MEYER Operation Type: Center Case Number: Visit Date: 5/4/2026 Number Present: 52 Completed Date: 5/4/2026 Age: From 4 To 5 Total Minutes: 120 Time In: 09:00 AM Time Out: 11:00 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for an unannounced routine visit. Upon arrival I was greeted by Ms. E. Kirby and I shared the reason for the visit. She assisted me with today’s visit. Your program currently operates with a Five Star (5) Center License. The license was posted, with restrictions to: first shift (1st) care, meets enhanced ratios, and space and meets reduced ratios. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven (97%) percent before today’s visit. The following items were monitored: Licensing Posted, Permit Restrictions, Staff/Child Ratio, Supervision, Specialized Trainings, Storage of Hazardous Substances and Storage of Medication, Program Records, and General Safety. The following items above were found in compliance. The following inspections are below. •Last fire drill was conducted on 4/17/26 •Last Shelter in Place/Lockdown Drill was conducted 3/5/26 •Last Playground Inspection was conducted on 4/15/26 •The fire inspection occurred on 3/9/25 and is in compliance •The last sanitation inspection occurred on 3/31/26 and received a superior classification. The facility uses Creative Curriculum and formative assessments. They offer several family events per year and have a policy of reporting for children who are chronically absent. The children were observed in all the rooms transitioning from AM snack, to handwashing then to free choice play in their rooms. Outdoor Learning Environment was observed for compliance and the mulch under the critical height structures needs to be turned and raked to prevent further compaction. Staff Records: The staff-training worksheets were during the visit. There were two (2) new staff files to monitor and both qualifying letters were verified. Medication: Medication was monitored and in compliance. Nutrition: The menu was not posted but was corrected during the visit, the allergy list was posted. For lunch the children were having chicken, green beans, pineapple, crackers and milk. There was also a peperoni and cheese option. Weapons: Today you reported that your facility was in compliance with childcare requirements regarding firearms. Transportation: Transportation is not provided. During my visit, I observed the remediation practices recommended by Clean Classrooms and the Union County Environmental Health department. Two (2) violations were observed today and both were corrected during the visit. The violations are as follows: Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu for the week was not current for the week. 10A NCAC 09 .0901(b) 813 Electric fans were not mounted out of reach or did not have a mesh guard to prevent access. In space F there was an electic fan on the floor, uncovered and in reach of children. It was not in operation. 10A NCAC 09 .0604(d) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent (75%). Any violation (s) documented may impact the compliance history score. The center's compliance history was reviewed with the operator. The compliance history could be impacted after today’s visit. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. Rated License •The facility received their five (5) star center license in 2025 and is due for their re-permitting in May of 2028. Technical Assistance •The Healthy Social Behaviors Project offers coaching for child care providers who would like support managing challenging classroom behaviors. The goal is to empower teachers to create learning environments that promote pro-social skills, like self-control and problem solving. Support is available by phone (888-600-1685, Option 1), email, or through an online form. •Trainings Available from Birth-Three Quality Initiative Explore the many live and on-demand trainings from Birth-Three Quality Initiative. Courses include Nurturing Little Minds, Applying NCFELD to Infant-Toddler Planning, and Building Relationships with Infants and Toddlers •As a reminder, all staff personal belongings should be kept out of the reach of children. This includes bags and purses. •Ms. Kirby and I discussed a child who recently was taken to the Emergency Room (ER) for a medical emergency while in the care of the facility. She reported the child had returned with no restrictions. We discussed a medical narrative for the child’s file from the medical professional stating there are no restrictions for the child. •Ms. Kirby and I discussed a staff member who has had two (2) medical events over the school year where she briefly loses awareness of her surroundings. The staff member does have a health questionnaire on file from this school year certifying emotional and physical fitness to care for children. It also states “As the director, I understand that I may request another evaluation of a staff’s members emotional and physical fitness to care for children when there is a reason to believe that there has been deterioration in the staff member’s emotional or physical fitness to care for children. This request may be made based upon factors such as observations, of myself or other staff members, reports of concerns from family, reports from law enforcement, or reports from medical personal”. Upon arrival I was greeted by Ms. E. Kirby and I shared the reason for the visit. She assisted me with today’s visit. Your program currently operates with a five (5) star center license. The license was posted, with restrictions to first (1st) shift care, meets enhanced ratios, meets enhanced space and meets reduced ratios. