I introduced myself to the school administrator and classroom teachers. I conducted a walkthrough of the indoor and outdoor spaces with Ms. Bynum. All spaces used for children were monitored. The children were observed participating in center activities, routine care, eating lunch, and children engaged in outdoor play. The lunch menu consist of chicken nugget, potato tots, apple, wheat roll, and milk. CURRENT LICENSE STATUS: This program operated with a Four (4) Star Licensure that was issued April 6, 2023, earning 7 points in program standard, 3 points in staff education Component, and 1 quality point through enhanced space and reduced ratios. INSPECTIONS: The last sanitation inspection was conducted on 2/27/23. The last fire inspection was conducted on 3/1/23. The last fire drill was conducted on 11/27/23. The last outdoor inspection was conducted on 11/29/23. The last Emergency drill (lockdown drill) was conducted 10/26/23. All applicable child care requirements were assessed during today’s visit. The facility does not provide transportation. Therefore, this requirement was not monitored. A checklist was used to note the requirements monitoring today. PROGRAM RECORDS: Staff on-going training requirements were assessed from November 30, 2022 – November 30, 2023, and was in compliance. Nine children’s records were viewed today. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 1314 Emergency information did not name child's health care professional. Three children files did not have a health care professional listed. .0802(c)(2) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. Three children did not have a health assessment on file. GS110-91(1) 1322 A written statement from each child's parent giving standing permission which may be valid for up to twelve months for participation in off premise activities that occur on a regular basis was not available. One child did not have written consent to play outside the fenced area. .1005(b)(4) 1323 Each child was not immunized as per Article 6 of Chapter 130A and an immunization record was not on file before or within 30 days after admission. One child's file did not have documentation of immunization record. 10A NCAC 09 .0302(d)(2) 1851 The operator did not notify the parent of each child enrolled in writing of the smoking and tobacco restriction. One child's file didn't have written documentation on file. .0604(j) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. Two children files did not have a written statement of acknowledgement for head trauma policy. .0608(b)(1-6) The above violations must be corrected immediately. Please email me a signed letter of compliance by 12/13/23. Before the letter is sent, all violations should be corrected; please contact me if additional time is needed to correct a specific violation. Include the following in your letter: facility ID number, list the item number describing how and when the violations were corrected and tell me how you plan to maintain compliance in the future with each item. COMPLIANCE HISTORY Prior to today's visit your compliance history was 100%. TECHNICAL ASSISTANCE: The information below was shared with you to maintain quality within your program and to prevent future violations. Reminder: *One employee is schedule to complete the playground training in January- 2024. Please place training certificate in your file to be monitored during my next visit . * During today’s visit I checked nine (9) children’s files and three of the files did not have a health assessment. Please make sure that this information is on file if stored off site. *One child did not have an immunization record on file; all children must to have immunization on file within 30 days.
