The visit was conducted with you, Lisa Smith Robinson, Program Coordinator. This program currently operates with a five -star license issued on February 17, 2022. The permit restrictions include Daytime care only, playground does not meet child care playground safety, meets enhanced ratios, meets enhanced space. The NC Secretary of State website was reviewed on 219/26 and Young Men`s Christian Association of Catawba Valley, Inc. was listed as current-active. The facilities contact information was reviewed and confirmed. The last annual compliance visit was conducted 3/3/25. The most recent monthly fire drill was conducted on 2/4/26. The most recent quarterly lockdown/shelter-in-place drill was conducted on 12/18/25. The facilities EPR Plan was reviewed on 12/30/25. The center's compliance history was reviewed with the operator. The program’s compliance history was 96% percent as of 2/1/26. Space used for this program is the gym at the school. School age children were observed during arrival, during outdoor gross motor play, free choice activities, snack, bathroom/handwashing routines, and departure. Supervision and staff /child ratios were maintained. The caregivers were interacting and meeting the developmental needs for each of the children. A checklist was used to note the requirements I monitored today. The analysis date for the most recent lead water test was 4/9/2024. Lead testing must be completed every three years. Files for three new staff, one existing staff, and applicable records for all staff were reviewed. One staff member was not listed on the ABCMS Provider Portal roster for this facility. A sample of five children’s records were reviewed. The violation observed today was discussed with you and documented in the handwritten Visit Summary left with you at the conclusion of this visit. The following violation was observed: Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. An employee, with a hire date of 12/30/25, was not listed in the ABCMS system for the program. G.S. 110-90.2 & .2703(r) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation documented must be corrected immediately. On or before March 5, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. The NC Secretary of State website was viewed before the visit and The Young Men’s Christian Association of Catawba Valley Inc. was current/active as of June 12, 2025. The school age children in care were divided into two groups. A Superior classification was issued with 0 demerits noted. One staff file was reviewed for an employee who is working during the summer at this site. The employee is assigned to another site during the school year. The fire marshal conducted an inspection on 5/27/25 . The facility is approved for daytime care only. The last fire drill was completed on 5/15/25. A lockdown drill was conducted on 3/31/25. Medication was in compliance. None of the enrolled children had medication. One violation was observed today. Violation Number Comment Rule 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. The permit restrictions were in compliance. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios were in compliance. Children in care were participating in outdoor gross motor activities, indoor free play activities, hand washing/bathroom routines, snack, and departure. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Upon arrival, the purpose of today’s visit was reviewed with you, Ann Effler, Regional YMCA director. Your program currently operates with a five star license effective February 17, 2022. The permit restrictions were in compliance. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios were in compliance. Children in care were participating in outdoor gross motor activities, indoor free play activities, hand washing/bathroom routines, snack, and departure. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Files for five new staff were reviewed with one violation documented. The most recent sanitation inspection for your facility was conducted on November 13, 2023. A superior sanitation classification was issued with 11 demerits. The most recent fire inspection for your facility was conducted on June 20, 2023. The facility was approved for day time care. A fire drill was conducted on November 27, 2023. One violation was observed and documented today.
