The last annual compliance visit was conducted on 9/17/25. I completed a walkthrough of the indoor space used by children and monitored for the following: supervision, staff/child ratios, CPR and First Aid certification, storage of hazardous products and medication, adequate and approved space and program records. Staff files and the staff and training worksheet from the last annual compliance visit was used to monitor for current required information for the existing staff. All staff have current criminal background check qualifying letters on file. There have been no new staff hired since the last monitoring visit. A fire drill was conducted on 4/24/26. A lockdown drill was last completed on 2/11/26. Shelter-in-place or lockdown drills were not recorded as required. A superior sanitation inspection was completed on 9/15/25 with 2 demerits. The last fire inspection was conducted on 3/30/26. The playground inspections have been completed as required. The following items were posted for easy reference: license, NC Summary of Law, First Aid poster, evacuation routes, safe arrival and departure procedures, menu, staff/child ratio chart, emergency telephone numbers, emergency procedures form, tobacco free signage and emergency medical care plan. During the visit there were seventeen (17) children present. I observed the children participating in large group teacher-directed activities and bathroom/handwashing routines. The teachers were actively engaged in all activities with the children. All interactions were positive. There were two (2) violations of childcare requirements observed during the visit. One (1) of the two (2) violations was corrected during the visit. Violation Number Comment Rule 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The lockdown drill conducted on 11/3/25 was not recorded on the drill log report. J. Pettiford recorded the drill during this visit. .0604(u);.0302(d)(8) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. J. Pettiford completed all health and safety training topics in January/February 2021, these topics were not completed again until March/April 2026. S. Wilcox completed all health and safety training topics in January/February 2021, these topics were not completed again until March/April 2026. .1103(b) The violation documented during the visit must be corrected immediately. To verify correction of the violation you shall submit a signed letter to me explaining in detail how you have corrected the violations described above. Upon arrival, I met with Shanelle Wilcox, Teacher. This NC Pre-K classroom is located inside a public school. I conducted a walk-through of the classroom. The facility operates with a 5 star license issued 3/21/19 and had a 99% compliance history prior to this visit. There were 15 children present during the visit. The children were observed during indoor free play in activity areas, and bathroom/ handwashing routines. The Annual Compliance Monitoring Checklist for Child Care Centers was used as a guide to note the requirements monitored today. Prior to the visit an email was sent requesting an updated copy of the staff and training worksheets. The worksheets were received prior to the visit. There have been no new staff hired since the last monitoring visit. During the visit one (1) staff file was monitored. I monitored the regulatory system to verify current criminal record qualification letters for staff. Two (2) children's files were monitored during the visit. Fire drills and shelter-in-place/lockdown drills have been completed as required; however, they fire drills were not documented as required. The last fire drill was conducted on 8/28/25, the last lockdown drill was conducted on 9/15/25. The monthly playground inspections have been completed as required. A superior sanitation inspection was conducted on 9/15/25 with 2 demerits. The last fire inspection was completed on 3/4/25. The following items were posted for easy reference: license, NC Summary of Child Care Law, safe arrival and departure procedures, emergency medical care plan, emergency telephone numbers, daily schedule, activity plan, menu, applicable staff/child ratio charts, tobacco free signage, First Aid poster and evacuation routes. I monitored the following items during the visit: supervision, staff/child ratio, staff, health, safety, program records, management and medication. NC Pre-K: DIAL-4 developmental screenings are currently underway as school just began on August 25, 2025. The facility uses Creative Curriculum. On-going developmental assessments are completed throughout the school year using Teaching Strategies Gold, checkpoints, and self-made teacher assessments. The Teaching Strategies Gold is completed three times per year, assessing the children in social, physical, cognitive language development, literacy, math, arts, social studies and science. The information gathered from the assessments is used to prepare information that is shared with parents during parent/teacher conferences that are held two times per year. A developmental and learning report is prepared and shared with the parent. Home visits are conducted twice per year. The facility meets the 1:9 staff/child ratio requirements. The classroom operates from 7:00am until 2:30pm to meet the required 32.5 hours per week requirement. Newsletters are sent home monthly and include information on menus and upcoming events. Family engagement activities occur monthly. There is a parent engagement activity scheduled for 9/25/25. Parents are encouraged to engage in their child's education and asked to complete take home activities with their child. The facility provides transportation using the public-school bus. The following violations of child care requirements were observed today. Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. During this visit it was observed that a fire drill for August 2025 was not documented on the drill record. School personnel provided documentation showing that a fire drill was conducted on 8/28/25. S. Wilcox, Teacher, completed the drill record during the visit. .0604(t); .0302(d)(5) The cited violation was corrected during the visit; therefore, a corrective actions letter is not required to be submitted. You are required to maintain compliance with all applicable childcare rules and regulations at all times. I encourage the facility administrator to visit the DCDEE website and view the documents under the QRIS Modernization tab. *Rule Rollout Training in Moodle: The Child Care Commission adopted rules to support QRIS modernization. The new section, .3200, provides the standards for earning a two-though five-star-rated license. To support the QRIS changes, additional rules were amended within the following sections: definitions, developmental day services, and NC Pre-Kindergarten services. You will need to have an NCID - the same NCID that you use for the health & safety training, WORKS login, and/or the CBC Portal to participate in Moodle training. I met with Johnie Pettiford, Lead Teacher. This Head Start classroom is located inside a public school. I conducted a walk-through of the classroom. The "Annual Compliance Monitoring Checklist for Child Care Centers" was used during the visit. The facility operates with a 5 star license issued 3/21/19 and has a 99% compliance history. There are currently sixteen (16) children enrolled and sixteen (16) children were present during the visit. The children were observed during indoor free play, bathroom/ handwashing routines, teacher directed group activities and gross motor play. Prior to the visit I sent an email requesting the staff and training worksheet to Jeannette Smith, Site Manager. The worksheets were received prior to the visit. I selected two (2) children's records for review. There has been one (1) new staff hired since the last monitoring visit. The file of the new staff member was monitored today. One (1) existing staff file was monitored during today's visit. I monitored regulatory to verify current criminal record qualification letters for all staff. A superior sanitation inspection was conducted on April 22, 2024. The last fire inspection was conducted on 9/4/24. Fire drills have been conducted as required. The last fire drill was conducted on 8/29/24. A lockdown drills and shelter-in-place drill have been conducted as required but have not been documented on the emergency drill form as required. This was corrected today. The monthly playground inspections have been completed as required. The facility provides transportation using the public school bus. Family child care homes are required to test water for lead once. o DCDEE encourages you to utilize the Clean Classrooms for Children’s program. You can take the pre-enrollment webinars which is available at www.cleanwaterforUSkids.org/carolina then enroll – At www.cleanwaterforUSkids.org/carolina completing the enrollment surveys for three program sections: 1) lead in water, 2) lead-based paint, and 3) asbestos. Keep mindful that once you evaluate potential hazards and receive results if there are recommendations and/or water mitigation support is needed they will provide direction in how to mitigate and ways to fund said mitigation.
