This documentation was reviewed with you, and a copy of the visit summary documentation was given to you at the end of the visit. Upon arrival, I was greeted by the center director, Bobbi Jo Mofield, and we conducted a general walk-through of the indoor and outdoor learning environments. There were 11 classes opened today. I observed groups of children engaged in free play in their classroom and outdoors and a few groups participating in group time activities. In addition, one group of school-aged children were on a field trip. RATED LICENSE STATUS The program operates with a 5-star rated license issued on 12/23/22. They received 6 points in program standards, 7 points in staff education and one quality point through the programmatic option of having a staff benefit package and an infrastructure of parent involvement. All staff should be registered and have their education assessed through WORKS. Best practice recommends that this be completed as a part of new employee orientation and then updated as staff education changes The center has selected pathway 1 and wishes to complete their Environment Rating Scales in late fall. We discussed today that I will follow-up with them in mid-late August to determine the best time to submit their request for scales. INSPECTIONS The last sanitation inspection was completed on 3/25/26. The last fire inspection was completed 5/1/26. The last documented fire drill was conducted on 4/29/26. The last documented lock down or shelter-in-place drill was conducted on 1/29/26. The last documented playground inspection was conducted on 4/24/26. MONITORING During this visit a full assessment of Child Care Requirements was conducted. The facility was assessed using items noted on the Annual Compliance for Centers checklist. The status of the owner, ES North Carolina, LLC is listed as current and active on the NC Secretary of State’s website. Staff on-going training was assessed from 7/1/25 – 6/30/26. The following violations were observed and recorded during today’s visit: Violation Number Comment Rule 847 Parent's medication authorization did not include required information.Multiple medication authorization forms for expired for children's emergency medications. 10A NCAC 09 .0803(4)(6-9) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. One child's epi-pen expired in February 2026. .0803(12) 1834 Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. One child's allergy care plan expired in April 2026. .0801(b) The above violations must be corrected immediately. By 6/10/26 please email me a signed letter of compliance. Before the letter is sent, all violations should be corrected; please contact me if additional time is needed to correct a specific violation. Include the following in your letter: facility ID number, list the item number describing how and when the violations were corrected and tell me how you plan to maintain compliance in the future with each item. If the compliance action letter is not received by the due date another visit to the center may be made. COMPLIANCE HISTORY Prior to today’s visit, the compliance history score for the center was 98%. According to NC General Statute 110-90 (4) (d) all facilities must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months. REMINDERS The 2nd school administrator position was recently filled. I left you a copy of the Pre-Service Requirements for Child Care Center Administrators form. Please have your new administrator complete this paperwork and email it to me as soon as possible. TECHNICAL ASSISTANCE Medication administration is one of many responsibilities of center staff complete on a routine basis. It is crucial that medication authorization forms be completed properly, that all medication is returned to the parent when the medication is no longer needed or when it expires and that all allergy care plans be reviewed as needed and updated on at least an annual basis. During today’s visit numerous medication authorization forms expired, and on child’s epi-pen medication expired in February 2026 and one allergy care plan expired in April 2026. Best practice would be to complete monthly checks of all center medication and updates be made as needed each month. Allergy care plans are valid for one year and medication authorization forms for all emergency medications must be updated every 6 months. Medication authorization forms for topic medications such as sunscreen, diaper cream or insect repellent is valid for one year. . CONSULTATION With the extreme heat conditions we have been experiencing, we are cautioning all programs to closely monitor the temperatures of playground surfaces. During a recent visit, a consultant found that the artificial turf, in an unshaded area, was hot to touch. This could result in children being burned while playing outdoors. I left a handout with you today titled, “Protecting Children from Thermal Burns.” This handout discussed prevention and treatment options should a child experience a burn. Child Care facilities are required to test their drinking and cooking water for lead every three years. Your water was last tested for lead on 7/12/23. In addition, child care facilities are now required to test lead and asbestos hazards in their buildings. The program was determined to be exempt from this testing. ACCESS TO THE ABCMS SYSTEM: The center is registered and using this system. I printed the center’s roster and we discussed the edits that should be completed. There are tutorials on the Criminal Background Check website to assist you, if needed. AGENCY UPDATES Please check the DCDEE website frequently for updates and changes to child care requirements and the new QRIS requirements. The director, Bobbi Mofield assisted me with today’s visit. A checklist was used to note the requirements I monitored today. We conducted a general walk-through of the indoor and outdoor play spaces. I observed groups of children engaged in free play in their indoor and outdoor learning environments. Upon arrival the school age children were leaving to go on a field trip to the movies. LICENSE STATUS The program holds a 5 star-rated license issued on 12/23/22. The program received 7 points in Program Standards, 6 points in Staff Education and a quality point through the programmatic option of having an infrastructure for parent involvement and a staff benefit package. The last emergency drill (lockdown drill) was conducted 6/20/23. The last documented playground inspection was conducted 6/9/23. The status of the owner, ES NORTH CAROLINA LLC, is listed as current and active on the NC Secretary of State’s website. All applicable child care requirements were monitored during today’s visit for transportation. The program transports the children; therefore, these requirements were assessed. All guidelines were incompliance during today’s visit. The following violations were observed and recorded during today’s visit. TECHNICAL ASSISTANCE ADMINISTERING MEDICATION A parent may give a caregiver standing authorization for up to 12 months to apply over the counter, topical ointments, topical teething ointment or gel, insect repellents, lotions, creams, fluoridated toothpaste, and powders, such as sunscreen, diapering creams, baby lotion, and baby powder, to a child, when needed. Please make sure that before any medication is received that an authorization form is completed.
