Name of Operation: QUAKER MEADOWS GENERATIONS Facility ID: 12000175 Consultant: GAIL DULA Operation Type: Center Case Number: Visit Date: 12/14/2023 Number Present: 93 Completed Date: 12/14/2023 Age: From 0 To 5 Total Minutes: 190 Time In: 10:30 AM Time Out: 01:40 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements during a routine unannounced visit. Upon arrival, the purpose of today’s visit was reviewed with you, Cheryl Leonhardt, Director. The Secretary of State website was accessed, and the Blue Ridge Community Action was current and active as of today December 14, 2023. Your program currently operates with a five star license effective September 24, 2018. The permit restrictions were in compliance. Children in care were observed during arrival, free play activities, lunch and outdoor gross motor play. I reviewed two new staff, who transferred from another Blue Ridge Community Action facility, were reviewed. The most recent sanitation inspection for your facility was conducted on June 6, 2023. A superior sanitation classification was issued with 10 demerits. A fire inspection was conducted on 1/12/23. A fire drill was conducted 11/27/23. Two new staff files were reviewed with one violation documented. Four violations were observed and documented during today’s visit. Violation Number Comment Rule 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. One child enrolled on 11/23/23 did not have a completed feeding schedule. 10A NCAC 09 .0902(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space #59 an unlocked cabinet contained the following: A spray can of Lysol, Champion Spray disinfectant, Champion foaming cleaner, aerosol spray Scum B gone, Novo air freshener, Noble disinfectant, and Scrubbing Bubbles in an aerosol can. .2820(b) 898 All electrical appliances were not used in accordance with the manufacturers instruction. Appliances with heating elements, such as bottle warmers, crock pots, curling irons, irons, coffee pots, and/or their cords were accessible to preschool-age children. In space #63 an unplugged glue gun was on a shelf and accessible to children. .0604(e) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. The director’s IT/SIDS training expired on 12/10/23. .1102(f) The above uncorrected violations must be brought into compliance immediately. You shall submit a written, signed and dated statement or email to me at the address below detailing specifically how the above violations was corrected. The violations must be corrected no later than December 28, 2023. Failure to correct the violations and send the written statement by the due date listed above may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. As discussed, violations cited today and/or during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. Your program must maintain a compliance score of at least 75% as required by G.S. 110-90 (4)(c). Technical Assistance: #540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. One child enrolled on 11/23/23 did not have a completed feeding schedule. All children under the age of 15 months must have a feeding schedule posted. #840 In space #59 an unlocked cabinet contained the following: A spray can of Lysol, Champion Spray disinfectant, Champion foaming cleaner, aerosol spray Scum B gone, Novo air freshener, Noble disinfectant, and Scrubbing Bubbles in an aerosol can. All hazardous items with multiple warnings and items in aerosol containers must be kept in locked storage. Items labeled “keep out of reach of children” with no other warnings can be stored five (5) feet off the ground. Have staff check their classroom thoroughly each morning to ensure all hazardous products are stored properly. Remind all staff to place all hazardous supplies back in locked storage after use. I also suggested you have staff check the classrooms thoroughly several times daily and after foster grandparents have opened cabinets. #898 In space #63 an unplugged glue gun was on a shelf and accessible to children. All electrical appliances were not used in accordance with the manufacturers instruction.
