Upon arrival I was greeted by Ms. K. Eldin I shared the reason for the visit. She assisted me with today’s visit. Your program currently operates with a Four (4) Star Family Child Care Home permit. The license was posted, with restrictions to: first (1st), second (2nd), and third (3rd) shift care, children in ground level care only and no more than five (5) preschool aged children at a time. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven (97%) percent before today’s visit. The following items were monitored: Licensing Posted, Permit Restrictions, Storage of Hazardous Substances and General Safety. Fire and Sanitation Inspections are not required for this licensed facility. The facility has completed the water and paint testing. The following items above were found out of compliance. The following inspections are below. •Last fire drill was conducted during the first of the month, and the form to document was left with Ms. Eldin The Indoor learning environment was not monitored today as there are no children currently present. The area is still in use as there are two (2) children enrolled. They are currently on vacation and not present during today’s visit. Outdoor Learning Environment-what was observed to be in compliance. Medication: Medication was not monitored. Weapons: Today you reported that your facility was in compliance with childcare requirements regarding firearms. Transportation: Transportation is not provided. There was one violation observed during today’s visit and it was corrected. The violations are as follows: Violation Number Comment Rule 923 Operator did not keep records of monthly fire drills, quarterly lockdown or shelter-in-place drills, incident reports, incident logs, accurate attendance, emergency numbers, emergency preparedness and response plan, field trip/transportation permission, pet vaccinations records and/or playground inspections monthly outdoor safety checks for a minimum of one year. Ms. Eldin did not have a copy of monthly fire drills, or emergency drills. .1721 (e)(7) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent (75%). Any violation (s) documented may impact the compliance history score. The center's compliance history was reviewed with the operator. The compliance history could be impacted after today’s visit. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. Rated License Updated Child Care Rules are located in Section .3200 pertaining to QRIS system, otherwise known as Pathways to Stars are available for review. The process will begin with these informational opportunities. •In September, childcare consultants hosted in-person facility operator/administrator meetings within the counties they serve to provide additional guidance on the changes. At Ms. Eldin’s annual compliance visit, we discussed if she would like to maintain a two (2) to five (5) Star permit or not. We discussed changes in her care schedule and caregivers. During today’s visit, Ms. Eldin has decided she does not want to pursue an enhanced license. A reduction of stars will be processed. Technical Assistance •We discussed Ms. Eldin only caring for her two (2) grandchildren and this falls into childcare rules pertaining to licensing requirements. If Ms. Eldin decides to only care for her two (2) grandchildren, she may choose to operate without a Star Rated permit, she may also care for up to two (2) unrelated children at that time as well. Ms. Eldin and I reviewed the steps she would need to take if she terminated her permit/license and wanted to care for more than two (2) unrelated children in the future. •We discussed trainings and postings required to maintain a one (1) star license. We jointly reviewed the visit summary and documentation of my findings and violations with you, I encouraged you to ask any questions and you stated that you had none. Upon arrival I was greeted by Mr. M. Eldin and Ms. K. Eldin I shared the reason for the visit. She assisted me during today’s visit. Your program currently operates with a four (4) star Family Childcare License. The license was posted with the restrictions to: first (1st) and second (2nd) shift care, maximum of five (5) preschool children at any time, fireplace/woodstove not used during operating hours, serves no more than two infant under one (1) years of age and does not provide transportation for children in her care. Ownership: If any changes to the ownership need to be made, you must notify your consultant at least thirty (30) days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. The compliance history is ninety-six (96%) percent prior to the visit and this information was reviewed with the family child care homes operator. The following items were monitored: Licensing Posted, Permit Restrictions, Staff/Child Ratio, Supervision, Specialized Trainings, Emergency Medical Care Plan, Storage of Hazardous Substances and Storage of Medication, Program Records, and General Safety. The following items above were found out of compliance. The following inspections are below. •Last fire drill was conducted in June of 2025. •Last Shelter in Place/Lockdown Drill was conducted in December 2024. •Last Playground Inspection was conducted in February 2025. The children have access to a play area designed for childcare, restroom, and the outdoor learning environment. Indoor Learning Environment: The children were playing with Lego type blocks and personal care activities. Outdoor Learning Environment: The outdoor area was in compliance and there was no standing water. Program Records: I reviewed all the required records. Nutrition: The facility was in compliance during today’s visit with Child Care Meal Patterns Requirements. Medication: No medication is administered. Transportation: Transportation is not provided. Weapons: Your facility reported that they were in compliance during today’s visit with Child Care Requirements regarding firearms. Two (2) violations were observed today. The violations are as follows: Violation Number Comment Rule 923 Operator did not keep records of monthly fire drills, quarterly lockdown or shelter-in-place drills, incident reports, incident logs, accurate attendance, emergency numbers, emergency preparedness and response plan, field trip/transportation permission, pet vaccinations records and/or playground inspections monthly outdoor safety checks for a minimum of one year. The last playground inspection was completed in February. .1721 (e)(7) 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete.
