Name of Operation: MISS SHEILA'S Facility ID: 12000283 Consultant: GAIL DULA Operation Type: Center Case Number: Visit Date: 8/17/2023 Number Present: 13 Completed Date: 8/17/2023 Age: From 1 To 8 Total Minutes: 75 Time In: 10:00 AM Time Out: 11:15 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for a routine unannounced visit. Upon arrival, the purpose of today’s visit was reviewed with you, Shelia Rutherford, Administrator. Your program currently operates with a three star license effective April 29, 2019. The permit restrictions were in compliance. A walk-through of the facility was conducted. All indoor and outdoor areas were monitored. I observed children in both classrooms. Children in care were participating in free play activities. Children in care were also observed during transitioned to outdoor gross motor activities,and bathroom/handwashing routines. Staff interacted in positive ways with the children. Child/staff ratios were maintained. There was no new staff. The most recent sanitation inspection for your facility was conducted on June 2, 2023. A superior sanitation classification was issued with 14 demerits. The most recent fire inspection for your facility was conducted on February 2, 2023. The facility was approved for day time care. Fire drills and lockdown/shelter in place drills had been conducted and recorded. The following violation(s) were observed today: Violation Number Comment Rule 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In space #1 pullups in plastic bags were in the cubbie and the container was less than 5 feet in height. A child two (2) years of age is enrolled in the space. .0604(q) The violation documented was corrected during the visit. The center's compliance history prior to today’s visit was reviewed with the operator. The program’s compliance history was 98% percent as of today August 17, 2023. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical assistance on the violation was reviewed and discussed with you. #858 In space #1 pull up in plastic bags were in the cubbie and less than 5 feet in height. A child two (2) years of age is enrolled in the space. Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed must be inaccessible to children under three years of age. You can keep these items on higher shelves (more than five feet off the ground) in classrooms used for care for children under three years of age. The items were removed during the visit.
