This visit was conducted with lead teachers Lauren Thomforde, and Allyson Kenyon. Your program currently operates with a Five Star license. Restrictions include first shift care only, meeting all enhanced requirements and reduced ratios minus one, Certified Developmental Day and preschool may not use school age equipment. The license was observed, and the restrictions were found in compliance. The NC Secretary of State website was reviewed on May 1, 2026, and Surry County Schools were listed as current and active. The program’s compliance history was 100% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children in free play activities, outdoor play, and lunch. I observed materials and activities that were available to the children in the classrooms. The last annual compliance visit was conducted on August 18, 2025. A sanitation inspection was completed on September 9, 2025, with a Superior classification. The last fire inspection was conducted on September 15, 2025. Program records and required postings were monitored. A fire drill was conducted on April 22, 2026. The last lock down drill was completed on October 16, 2025. This visit was also used to monitor compliance with applicable childcare requirements located in Child Care Rule Section .3000 in spaces 1 and 2, where children participating in the NC Pre-K program are cared for. This visit was conducted with lead teachers Lauren Thomforde, and Allyson Kenyon. Your program currently operates with a Five Star license. Restrictions include first shift care only, meeting all enhanced requirements and reduced ratios minus one, Certified Developmental Day and preschool may not use school age equipment. The license was observed, and the restrictions were found in compliance. The NC Secretary of State website was reviewed on May 1, 2026, and Surry County Schools were listed as current and active. The program’s compliance history was 100% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children in free play activities, outdoor play, and lunch. I observed materials and activities that were available to the children in the classrooms. The last annual compliance visit was conducted on August 18, 2025. A sanitation inspection was completed on September 9, 2025, with a Superior classification. The last fire inspection was conducted on September 15, 2025. Program records and required postings were monitored. A fire drill was conducted on April 22, 2026. The last lock down drill was completed on October 16, 2025. An outdoor inspection was documented on May 1, 2026. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. You stated) that you do provide transportation in the morning for six (6) and afternoon for seven (7) children. During today’s visit, I monitored emergency information, permission to transport forms for all children being transported. All DPI transportation forms were on file and signed by the appropriate individual. All bus drivers had their criminal background checks on file. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum. The developmental day requirements in section .2900 of the childcare rules were monitored for compliance. Children who receive developmental day services are currently cared for in spaces 401 and 402. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .2902(f) were verified in compliance. Lead teachers assigned to each of these spaces have NC Birth-through-Kindergarten licenses and administrator has Level III NCECAC with over six (6) years’ experience. The program operates six and half hours per day, Monday through Friday, nine months per year. Children’s individual plans for care and developmentally appropriate activity plans are followed, and opportunities for inclusion with children who are typically developing are offered. I confirm this facility provides the following five family involvement activities: quarterly parent education sessions, parent/teacher conferences twice a year; daily notes of progress provided to parents; opportunities for parent volunteers to assist with special classroom activities, and other learning experiences for children; and provides parents with referral information about other community programs and resources serving young children. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are lead teachers Lauren Thomforde, Allyson Kenyon and teachers, Pamela Rorrer, Ann Smith, Beatriz Alvarez, Christy O’Neal, and Lisa Upchurch, A selection of files was monitored for completed health assessments and developmental screenings. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum. You stated you completed the Dial screenings on (date) for all children and as new children are enrolled. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress and documentation was viewed today. Checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The first day of school was August 27, 2025. The classroom operates from 7:35a.m. to 3:00 p.m. Parent conferences are held two times per year. You stated that you use Planners, Class Dojo to communicate with parents. In addition, a communication planner is sent home to the parents which includes any information about the child’s day and any reminders. In addition, parents are welcome to volunteer, attend specials activities and eat lunch with the children. School events such as fall festivals and other school events also encourage family engagement. The environment rating scale score received on 12/3/2019 was 5.78. The following violations were cited during today’s visit: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. Rust and chipping/peeling paint on outside poles leading to the playground were accessible to the children. 10A NCAC 09 .0601(a) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last lockdown drill was documented on 10/16/2025. .0604(u);.0302(d)(8) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The license was observed, and the restrictions were found in compliance. This program is owned and operated by Surry County Schools. The program’s compliance history was 82% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having outdoor and indoor play, group time, lunch, and rest. I observed developmentally appropriate materials and activities were available to the children in both classrooms. The last annual compliance visit was conducted 9/21/22. A sanitation inspection was conducted 4/19/23 with a Superior classification. The last fire inspection was conducted 9/6/23. I observed a playground inspection documented 8/21/23. Staff files and children’s records were monitored per DCDEE procedures. This school provides transportation to and from school by public school bus in the mornings and afternoons for some children. I observed permission to transport forms acknowledging when and where children are to be transported, expected time of departure and arrival, and the transportation provider. I observed transportation forms completed and signed by a school administrator verifying that all vehicles are in good condition and free of hazards; have current registration and inspection; all drivers have proper license and have completed orientation on transporting preschool children; appropriate child restraint devices are used, if required by NC Child Restraint Laws; and all DCDEE procedures are being followed. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. A teacher's handbag was stored on a high shelf because it contained the teacher's emergency medication. The teacher reported ibuprofen was also in the handbag which was not locked. During the visit, the teacher took the ibuprofen out to her car where she stated she would keep it in the future. 15A NCAC 18A .2820(d) 1834 Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. I observed transportation forms completed and signed by a school administrator verifying that all vehicles are in good condition and free of hazards; have current registration and inspection; all drivers have proper license and have completed orientation on transporting preschool children; appropriate child restraint devices are used, if required by NC Child Restraint Laws; and all DCDEE procedures are being followed. Developmental Day requirements in section .2900 of the childcare rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .2900 were verified in compliance. Children who receive developmental day services are currently cared for in spaces 402 and 404. The program operates 7 hours and 15 minutes per day, Monday through Friday, 10 months per year. Children’s individual plans for care and developmentally appropriate activity plans are followed. Opportunities for inclusion with children who are typically developing are provided throughout each day. I confirmed this facility provides the following family engagement involvement activities: ___ parent/teacher conferences two or more times per year ___ daily notes of progress provided to parents by way of phone calls, text messages, photos, Ready Rosie, class Dojo, and the Backpack series information sheet. ___ opportunities for parent volunteers to assist with classroom needs such as preparing classroom materials at home. ___ parents are provided with referral information about other community programs and resources serving young children via the school social worker. Vision and dental screening services are provided for the children on site. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Lead teachers assigned to each of these spaces have NC B-K licenses and the administrator has a Level III NCECAC. A selection of children’s files was monitored for completed health assessments and developmental screenings. Please note children’s health assessments are required on file within 30 days of enrollment, by September 25th. The school uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses The Creative Curriculum. Checkpoint assessments are conducted every 11 weeks. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. A teacher's handbag was stored on a high shelf because it contained the teacher's emergency medication. The teacher reported ibuprofen was also in the handbag which was not locked. During the visit, the teacher took the ibuprofen out to her car where she stated she would keep it in the future. 15A NCAC 18A .2820(d) 1834 Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. Two children with emergency medications did not have medical action plans on file. .0801(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before September 26, 2023. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. Technical assistance provided during today’s visit: • Reminder: Health and Safety training topics are required to be completed every 5 years, and can also count as on-going training in the year they are taken. * All NC Pre-K teacher assistants are required to get 15 hours of on-going training per year. * You stated parents do not enter the classrooms and have not since the pandemic. Child care rules require that parents must be allowed to enter spaces where their child receives care.
