This visit was conducted with Betty Faw, owner/operator. Your program currently operates with one (1) Star license effective 7/24/15. Restrictions include 1st shift, daytime care only; maximum of five (5) preschool children at any time; Dogs shall not be accessible to children in care. There are currently two (2) pets in the home, one (1) dog and one (1) cat. No dogs were accessible to the children today. The license was observed, and the restrictions were found in compliance. Ms. Faw is the sole proprietor of this family childcare home. The program’s compliance history was 97% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the home was completed today. Indoor and outdoor areas were observed. Upon arrival, capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having movement and dancing as I arrived and free choice center play. I observed that developmentally appropriate materials and activities were available to the children in the licensed space. The last annual compliance visit was conducted on 5/1/25. A Verification of Required Information for Operator and Additional Caregivers form was sent to the provider prior to the visit. A FCCH and Centers Located in a Residence Operator’s Statement of Responsibility form was sent to the provider prior to the visit and reviewed and signed during the visit. We discussed owner status of this facility. Ms. Faw stated the only people living in the home with her is her son. Be reminded that you and your husband, and any child sixteen years of age and older must have criminal record checks conducted every five (5) years. You may go to the Division of Child Development & Early Education's website at https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. Program records and required postings were monitored. A fire drill was conducted on 2/13/26 at 1:00 pm with fifty-five (55) seconds to evacuate by B. Faw. A lockdown emergency drill was documented on 3/17/26 at 10:000 am with twenty (20) seconds to lockdown by B. Faw. An outdoor inspection was documented on 3/17/26 by B. Faw._____________. There are two (2) pets inside the home and all vaccinations, and parent acknowledgement forms were monitored. The staff file and one (1) children’s records were monitored per DCDEE procedures. Storage of hazardous items was monitored today. No medications were observed nor reported. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 1741 Prior to enrollment of children in a Family Child Care Home, and before animals come into the Family Child Care Home, the operator did not obtain each parent's signature on a form acknowledging the type of animal located on the premises and where the animal will be kept during child care operating hours and /or the signed form was not maintained in the child's file. Ms. Faw has obtained a new dog and the animal acknowledgement form has not been updated in the children's files. Ms. Faw stated she has had the dog two (2) months. 10 NCAC 09 .1719(b)(3) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send documentation verifying compliance on or before Monday, April 6, 2026. Today we discussed that your First Aid and CPR training is due again in April 2026. We discussed today that you have until May 1, 2026 to get your annual trainings complete. • On-Going Professional Development In addition to the required Health and Safety Trainings, staff can access professional development training on the DCDEE website in Moodle under Early Childhood Professional Development in the categories of Child Care and Development Fund (CCDF), Orientation and Regulatory. Updated training modules are being added, including a Child Care Rule Rollout with new rules effective July 1, 2025, found under the Regulatory tab. Specifically, under the Orientation tab, I would encourage all staff to complete the training for “Child Development Module.” This training module provides a basic overview about child development and is designed to help early educators support infants, toddlers, and preschool children to reach their next developmental milestone. In addition to gaining introductory knowledge about child development, early educators will learn about the many resources and professional agencies available in North Carolina to help with promoting a responsive and engaging learning environment to support all children served. Upon completion, participants will receive one (1) contact hour credit (CHC). An individual NCID username and password is needed to access the DCDEE Moodle. Indoor and outdoor areas were observed. We tested the smoke detector during today’s visit, and it worked today. Capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children having free choice play in the play area, having dance & movement and preparing for lunch. I observed materials and activities that were available to the children in the classrooms. The last annual compliance visit was conducted on 6/5/24. A Verification of Required Information for Operator and Additional Caregivers form was completed during the visit for the operator and additional caregiver. A FCCH and Centers Located in a Residence Operator’s Statement of Responsibility form was reviewed and signed during the visit. We discussed the owner status of this facility. Individuals living in the home were discussed and are recorded on the Verification of Required Information for Operator and Additional Caregiver form. This change has been in effect since February 2024. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new childcare providers working who were hired or moved into the childcare facility within five business days. A FCCH and Centers Located in a Residence Operator’s Statement of Responsibility form was also emailed to Ms. Faw on 5/8/24 and sent again on 6/3/24. It was reviewed and signed during the visit. We discussed owner status of this facility. Ms. Faw stated the only person living in the home with her was her son. A fire drill was recorded 9/26/23 There was no fire drill for October. A shelter in place drill for the facility was recorded 8/16/23. There are two (2) pets inside the home and all vaccinations and parent acknowledgement forms were monitored. One expires 2/20/25 and the other expires 1/9/26.
