The visit was conducted with you, Savanna Woody, Program Coordinator. This program currently operates with a five -star license issued on February 17, 2022. Permit restrictions were in compliance. The Secretary of State website was reviewed, and The Young Men’s Christian Association of Catawba Valley Inc. is current and active as of today 6/8/26. The last annual compliance visit was conducted 6/24/25. A checklist was used to note the requirements I monitored today. The most recent monthly fire drill was conducted on 5/6/26. The most recent quarterly lockdown/shelter-in-place drill was conducted on 3/10/26. The facilities EPR Plan was reviewed on 6/1/16. Medication was in compliance. The school age children present today were divided into five groups. I observed the school age children during lunch, bathroom/handwashing routines, audio story time, free choice, and board and card games, and departure. You provided me with applicable program, staff, and children’s records for review. Files for two new staff and two existing staff were reviewed with no violations. A sample of twenty-two children’s records were reviewed with no violations. Three violations observed. The violations were discussed with you and documented in the summary. You had to leave. Since no one else was listed on the Legal Designee form who could sign the summary, a handwritten summary was completed and signed during the visit. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. A current Activity plan was not posted. GS 110-91(12); .0508(a) 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. A child, six year of age, was playing on the grassy area in front of the school. The program coordinator and the group leaders were not in the proximity of the bathroom. .2506(d)(1-3) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. You, Savanna Woody, Program Coordinator assisted me with the visit. You provided me with applicable program and staff records for review. The most recent annual compliance visit was conducted on 6/24/25. This program currently operates with a issued on February 17, 2022. The license was posted, and the restrictions were in compliance. This facility has earned 6 points in program standards 4 points in education, and 1 quality. The North Carolina Secretary of State website was reviewed on was listed as The Young Men’s Christian Association of Catawba current-active. Limited monitoring was conducted. School age children present were divided into three groups. I observed the school age children during arrival, snack, bathroom/handwashing routines, homework/reading, free play and transition to the outdoor playground. Supervision and staff/child ratios to be in compliance. Two files for new staff and applicable records for existing staff were reviewed. All staff were listed on the ABCMS Provider Portal roster for this facility. The most recent approved fire inspection for your facility was conducted on 5/21/25. The analysis date for the most recent lead water test was 3/26/24. Lead testing must be completed every three years. Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. One child, enrolled on 8/10/25, had an expired inhaler in the emergency medication bag. The inhaler expired on 2/2026. .0803(12) The violation documented above was corrected during the visit. The expired medication was discarded. The program’s compliance history was 92 percent prior to today’s visit Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Failure to correct the violations and send the written statement by the due date may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. The visit was conducted with you, Logan Smith YMCA 21st Program Director and Caleb Clark Program Coordinator. The purpose of the visit was reviewed with you and you assisted with the visit. The NC Secretary of State website was viewed before the visit and The Young Men’s Christian Association of Catawba Valley Inc. was current/active as of June 24, 2025. The facilities contact information was reviewed and confirmed with you. A four-star license was issued on February 17, 2022. The following restrictions were in compliance: Daytime care only, School age children only, Playground does not meet child care playground safety, meets enhance ratios, meets enhanced space. School age children present today were observed during lunch, a science activity, bathroom handwashing routines, a reading activity, departure, and transitions. Some enrolled students were not in attendance due to summer school. They will be attending next week. The last sanitation inspection was conducted 5/2825. A Superior classification was issued with 0 demerits noted. Two new staff files and one existing staff file was reviewed with one violation documented. Some of the staff are from another YMCA and working for the summer. The fire marshal conducted an inspection on 5/27/25. The facility is approved for daytime care only. The last fire drill was completed on 6/9/25. A lockdown drill was conducted on 6/10/25. One violation regarding medication was documented and discussed with you. Two violations were observed documented and discussed with you today’s visit. Violation Number Comment Rule 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. One child had an Epi-pen that was not in the original box with the label. The child’s name had been written on the Epi-pen. .0803(2)(a) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. The visit was conducted with you, Kayla Buchanan, Program Coordinator. The Secretary of State for North Carolina website was viewed before today’s visit and Young Men's Christian Association was current and active. A four-star license was issued to this program on February 17, 2022. A checklist was used to note requirements monitored during the visit. School age children present and staff were observed during free choice activities bathroom / hand washing, lunch, and a group leader directed activity. A fire inspection was conducted on June 20, 2023. Shelter in place drills and fire drills had been conducted and documented on the DCDEE form. A sanitation inspection was conducted on September 26, 2022. Two violations were observed today and were discussed with you. A hand written summary was signed during the visit due to a connectivity issue. Violation Number Comment Rule 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. An open purse, belonging to an employee, was on the counter in space #1. A bottle of Choraseptic sore throat spray was in the purse.
