non-prescription diaper creams and sunscreen shall be kept inaccessible to children when not in use but are not required to be in locked storage. 10A NCAC 09 .0802 EMERGENCY MEDICAL CARE (b) One person identified as the person or alternate responsible for carrying out the emergency medical care plan and ensuring that appropriate medical care is given shall: (1) be on the premises at all times; and (2) accompany children for off-premises activities. Your compliance history affects the star rating. It is imperative that staff maintain compliance with child care rules and requirements at all times. Violations of child care requirements affect the facility’s compliance history. You must maintain a minimum compliance history of seventy-five (75) percent. In addition, children in Classroom 3 (Edwards) were washing their hands in the classroom sink after toileting, not the bathroom sink because Ms. Edwards stated that the push type faucets did not run long enough for handwashing. It is important that the sinks are sanitized each time there is a change of use between toileting and eating/messy play which is required by Sanitation Rule 15A NCAC 18A .2818 LAVATORIES: (c) Lavatories shall be cleaned and disinfected with each change of use, when visibly soiled, and at least daily. Change of use occurs when a handwash lavatory is used outside of its original intent. Change of use includes, but is not limited to, a classroom handwash lavatory used for rinsing toothbrushes, a food preparation handwash lavatory used for toy cleaning, or a classroom handwash lavatory used for diaper changing handwashing. You will remain in the current assigned cohort (Cohort 1), with a preparation year ending June 30, 2024, and the reassessment year, when an Environment Rating Scale Assessment will be completed, is from July 1, 2024, through June 30, 2025. Because a Change of Location occurred, you are not required to complete an Environment Rating Scale Assessment or Rated License Assessment while on the Temporary License. All required inspections have been received and the previously issued Five Star Licensed will be processed and mailed directly to you following today’s visit. Your previously assigned Child Care Consultant, Audrey Davis, will resume monitoring of your facility once the Five Star License is issued. Name of Operation: GRANITE CITY HEAD START Facility ID: 86000430 Consultant: TAMMY CHILDRESS Operation Type: Center Case Number: Visit Date: 3/18/2024 Number Present: 47 Completed Date: 3/18/2024 Age: From 3 To 5 Total Minutes: 185 Time In: 08:35 AM Time Out: 11:40 AM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance with all applicable child care requirements during the second temporary time visit following a change of location. The visit was conducted with Karl Allen, Site Director, by Tammy Childress, Lead Child Care Consultant. Currently this center operates with a Temporary License, issued on November 28, 2023, to operate first shift. The facility is licensed to provide care to children ages zero to five-years-old and is approved to operate three classrooms within the building with an occupancy of seventy-six. Restrictions are daytime care only, meets enhanced ratios minus one, meets enhanced space, and Spaces 1 and 5 may not be used. The Secretary of State website was checked today, and the owner, Yadkin Valley Economic Development District, Incorporated, is current on all required documents and active on the NC Business Registry. If any changes to the corporation need to be made or you decide to sale your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. All designated indoor and outdoor spaces for child care and staff records not previously monitored during the first temporary time period visit on January 17, 2024,were monitored today. All program records and documents required to be posted were monitored. The facility does not provide transportation. During today’s visit children present were observed to be engaged in large group activities, breakfast, free choice activities, personal care routines and outdoor play. The Creative Curriculum is used. Two classrooms are designated as NC Pre-K and NC Pre-Kindergarten Services were monitored. The following violations were cited during today’s visit: Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menus posted in Classroom 1 and 3 were for the prior week. 10A NCAC 09 .0901(b) 528 Food substitution was not of comparable food value or recorded on the menu prior to the meal or snack being served. The menu in Classroom 3 was for the current week, but listed that eh fruit served to children for breakfast would be sliced strawberries. The children in all classrooms were observed to be served oranges rather that strawberries with breakfast. 