Upon arrival, I reviewed the purpose of the visit with Lead Teachers, Ms. Moses and Ms. Shaw. The Lead teachers assisted with the visit. Your program operates with a license issued on June 26, 2024. The follows restrictions were in compliance: daytime care, meets enhanced ratios, meets enhanced space and enhanced ratios minus 1 in each group. Two classrooms were monitored today. I observed children during indoor free play activities, bathroom/handwashing routines, teacher directed activities, and snack. Supervision and staff/child ratios in compliance. Staff had positive interaction with the children. One new staff file and one existing staff file was reviewed using the DPI verification form with no violations. A sampling of four children’s files were reviewed using the DPI verification form with no violations. The last monthly fire drill was practiced on 12/10/24 The last monthly playground inspection was completed on 12/19/24. The most recent lead water testing results were completed on 3/24/24. Lead testing must be completed every three years The most recent sanitation inspection for your facility was conducted on 5/17/24. A superior sanitation classification was issued with 8 demerits. The most recent fire inspection for your facility was conducted on 11/6/24. The facility was approved for day time care. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. NC Pre K requirements were in compliance. One violation was observed today: Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. An aerosol spray can of all purpose cleaner was on top of a cabinet in space # 3 .2820(b) The above violation was corrected during the visit. Compliance History As discussed, violations cited today and/or during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. Your program must maintain a compliance score of at least 75%. The center's compliance history was at 90% prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical Assistance: #840- An aerosol spray can of all purpose cleaner was on top of a cabinet in space # 3. All hazardous items with multiple warnings and items in aerosol containers must be kept in locked storage. Items labeled “keep out of reach of children” with no other warnings can be stored five (5) feet off the ground. The aerosol spray was placed in locked storage during the visit. I reminded you to check your classroom thoroughly each morning to ensure all hazardous products are stored properly. Remind cleaning staff to place all hazardous supplies back in locked storage after use. Consultation: The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. The permit restrictions were in compliance. Two classrooms and the outdoor gross motor area were monitored. Children in care were observed during snack, free play activities, a teacher directed activity, bathroom/handwashing, handwashing, and transition to lunch. Supervision and staff/child ratios were in compliance. Positive interaction was observed. A sanitation inspection was conducted by Environmental Health on March 30, 2023 with 15 demerits noted. One child had an Albuterol inhaler. The inhaler expired on 12/23. 10A NCAC 09 .0803(1)(d) The above uncorrected violations must be brought into compliance immediately. You shall submit a written, signed and dated statement or email to me at the address below detailing specifically how the above violation was corrected. The violation must be corrected no later than February 13, 2024. The correction letter must include the facility name and ID#. Failure to correct the violations and send the written statement by the due date listed above, noncompliance, may lead to an administrative action. Medications including prescription and non-prescription items must be stored in a locked cabinet or other locked container and may not be stored above food. Designated emergency medications may be stored out of reach of children, but are not required to be in locked storage. Non-prescription diaper creams and sunscreen may be kept out of reach of children when not in use, but are not required to be in locked storage. Teachers must keep any personal medications they bring into the facility in locked storage as well. I suggested you have staff keep any of their personal medication in the same locked storage areas as the children’s medications to ensure it is stored properly. The cabinet was locked during the visit. #843 One child and an Albuterol inhaler that expired 12/23. Teachers or school nurse should check all medication when they are brought into the facility and regularly thereafter to ensure they have not expired. Expired medication should be sent home immediately. Any medication the parent fails to retrieve within 72 hours of completion of treatment, or withdrawal of authorization, should be discarded. Medication should not be accepted if it is already expired. As discussed, violations cited today and/or during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. Your program must maintain a compliance score of at least 75%. Consultation or other topics discussed today (the following items reviewed were in compliance today): -we discussed your rated license. Your facility is in cohort 1. A prep year assessment has been ordered and your window is in place. I reminded you to have all staff check their WORKS accounts to ensure that their education has been updated. -Ms. Roberts needs to complete 5 hour of training before 2/8/24. -The Safe Arrival and Departure policy needs to be updated to reflect the facility’s method of documenting children’s arrival and departure. This needs to be specific to this program. -We discussed Action Plans. Be sure these forms and the medication associated with them are up to date. -I reminded you to recheck the incident report to ensure that all required information has been completed. -New staff must complete Heath & Safety training within one year. -We discussed substitutes. Health & Safety training must be completed after the 10th day as substitute work.
