Name of Operation: MOLLY'S IN HOME DAYCARE Facility ID: 60004162 Consultant: ABIGAIL AVALOS Operation Type: Family CC Home Case Number: Visit Date: 3/12/2024 Number Present: 5 Completed Date: 3/12/2024 Age: From 0 To 5 Total Minutes: 68 Time In: 01:02 PM Time Out: 02:10 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for a routine unannounced visit. Upon arrival I was greeted by Ms. Molly Brietz, Owner. I shared the reason for the visit. Your program currently operates with a Four-Star Center License effective 11/18/22. The license was posted, with the following restrictions: - 1st Shift - Children in care on ground level only - Maximum of 5 preschool children at any time - Serves no more than two infants under one year of age I monitored the following items: supervision, staff/child ratio, staff, health, safety, and program records. I monitored the childcare room, bathroom, kitchen, and outdoor learning environment. Ownership: This family child care center is owned by you, Ms. Molly Brietz, the individual owner and you do reside in the home. Inspections: All inspections were completed and in compliance. Indoor Learning Environment: I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children were participating in nap/quiet time. As a reminder, children who are sleeping or napping in another room cannot be behind a closed door. Outdoor Learning Environment: The outdoor learning environment was monitored, I observed a storage area with the door ajar and the key on the lock. Inside the storage area were hazards items such as bleach, paint, pest spray, batteries and other tools, this was corrected during the visit. Program Records: I reviewed all the required records. The last fire drill was conducted on 2/23/24 and the last emergency drill was conducted on 2/20/24. Please ensure to update your Emergency Preparedness Plan (EPR) to reflect all current information regarding your enrollment, contact numbers and any updates to your emergency plans. This will be reviewed at your annual compliance visit. You shared that updated vaccination records are completed for all pets, please make sure to have their information on file. Staff Records: We discussed that five (5) on-going training hours need to be completed before your annual compliance year is over. Please ensure to document these in your training log and have the certificates available and on file for review on or before 9/21/24. As a reminder, CPR/First Aid trainings do not count towards on-going training hours. Nutrition: The facility was in compliance with child care meal pattern requirements. Medication: No medication is administered. I observed a diaper cream with an expiration date of 5/2023, please have the family bring a new diaper cream and ensure that the ointment is not expired. Transportation: Transportation is not being provided. Weapons: The facility was in compliance with child care requirements regarding firearms. Two (2) violations were documented, one (1) was corrected during the visit. Violation Number Comment Rule 702 Potentially hazardous items, including but not limited to, power tools, nails, chemicals, propane stoves, lawn mowers, and gasoline or kerosene, whether or not intended for use by children were not stored in locked areas, removed from the premises, or otherwise inaccessible to children. I observed a storage area with the door ajar and the key on the lock. Inside the storage area were hazards items such as bleach, paint, pest spray, batteries and other tools. .1719 (a) (1) 1403 Prescription or over-the-counter medication and topical, non-medical ointment, repellant, lotion, cream, fluoridated toothpaste, or powder was administered to any child without the required information. I observed a diaper cream with an expiration date of 5/2023. 10A NCAC 09 .1720(c)(1&2) Corrective Action Plan: The violation not corrected during the visit must be corrected immediately. On or before 3/26/24, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. Please email the information to: abigail.avalos@dhhs.nc.gov a compliance letter template was shared with you please ensure that all these components are completed: • Name of your facility • Your facility ID number • Date the visit made • Date that you are submitting the compliance letter • Provide each cited violation number • Describe when and how each violation was corrected • Describe how you will prevent the reoccurrence of each violation (you need to specify how you will prevent the violation from re-occurring) • Sign with your signature Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. Compliance History: The program’s compliance history was ninety-seven percent (97%) prior to today’s visit. Rated License: This facility is in Cohort #3. Your preparation year is from 7/1/25 to 6/30/26. Your reassessment year will be from 7/1/26 to 6/30/27. More information will be shared with you as we get closer to your prep year date, if you decide to have the rated license assessment completed for your FCCH please let me know. Technical Assistance: Please ensure that all annual documents are being updated and completed. These items will be monitored for completion at your annual compliance visit: - Staff Development Plan - Annual Staff Evaluation - Medical Forms (Health Questionnaire/Emergency Info) - Polices (EMC, EPR, and any other policies that are amended) Resources and Reminders: We discussed that you will need to enroll your family child care home in the Clean Classrooms for Carolina Kids program to identify and eliminate exposure to lead and asbestos hazards in building infrastructure.
