This visit was conducted with Gatha Bass, Administrator. This program currently operates with a FIVE (5) Star license effective 11/1/24. Restrictions include 1st shift, Daytime Care: Meets enhanced space. The NC Secretary of State website was reviewed on 2/23/26 and KinderCare Education LLC was listed as current and active. The program’s compliance history was 93% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children having free choice center play, outdoor gross motor play, and having lunch. No new staff were reported at this program. No files for new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. Storage and administration of medication were monitored. Medication authorization was monitored. Diaper creams and ointments were the only medications reported today. Storage hazardous items were monitored today. A playground inspection was recorded 1/29/26 at 4:20 pm by G. Bass. A fire drill was recorded 1/29/26 at 11:00 am with two (2) minutes to evacuate thirty-five (35) children and nine (9) adults by G. Bass. A lockdown emergency drill for the facility was recorded 12/2/25 at 4:00 pm with two (2) minutes four (4) seconds to lockdown by G. Bass. The most recent fire inspection was on 5/6/25 by N. Hamby. I observed a sanitation inspection that was last conducted on 7/22/25 earning a Superior classification with five (5) demerits. The program does provide transportation. Today I monitored a 2019 Chevrolet commercial private minibus # 19085. This minibus is insured as KinderCare Education LLC in Wilkes County by Arch Insurance Company 12/1/25 expiring on 12/1/26 with policy number 71CAB1045716. The license plate JN9412 expires on 5/31/26. Today I observed the KinderCare Supervision Record with on/off, name to face times listed of children being transported. I also observed the KinderCare transportation permission form for children and the KinderCare enrollment agreement for each child being transported. The following violation was cited during today’s visit: Violation Number Comment Rule 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. Today I observed a rubber glove, a broken cup, paper and pieces of broken toys on the sidewalk going out to the large playground. A trash can was within reach in this area. 10A NCAC 09 .0604(p) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before Tuesday, March 10, 2025. We discussed that you completed the Accreditation requirements to meet 5 Stars and received your new permit effective 11/1/24. Your certificate is valid until July 1, 2029. Today you were given three (3) accreditation documents to complete since you went through the accreditation prior to the QRIS being implemented. I received these forms at the conclusion of today's visit. Specifically, under the Orientation tab, I would encourage all staff to complete the training for “Child Development Module.” This training module provides a basic overview about child development and is designed to help early educators support infants, toddlers, and preschool children to reach their next developmental milestone. In addition to gaining introductory knowledge about child development, early educators will learn about the many resources and professional agencies available in North Carolina to help with promoting a responsive and engaging learning environment to support all children served. Restrictions include 1st shift, Daytime Care: Meets enhanced space. The license permit was observed, and the restrictions were found in compliance. The NC Secretary of State was reviewed on 8/4/25 and KinderCare Education LLC is current & active. The program’s compliance history was 94% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children having free choice center time, outdoor gross motor play, and infants having individual needs met. I observed materials and activities that were available to the children in the classrooms. Your facility uses the KinderCare Education Curriculum. The last annual compliance visit was conducted on 9/10/24. A sanitation inspection was completed on 7/22/25 with a Superior classification and five (5) demerits. The last fire inspection was conducted 5/6/25 by Niki Hamby. Program records and required postings were monitored. A fire drill was conducted on 7/31/25 at 4:00 pm with one (1) minute, fifty-five (55) seconds to evacuate forty (40) children and ten (10) adults by G. Bass. A lockdown emergency drill was documented on 6/6/25 at 9:30 am with two (2) minutes and forty (40) seconds to lockdown by G. Bass. An outdoor inspection was documented on 7/31/25 at 3:15 pm by G. Bass. One (1) new staff file, two (2) regular staff and six (6) children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Hazardous items were stored in locked closets. Storage and administration of medication were monitored. Medication authorization was monitored. The program does not provide transportation during the summer. The most recent fire inspection was received on the last annual compliance visit. I observed a sanitation inspection was last conducted on 1/2/25 earning a Superior classification and ten (10) demerits. You stated that you do provide transportation in the pm for nine (9) children using one (1) Chevrolet light duty bus, numbered 19085. Bus number 19101 is not in use currently. During today’s visit, I monitored emergency information, permission to transport forms for all children being transported, and the bus log. The vehicle was clean, in good repair, had working seatbelts, had adequate tire tread, and were current on inspection and insurance with Arch Insurance Company and is valid through 5/31/25. This visit was conducted by Karen Young, Childcare Consultant with Gatha Bass, Director. Your program currently operates with a THREE (3) Star license effective 5/16/22. Restrictions include 1st shift, Daytime Care: Meets enhanced space. The license was observed, and the restrictions were found in compliance. The NC Secretary of State website was reviewed on 9/9/24 and KinderCare Education LLC was listed as current and active. The program’s compliance history was 94% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children drawing at the table, preparing for outdoor play and having gross motor play on the playground. I observed developmentally appropriate materials and activities were available to the children in the classrooms. The last annual compliance visit was conducted on 10/6/23. A sanitation inspection was completed 7/19/24 with a Superior classification with nine (9) demerits. The last fire inspection was conducted 5/2/24 and a copy of this inspection was received today. Program records and required postings were monitored. A fire drill was conducted on 8/30/24 @ 2:15 pm with two (2) minutes one (1) second to evacuate forty-two (42) children and ten (10) adults by G. Bass. An emergency Lockdown drill was documented on 6/3/24 at 10:15 am with two (2) minutes five (5) seconds to lockdown by G Bass. An outdoor inspection was documented on 8/30/24 with mulch needed noted on teh inspection form. Three (3) staff files ,including one (1) new staff and five (5) children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. You stated that you do provide transportation in the pm for 10 children using two (2) Chevrolet light duty buses, numbered 19101 and 19085. During today’s visit, I monitored emergency information, permission to transport forms for all children being transported, and the bus log. I observed first aid kits and fire extinguishers on the vehicle. I monitored two (2) vehicles today. I observed two 2019 Chevrolet light duty buses. Tag numbers are JN-9412 for bus 19085 and JN-9432 for bus 19101. The vehicles were clean, in good repair, had working seatbelts, had adequate tire tread, and were current on inspection and insurance with Arch Insurance Company. Your next inspection date on this vehicle is due 3/23/25 and the insurance is valid through 5/2025 . The following violations were cited during today’s visit: Violation Number Comment Rule 847 Parent's medication authorization did not include required information. I observed in the infant room,Space 1, there was a medication permission to administer form that had no amount listed in the blank. I observed the mulch surrounding the two (2) permanent dinosaur structures on the large preschool playground had only one (1) inch of mulch in the fall zone around the dinosaur structures. .0605(k)(1-4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Technical assistance provided during today’s visit: • Mulch is to measure at least six (6) inches in fall zones where equipment is permanently installed to keep children safe if a fall occurs. See child care rule 10A NCAC 09 .0605(c)(1-4). Ms. Bass stated that she has requested mulch be delivered to her KinderCare center but none has been delivered to her as of today. • Parents are to fill in all information on the Permission to administer forms for over-the-counter medications to ensure medications are administered as directed and as prescribed. See child care rule 10A NCAC 09 .0803(4)&(6-9). • Incident Logs are to be kept current with the incident report information to ensure proper documentation of children’s incidents for future reference. See child care rule 10A NCAC 09 .0802(g)(1-6). Ms. Bass completed the Incident Log during the visit today. She stated she is trying to get organized again now after the school year has recently begun. Consultation during today’s visit included: • You must notify your consultant 30 days prior to changing the ownership status of your facility. • General cleaning and organizing is needed throughout the center. • I recommend the use of a “clean” and a “dirty” laundry basket to ensure the dirty laundry is kept separate from the clean laundry. • There is a new Summary of the Law that was updated 2023. You may go to the DCDEE website , under Provider > forms and documents > then scroll down to Summary of the Law to print off the latest copy and place in your center. * The cohorts are mo more due to Senate Bill 245. This visit was conducted by Karen Young, Childcare Consultant, with Gathie Bass, Administrator. Your program currently operates with a Three (3) Star license effective 5/16/22. Restrictions include 1st shift daytime care; meets enhanced space. The NC Secretary of State website was reviewed on 3/5/24 and KinderCare Education, LLC was listed as current and active. The program’s compliance history was 94% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Books in the four year old classroom were in bad repair. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children having gross motor play outside, and free choice time. Wilkes County Schools had a teacher workday today, so school age children were present during the visit today. One (1) new staff were reported at this program. Files for new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. No prescription medications were observed nor reported. Over the counter medications were monitored today. Storage hazardous items was monitored today. A playground inspection was recorded 2/29/24 at 5:00 pm by G. Bass. A fire drill was recorded 2/29/24 at 4 :00 pm with one (1) minute thirty (30) seconds to evacuate sixty-five (65) children and twelve (12) adults by G. Bass. A lockdown drill for the facility was recorded 11/30/23 at 10:30 am with 2.5 seconds to lockdown by G. Bass. There was no documentation of an emergency drill within three (3) months, which should have occurred in February of 2024. The most recent fire inspection was on 5/24/23 by N. Hamby and I received a copy of this inspection on the last Annual Compliance visit. I observed a sanitation inspection was last conducted on 1/31/24 earning a Superior classification and six (6) demerits. The program does provide transportation. Today I observed two (2) minibuses on property currently being used. One minibus, license JN-9432 is driven by G. Bass with three (3) school age children picked up from two (2) local elementary schools. The other minibus, license JN-9412, is driven by F. Winters. Both minibuses are due for tag renewal in May 2024. Both buses are insured through Arch Insurance Company. The following violation was cited at today’s visit. Violation Number Comment Rule 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last emergency drill was documented 11/30/23. There was no documentation of an emergency drill within three (3) months, which should have been in February 2024. .0604(u);.0302(d)(8) You must correct the violation cited during today's visit immediately and send me documentation verifying compliance on or before Monday, March 19, 2024. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. Technical assistance discussed during today’s visit included: We discussed that emergency drills must be completed at least every three months, regardless of the year's quarter you are in. Refer to Child Care Rule 10A NCAC .09 .0604(u) and .0302(d)(8). Additional Information/Consultation: • You must notify your consultant 30 days prior to changing the ownership status of your facility. • You may reach out to your local Wilkes County Partnership to schedule mock assessments as it gets closer to your license reassessment time. • During today’s visit we discussed the need for help with behavioral issues. Ms. Bass stated she has reached out the Wilkes Partnership for assistance, however, she has had no assistance since November of 2023. * Books should be replaced when they become torn and worn. Ms. Bass stated she has new books on order to replace worn books in the four year old classroom. * Both minibuses need to be swept out and trash removed. Ms. Bass stated she will sweep and remove trash from both buses today. At the completion of the visit, this visit summary was reviewed with and provided to you.
