    Name of Operation: MOORESVILLE INTERMEDIATE BASP Facility ID: 49000414 Consultant: TONI WASHINGTON Operation Type: Center Case Number: Visit Date: 3/27/2024 Number Present: 0 Completed Date: 3/27/2024 Age: From 0 To 0 Total Minutes: 75 Time In: 01:00 PM Time Out: 02:15 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for a routine unannounced visit. You, Cindy Karriker, Program Director assisted me with the visit. Your program currently operates with a 4-star license. A letter was sent to the Program director and put in the facility file that the facility is closed due to enrollment. The official date of the no enrollment was January 29,2024. I talked to the Program director and the facility plans to be back in operation by or before May 1, 2024. Your last ACV was on September 25, 2023. I observed your Four-Star License, NC Child Care Summary of the Law, current menu, Sanitation placard, Emergency Medical Care Plan, daily schedule, activity plans posted as required. The Emergency Medical Care Plan needs to be updated to reflect current staff. I observed adequate approved space used during today’s visit. I observed permit restrictions maintained during today’s visit. I did not observe children because this facility is closed due to no enrollment. I observed a menu posted for the month of March. I observed manipulatives, dramatic play, art, and science. I observed toys and equipment to be of sufficient quantity for the use of children. I observed that all materials are developmentally appropriate in all the spaces and materials are in good repair. I observed no hazardous materials or equipment to be accessible to the children during today’s visit. There are currently no children on medications due to no enrollment. Your sanitation inspection was conducted on January 25, 2023, with a Superior classification and 0 demerits. This sanitation inspection is not in compliance. Your last Fire Inspection was conducted on September 20, 2023. I observed monthly Outdoor Inspections documented. I observed your last outdoor inspection was dated January 11, 2024. I observed your monthly Fire Drill Log, a drill was conducted on January 11, 2024. Fire Drills are to be conducted every month. This was the last drill before the facility closed for nonenrolment. I observed your Emergency Drill Log and the log was current and up to date. The last drill was a Lockdown, and it was conducted on November 15, 2023. This drill was current at the time of closure. I observed all staff Criminal Background Qualification letters (each staff was checked in the NC-ABCMS), CPR & First Aid trainings, BSAC trainings, and Recognizing & Responding to Suspicious of Child Maltreatment training to be current and in compliance as required. At the completion of the visit, this visit summary was printed, reviewed, and a copy was left with you. The following violation was observed and cited during my visit today. Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. Center needs a sanitation inspection The current on posted is dated January 25,2023. 10A NCAC 09 .0304(b) Today, the following Technical Assistance (TA) was provided: 1. Make sure a current Sanitation Inspection and Fire Inspection are sent to the consultant after each inspection. All violations cited must be corrected immediately. Please submit a written, signed, and dated statement to me at the email/address below detailing the steps taken to correct each violation. Your letter must include the following: 1. The name of your center 2. The center’s ID number 3. The date you write the letter 4. Address each violation stating how you corrected the violation and are now in compliance. 5. Address your plan to ensure that you will not have that violation again. 6. Your signature I advise you to be prepared to provide supporting documentation in the form of photos, documents, certificates, etc. should I request them in order to clarify or verify compliance. Please Note: If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. If you cannot meet the requirements by this date, you shall contact me with a timeline of the corrections. In some cases, this timeline may be extended. Failure to correct the violations and send the written statement by the due date listed above may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Compliance History: You are required to maintain compliance with all applicable child care rules and regulations at all times. North Carolina General Statute 110-90(4)(d) requires all child care facilities to maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action against the facility's license. Any violations cited during a visit may affect your score.
