The visit was conducted with you, Cynthia Goode, Program Coordinator. You provided me with applicable program, staff, and children’s records for review. The most recent annual compliance visit was conducted on April 14, 2025. This facility currently operates with a four-star rated license issued on October 17, 2025, meeting daytime care only, school age only, meets enhanced space, and meets enhanced ratios minus one. This facility has earned seven points in program standards, two points in staff education, and one quality point for having a staff benefits package and an infrastructure of parent involvement. The license was posted and the restrictions were in compliance. The program’s compliance history was 95 percent prior to today’s visit. The North Carolina Secretary of State website was viewed before the visit and Young Men’s Christian Association of Catawba Valley, Inc. was current/active as of March 25, 2026. A checklist was used to note the requirements I monitored today. Children and staff were observed during arrival, handwashing routines, snack, homework, and free play activities in the theatre. You stated you utilize the cafeteria, gymnasium, and theatre for indoor child care space. Files for one new staff member, one existing staff member, and applicable records for all staff were reviewed. All staff were listed on the ABCMS Provider Portal roster for this facility. Three children’s records were reviewed. The most recent monthly fire drill was conducted on March 23, 2026, at 3:38pm. The most recent quarterly lockdown/shelter-in-place drill was conducted on February 17, 2026, at 4:15pm. This facility will be closing for the summer. The last operational day for children and staff will be May 29, 2025. The violations observed today were discussed with you and documented in Visit Summary left with you at the conclusion of the visit. The following violations were observed: Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu posted for March did not include snack listed each day for March 18 through March 31, 2026. 10A NCAC 09 .0901(b) 1325 Parent's statement includes the child's name and date of enrollment and the date the parent signed the statement. The discipline policy parental acknowledgement form for one child enrolled on March 23, 2026, did not include the child's enrollment date. .1804(b) The violations documented above must be corrected immediately. On or before April 8, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. At least one dated copy of the current week's menu shall be posted where it can be seen by parents and food preparation staff when food is prepared or provided by the center. A variety of food shall be included in meals and snacks. Any substitution shall be of comparable food value and shall be recorded on the menu prior to the meal or snack being served. Children’s Records- -The Program Coordinator added the child’s enrollment date to the child’s application containing the parental acknowledgement for the discipline policy during the visit. I suggested that an area to include the child’s enrollment date be added to the child’s application and policy acknowledgement page. In the meantime, be sure that each child’s enrollment date is recorded on the application and policy acknowledgement form. Consultation: We discussed the following during the visit- -We discussed your Emergency Medical Care Plan. 10A NCAC 09 .0802(a) states the EMCP must include the name of the person and at least one alternate, responsible for carrying out the plan of action, ensuring that appropriate medical care is given. One person identified as the person or alternate responsible for carrying out the emergency medical care plan and ensuring that appropriate medical care is given shall be on the premises at all times and accompany children for off-premises activities. I suggested that you update your Emergency Medical Care Plan to remove a staff member who is longer employed at this facility. Cynthia Goode was the only current staff member listed on the Emergency Medical Care Plan. I suggested additional staff members be added to your plan to ensure there is someone on the premises at all times who is listed on your plan. The NC Secretary of State website was viewed before the visit and YMCA of Catawba Valley, Inc. was current/active as of December 13, 2023. A walk-through of licensed space was completed today. Staff and children were observed during an art activity and gross motor activities outdoors. The most recent sanitation inspection for your facility was conducted on August 31, 2023. A superior sanitation classification was issued with 11 demerits noted on the grade card. The most recent fire inspection for your facility was conducted on June 20, 2023. You stated two new staff have been employed since your most recent annual compliance visit conducted on May 25, 2023. The following violation of child care requirements was observed during today’s visit. The violation and technical assistance were thoroughly reviewed with you and documented in the Visit Summary sent to you at the conclusion of the visit. Individual cubicles, lockers, or coat hooks shall be provided for storage of coats, hats, bags, or other items and accessories.