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven (97%) percent as of today’s visit. The following items were monitored: Licensing Posted, Permit Restrictions, Staff/Child Ratio, Supervision, Specialized Trainings, Storage of Hazardous Substances and Storage of Medication, Program Records, and General Safety. Fire and Sanitation Inspections have not occurred since the last visit and remain current. The following items above were found in compliance. The following inspections are below. •Last fire drill was conducted on 4/30/25. •Last Shelter in Place/Lockdown Drill was conducted 3/5/25. •Last Playground Inspection was conducted on 4/29/25. The children were observed in the rooms serving pre-school aged children were transitioning from handwashing to breakfast, participating in morning meetings and welcome activities, and practicing end of year songs and program. Outdoor Learning Environment was observed to be in compliance. Staff Records: The staff-training worksheets were completed before the visit. There was one (1) new staff files to monitor. The facility has an active plan for children who are absent for more than three (3) days. Parent involvement is being met but parent/guardian-teacher end of year conferences and a Pre K Prom. The facility has been approved by the North Carolina Pre K, for three (3) classrooms with twenty (20) children in each room. During the visit, I measured two (2) rooms which could be added increasing the facilities NC Pre K at Weekday to four (4) classrooms. Ms. Kirby and I reviewed the space and capacity of each room. Moving forward, I asked Ms. Kirby to formally request her capacity to increase and would share with her the new capacity numbers in each room measured within five (5) business days. Medication: Medication was monitored and two forms were found to be out of compliance. Nutrition: The menu and allergy listings were posted and for breakfast the children were having shredded whole grain wheat squares, pineapple and milk. Weapons: Today you reported that your facility was in compliance with childcare requirements regarding firearms. Transportation: Transportation is not provided. One (1) violation was observed today. I used the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable childcare requirements pertinent to this facility and the Annual Compliance Monitoring Checklist for Child Care Centers. I monitored the following items today: supervision, staff/child ratio, staff, health, safety, and program records. Program Records: We reviewed all the required records and were in compliance. Inspections: All inspections were monitored, and inspection dates are listed below. •Fire drill was conducted on 11/24/24. •Emergency drill was conducted on 9/27/24. •Fire inspection has been updated. •Playground inspection was completed on 11/15/24. •Sanitation inspection was conducted on 5/22/24 and received a Superior Classification. •EPR manual was updated and reviewed on 8/24/24. •Incident logs and sheets were completed and up to date. Indoor Learning Environment: I observed children in the indoor learning environment and the following items were posted in the classrooms that were monitored: Daily schedules, activity plans, staff/child ratio worksheets, emergency numbers, emergency procedures, arrival/departure procedures, summary of law and menus. Children were participating in self-directed free choice center activities, gross motor activities, and watching a developmentally appropriate themed and time length video about social skills before transitioning to outdoor learning time. Outdoor Learning Environment: The outdoor learning environments were monitored and in compliance. The playground is on a slope and the protective surfacing will slide to the lower end of the playground. The protective surface around the critical height climbing structure should be tilled and raked for compaction but is within compliance. Staff Records: The staff-training worksheet was completed prior to the visit. There was one (1) new staff files to review today, and one (1) existing staff file. Please refer to the staff/training worksheet to review which files were monitored. Children’s Records: There are fifty-four (54) children enrolled in this center. There were forty-eight (48) in attendance today. Six (6) children’s files were reviewed today, please refer to the children’s worksheet to review which files was monitored. Medication: All emergency and other medications were reviewed today and were in compliance. Nutrition: The facility was in compliance during today’s visit with Child Care Meal Patterns Requirements. The menu and allergy list were posted and for lunch the children were having pears, milk biscuits, sausage egg and cheese casserole, and milk with a vegan sausage option. Weapons: Your facility reported that they were in compliance during today’s visit with Child Care Requirements regarding firearms. Transportation: The facility reported they do not provide transportation. There was one (1) violation observed during the visit. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. o Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. Technical Assistance •Resource Center (SSRC) is funded by and in partnership with the Alliance for Children and is hosted by South Piedmont Community College. The SSRC offers a variety of materials to support children ages 0-5 in their learning and development. The SSRC is a lending library of more than 5,000 resources with a delivery service. Family child care homes are required to test water for lead once. •Licensing fees will be available to view on November, 30th 2024 and will be due by the end of December of 2024. •G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. o The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php? id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. o Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Upon arrival I was greeted by E. Kirby I shared the reason for the visit. Ms. Kirby accompanied me throughout the walk-through of the facility and the outdoor learning environments. Your program currently operates with a Five-Star Center License effective 6/13/19. The license was posted, with restrictions to 1st shift, meets enhanced ratios, meets enhanced space, and meets enhanced ratios minus one in each group. Ownership: The facility owner is Central United Methodist Church of Monroe, Inc. and SSDI #0724448 If any changes to the ownership need to be made, you must notify your consultant at least thirty (30) days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. NCPRE K Requirements: Program Records: The NC Pre-K Site Monitoring Tool for the 2023/2024 school year was on site and available for review as well as the Brigance screening tool. The official NC Pre-K Day, facility requirements, nutrition, curriculum including developmental screenings and instructional assessments, staff to child ratio and class sizes, family engagement, and scheduled were reviewed. Indoor Learning Environment: There are three (3) spaces of the eleven (11) approved spaces in operation currently. I observed all spaces being used by children today. The facility has reached out to add more children to the NC-Pre K program to add to their licensed rooms. I used the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable childcare requirements pertinent to this facility and the Annual Compliance Monitoring Checklist for Child Care Centers. I monitored the following items today: supervision, staff/child ratio, staff, health, safety, and program records. All classroom spaces, the kitchen, restrooms, and both playgrounds. Inspections: All inspections were monitored, and inspection dates are listed below. •Fire drill was conducted on 11/28/23 •Emergency drill was conducted on 9/20/23. •Fire inspection was completed on 3/15/23. •Playground inspection was completed on 11/20/23. •Sanitation inspection was completed on 2/8/2023 with a Superior rating. Indoor Learning Environment: I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children were preparing for breakfast by washing hands and toileting, then eating breakfast which consisted of tropical fruit mix, biscuits, and milk that was in compliance with the menu. They were participating in a “Welcome/Good Morning” song and activity and preparing for outside time. All learning centers were well equipped with learning activities, a variety of supplies and were overall provided all the needed tools for the children's learning. Outdoor Learning Environment: The outdoor learning environments were monitored and out of compliance. The breezeway are consisted of a gross motor, art, fine motor areas and a small garden. The larger playground’s equipment was in good repair but the maintenance staff was in the middle of upgrading the bottom of the fence to be in compliance. While the depth of the bottom of the fence has openings that are not less than three and one half (3 and ½) inches or greater than nine (9) inches There were also protrusions with rugged and pointed end that could be a hazard. While most of the playground is finished with an appropriate fix, there is an area near the gate that falls out of compliance. Program Records: We reviewed all the required records and were in compliance. Staff Records: The staff-training worksheet was completed prior to the visit. There were six (6) new staff files to review today, and two (2) existing staff file. Please refer to the staff/training worksheet to review which files were monitored. The staff and training worksheet and the folders were in good order and very organized. All files were in compliance. Children’s Records: There are fifty-one (51) children enrolled in this center. There were forty-four (44) in attendance today. Five (5) children’s files were reviewed today, please refer to the children’s worksheet to review which files was monitored. All information was in compliance with dental and vision screenings. Medication: All emergency and other medications were reviewed today and were in compliance. Nutrition: The facility was in/out of compliance during today’s visit with Child Care Meal Patterns Requirements. The menu was posted and all substitutions for dietary restrictions were also posted. Weapons: Your facility reported that they were in of compliance during today’s visit with Child Care Requirements regarding firearms. Transportation: One (1) vehicle were monitored today, The fifteen (15) passenger van had recently been inspected and was current with insurance, registration, no-smoking sign and children’s information and pictures. The facility has a larger bus that seats more than sixteen passengers, has a NC DOT# and was off site today. One (1) violation was observed today The violation is as follows: Violation Number Comment Rule 714 Openings in equipment, steps, decks, handrails, and fencing were not less than 3 1/2 inches or greater than 9 inches. If sufficient information is not received by the due date, a follow-up visit may be completed. The program’s compliance history was ninety seven percent (97%) prior to today’s visit. The compliance history could be impacted after today’s visit. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. Rated License: Weekday Children’s Ministries’ rated license cohort is cohort # two (2). The preparation year of the assessment is July 1st, 2024 to June 30th, 2025 and the assessment year is July 1st, 2025 to June 30th, 2026. Listed below are some items to do to get ready for the upcoming preparation and assessment years. •Have all staff update their information in the WORKS database. For more information, please go to https://ncchildcare.ncdhhs.gov/Services/DCDEE-WORKS. Have all teachers and staff print out their WORKS letter, so you can see if their education is updated. If any teachers have a BSAC certificate have them upload the certificate into their account. •Access the resources for your upcoming assessment at www.ncrlap.org. •Review the materials lists for each classroom. This list is to be used as a resource reminding you of some of the required items to promote quality in programming. •Contact CCRI and The Alliance for Children to have them assist you in providing guidance in preparation for the scales. Technical Assistance: •One staff member had all the required orientation hours within the one (1) week and six (6) time frame to be in compliance but is missing a one (1) hour training about medication.
