Upon arrival, the purpose of today’s visit was reviewed with you, Ruth Judd, on site program coordinator. Jennifer Hawkins, Regional YMCA directors arrived during the visit. Your program currently operates with a five star license effective February 17, 2022August 19, 2022. The permit restrictions were in compliance. I observed the school age children care during transition from lunch, head count procedures, bathroom/hand washing routines and during free play and group games in the gym. Supervision and staff/child ratios were in compliance. Files for four new staff were reviewed with no violations. The most recent sanitation inspection for your facility was conducted on May10, 2023. A superior sanitation classification was issued with 9 demerits. The most recent fire inspection for your facility was conducted on June 7, 2024. The facility was approved for day time care. A copy of the inspection was emailed to me. Medication was in compliance. One violation was documented today and discussed with you regarding the sanitation inspection. Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. rating.0304(b). A yearly sanitation inspection had not been conducted. The last inspection conducted by environmental health was completed on May 10, 2023. 10A NCAC 09 .0304(b) The violation documented must be corrected immediately. On or before June 27, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. If you email the compliance letter, it must be sent from the email address on file with DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Upon arrival, the purpose of today’s visit was reviewed with Vivian Vang, Group Leader. A substitute was also working. Cassidy Dale, Regional Director, arrived during the visit. Ms. Dale completed the Staff training Worksheet during the visit. Your program currently operates with a five star license effective August 19, 2022. The permit restrictions were in compliance. Children in care were observed during snack, transition to the restroom, transition to Space #1, a teacher directed activity, outdoor gross motor play and departure. A sanitation inspection was conducted by Environmental Health on May 10, 2023, with 9 demerits noted. The most recent fire inspection for your facility was conducted on June 20, 2023. The facility was approved for daytime care. The latest fire drill was conducted on January 8, 2024. Children’s records were reviewed with no violations. Staff files were reviewed with no violations. Medication was in compliance. Two violations were documented during today’s visit Violation Number Comment Rule 546 Opportunities were not provided for children to participate in the planning and implementation of activities. There was no documentation regarding opportunities for the children to participate in planning and implementing activities. 10A NCAC 09. 2508(b) 1424 School-aged children were not adequately supervised. During a transition, children were lined up on the sidewalk outside of the gym. One group leader called names of children to divide them into groups. One group was sent into space #1 while the other group prepared to go to the playground. Both group leaders were outside on the sidewalk while children in group #1 entered the space as their name was called. Both group leaders remained outside until all of the children were divided. .2506(d)(1-3) The above uncorrected violations must be brought into compliance immediately. You shall submit a written, signed and dated statement or email to me at the address below detailing specifically how the above violation was corrected. The violation must be corrected no later than February 12, 2024. The correction letter must include the facility name and ID#. Failure to correct the violations and send the written statement by the due date listed above, noncompliance, may lead to an administrative action.