10A NCAC 09 .0901(b) 1914 The person identified as the person or alternate person responsible for carrying out the emergency medical care plan was not on the premises at all times and/or did not accompany children for off premise activities. The alternate person named in Classroom 3, is no longer employed in the classroom or on-site. .0802(b)(1-2) All violations must be corrected immediately. You must submit documentation on how each violation was corrected to Ms. Childress by April 1, 2024. The documentation should be submitted by email to Ms. Childress at tammy.childress@dhhs.nc.gov. Based upon the violations documented during today’s visit, the following rules were reviewed with Mr. Allen: 10A NCAC 09 .0901 GENERAL NUTRITION REQUIREMENTS (b) When food is prepared by or provided by the center, menus for nutritious meals and snacks shall be planned at least one week in advance. At least one dated copy of the current week's menu shall be posted where it can be seen by parents and food preparation staff when food is prepared or provided by the center. A variety of food shall be included in meals and snacks. Any substitution shall be of comparable food value and shall be recorded on the menu prior to the meal or snack being served. 15A NCAC 18A .2820 STORAGE (d) Medications including prescription and non-prescription items shall be stored in a locked cabinet or other locked container and shall not be stored above food. Designated emergency medications shall be kept inaccessible to children but are not required to be in locked storage. Non-prescription diaper creams and sunscreen shall be kept inaccessible to children when not in use but are not required to be in locked storage. 10A NCAC 09 .0802 EMERGENCY MEDICAL CARE (b) One person identified as the person or alternate responsible for carrying out the emergency medical care plan and ensuring that appropriate medical care is given shall: (1) be on the premises at all times; and (2) accompany children for off-premises activities. Your compliance history affects the star rating. It is imperative that staff maintain compliance with child care rules and requirements at all times. Violations of child care requirements affect the facility’s compliance history. You must maintain a minimum compliance history of seventy-five (75) percent. In addition, children in Classroom 3 (Edwards) were washing their hands in the classroom sink after toileting, not the bathroom sink because Ms. Edwards stated that the push type faucets did not run long enough for handwashing. It is important that the sinks are sanitized each time there is a change of use between toileting and eating/messy play which is required by Sanitation Rule 15A NCAC 18A .2818 LAVATORIES: (c) Lavatories shall be cleaned and disinfected with each change of use, when visibly soiled, and at least daily. Change of use occurs when a handwash lavatory is used outside of its original intent. Change of use includes, but is not limited to, a classroom handwash lavatory used for rinsing toothbrushes, a food preparation handwash lavatory used for toy cleaning, or a classroom handwash lavatory used for diaper changing handwashing. You will remain in the current assigned cohort (Cohort 1), with a preparation year ending June 30, 2024, and the reassessment year, when an Environment Rating Scale Assessment will be completed, is from July 1, 2024, through June 30, 2025. Because a Change of Location occurred, you are not required to complete an Environment Rating Scale Assessment or Rated License Assessment while on the Temporary License. All required inspections have been received and the previously issued Five Star Licensed will be processed and mailed directly to you following today’s visit. Your previously assigned Child Care Consultant, Audrey Davis, will resume monitoring of your facility once the Five Star License is issued. Emergency Preparedness and Response Plan You must update your existing Emergency Preparedness and Response Plan to include the new license number and to make any edits regarding the location of the facility. You will need to print a copy of the revised plan and review it with all staff. Document the date of the review and maintain in staff files. Continue to conduct annual reviews of the plan with all staff and print off the first page of the plan, showing the current date for the year that you reviewed it in the system. Lead-Based Paint and Asbestos Testing You have enrolled started enrollment for lead-based paint and asbestos testing. Once testing results are received or proof that that testing is not required, please send this information to your assigned child care consultant. Karl Allen, On-Site Director, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Mr. Allen and three additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. On 11/30/2023, staff members allowed 30 children, 3-year-old to 5-year-old, to play in a combined space on the playground. On 12/14/2023, staff members allowed 20 children, 3-year-old to 5-year-old, to play in a combined space on the playground. Based upon the center's license, the maximum group size, for a group including 3-year-old children, was 18 children. GS 110-91(7);.0713(a-d) Violations must be corrected immediately. Within one week, 12/21/2023, you must submit documentation of the corrections you made and your plan to maintain compliance with the identified child care requirements to me at scott.mason@dhhs.nc.gov.
