Name of Operation: ALL STARZ CHILDREN'S ACADEMY Facility ID: 92002485 Consultant: KIMBERLY GIROUARD Operation Type: Center Case Number: Visit Date: 2/12/2026 Number Present: 211 Completed Date: 2/12/2026 Age: From 0 To 5 Total Minutes: 450 Time In: 09:15 AM Time Out: 04:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor All Starz Children’s Academy for compliance with applicable childcare requirements for an annual compliance visit. A full assessment of childcare requirements was conducted today. Your program currently operates with a Five-star license. You received five stars on April 15, 2024. You are licensed for 299 children on first shift. You are approved to care for children 0-5 years of age. Your facility meets enhanced ratios and enhanced space. When I entered the building, I observed your childcare license posted in a visible location. Today’s visit was conducted with you, Ms. Kym Paterniti, Center Director. You accompanied me throughout my walk-through of the facility today. I, Kimberly Girouard, Child Care Licensing Consultant, monitored (14) fourteen classrooms, the kitchen, four (4) playgrounds, staff/child ratios, supervision, capacity, group size, staff interaction and discipline, required trainings, criminal background checks, emergency medical care plan, administration of medications, storage of substances and medications, health and safety requirements, adequate and approved space, program records, and permit restrictions. At the time of my visit there were a total of 211 children present between the ages of 0yrs. – 5yrs. years of age. The children were observed engaged in indoor center activities and outdoor play. While inside I observed the children participating in circle time, engaged in art activities, engaged in center play, eating lunch, and going down for naps. Two infant rooms were monitored today. The infants were observed engaged in floor activities, group time, napping, and going down for naps. While conducting my walk-through of the facility today, I observed the staff engaged in the children’s activities while providing appropriate supervision. Each of the four playgrounds offers a pleasant age-appropriate outdoor environment. All Starz Children’s Academy uses Creative Curriculum and currently they are using an internal assessment. VERIFICATION OF RECORDS: *Last fire inspection – 12/16/25 *Sanitation inspection – 5/30/2025 (5 demerits and a superior rating) *I reviewed the NC Secretary of State’s website and observed the owner of this facility, All Star’s Children’s Academy, Inc. is currently listed as current and active. * EPR plan was revised on September 5, 2025 * Fire drill – 1/29/26 * Lockdown drill – 12/4/2025 * Playground inspections were found to be in compliance. One violation was cited during today’s visit. Violation Number Comment Rule 1320 Children's records that include an application for enrollment, medical and immunization records, and permission to seek emergency medical care was not on file for each child. Two children's files were missing the required medical assessment information to be completed by the child's parent on the Medical Information form. GS 110-91(1);.0302(d)(2); .0304(g) TECHNICAL ASSISTANCE One violation was cited today for missing immunization records. We discussed the importance of using the file check list for children’s files to confirm all required documentation is completed properly before being placed in the child’s file. RATED LICENSE: All Straz Children’s Academy will be due to renew their rated license in March 2027. DCDEE has transitioned to a new Rated License process referred to as Pathways to the Stars and is in effect now. During today’s visit Ms. Paterniti and I discussed Pathway 1 and Pathway 2. I showed Ms. Paterniti where she can find information regarding our new QRIS Pathway to the Stars on the DCDEE website. Suggestions shared today: 1) I reviewed the QRIS websites with Ms. Paterniti and suggested she continue to review them as well and to call me if she has any questions. 2) I suggested Ms. Paterniti confirm all of her staff are currently in WORKS and all of their education is up to date in the system. 3) During our discussions today regarding the QRIS Modernization Pathway To The Stars, Ms. Paterniti and I discussed aiming towards having her application completed and ready to submit around January 2027 time frame. CORRECTIVE ACTION LETTER Please submit a corrective action letter for the above violation. While the Division allows you time to explain how you have corrected violations when they are documented, it is expected that you will correct all violations immediately. Your corrective action letter must include the following: Name of your program, Id number of your program, Date of Letter, Violation(s) number(s), and How and When each violation was corrected.
