Currently this facility operates with a Five (5) Star license that was issued January 23rd , 2020. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety six percent (96%) as of 9/16/25. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. I monitored your program for compliance with applicable child care requirements including compliance with requirements located in Child Care Rule Section .3000 in classroom 200, where children participating in the NC Pre-K program are cared for. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Ms. K. Ceparano and Ms. J. Sorrell assisted me with today’s visit. Children throughout the facility were participating in group time, free play in activity areas, transitions, and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. The last sanitation inspection was 5/9/25. The last fire inspection was 9/9/25. The last fire drill was conducted on 8/28/25. Playground inspection was conducted on 8/22/25. Three violations were cited during today’s visit. One was corrected during today’s visit. Technical assistance was given to the provider to ensure compliance. Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. On child's epi pen is expired, and it was not discarded or sent home to the parent within 72 hours. .0803(12) 1757 A valid qualification letter was not on file and available to review at the facility. A valid qualification letter was not on file for J. Sorrell. G.S. 110-90.2(b) & (d) & .2703(e) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. For all children with a history of anaphylaxis (severe allergic reaction), or for those with peanut and/or tree nut allergy (whether or not they have had anaphylaxis), epinephrine should be readily available. This will usually be provided as a premeasured dose in an auto-injector, such as EpiPen or EpiPen Jr. Specific indications for administration of epinephrine should be provided in the detailed care plan. Within the context of state laws, appropriate personnel should be prepared to administer epinephrine when needed. Ensure that all new staff members’ education information is current in the W.O.R.K.S. database. Each staff member must register with the WORKS system as soon as they are hired at any location. Education evaluations can take some time to process so beginning the process early will help to ensure we have the accurate information needed when it is time to complete the rated license assessment. The last Annual Compliance Visit was conducted on March 28, 2023. This facility operates with a Five(5) Star License which was issued on January 18, 2019. When I arrived at the facility, I reported to the office, introduced myself, signed in and proceeded to the classroom. At the classroom I was greeted by Ms. Kandice Ceparano, Lead teacher, I stated the purpose of visit and she assisted me during today’s monitoring. Attendance was taken and observations made. The children were observed in transition from the library, routine practices and enjoying story time, indoor free play and preparing for lunch. The last Fire Inspection was also conducted on 8/24/2023 and the facility was approved for daytime care only. The last Sanitation Inspection was conducted on 2/7/2024 with a “Superior” classification. A full assessment of child care requirements was conducted during this date. I monitored program requirements, including postings such as, permit with restrictions maintained, summary of laws, daily schedule, First-Aid posters and the staff/child ratio chart. I monitored program records, a sampling of children’s records, staff personnel files, medical files and on-going trainings. The following violations were observed, cited and technical assistance was offered to maintain compliance. Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. Fire drill were not documented for December 2023-February 2024. .0604(t); .0302(d)(5) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. One staff member must complete and additional 4.45 hours of training to meet requirements, based on education. .1103(a) The above violations must be corrected immediately, and a letter sent to my office stating how the violations were corrected. Please include with the letter the item numbers cited and any documentation to support the corrective actions taken. Your letter should be received by my office on or before April 4, 2024. You were provided an opportunity to ask questions regarding child care requirements. We discussed the items of non-compliance and the rationale in rule. TECHNICAL ASSISTANCE: Fire and Emergency Drills: Caregivers/teachers should practice fire drills with their classroom, at least monthly, to practice and carry out the fire emergency action plan in the event of an actual fire. Staff should be trained that the first priority is to remove the children from the facility safely and quickly. Putting out the fire is secondary to the safe exit of children and the staff. Typically in DPI, the Pre-K classrooms participate in the schools practice drills, however, if you find that school has not conducted a fire drill within the last 28-30 days, you should plan your individual classroom fire drill and document it on you drill logs. This is also best practice with the “shelter-in-place/lock-down drills, which must be conducted at least quarterly. In-service Training: 10A NCAC 09 .1106 DOCUMENTATION OF ON-GOING TRAINING AND PROFESSIONAL DEVELOPMENT (a) Each center shall have a record of training activities in which each staff member participates, including copies of training certificates or official documentation provided by the trainer. That record shall include the subject matter, topic area in G.S. 110-91(11), training provider, date provided, hours, and name of staff who completed the training. This documentation shall be on file and current. The last Fire Inspection was also conducted on 8/24/2023 and the facility was approved for daytime care only. The last Sanitation Inspection was conducted on 2/7/2024 with a “Superior” classification. A full assessment of child care requirements was conducted during this date. I monitored program requirements, including postings such as, permit with restrictions maintained, summary of laws, daily schedule, First-Aid posters and the staff/child ratio chart. I monitored program records, a sampling of children’s records, staff personnel files, medical files and on-going trainings. The following violations were observed, cited and technical assistance was offered to maintain compliance. Fire drill were not documented for December 2023-February 2024. .0604(t); .0302(d)(5) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. One staff member must complete and additional 4.45 hours of training to meet requirements, based on education. .1103(a) The above violations must be corrected immediately, and a letter sent to my office stating how the violations were corrected. Please include with the letter the item numbers cited and any documentation to support the corrective actions taken. Your letter should be received by my office on or before April 4, 2024. You were provided an opportunity to ask questions regarding child care requirements. We discussed the items of non-compliance and the rationale in rule. TECHNICAL ASSISTANCE: Fire and Emergency Drills: Caregivers/teachers should practice fire drills with their classroom, at least monthly, to practice and carry out the fire emergency action plan in the event of an actual fire. Staff should be trained that the first priority is to remove the children from the facility safely and quickly. Putting out the fire is secondary to the safe exit of children and the staff. Typically in DPI, the Pre-K classrooms participate in the schools practice drills, however, if you find that school has not conducted a fire drill within the last 28-30 days, you should plan your individual classroom fire drill and document it on you drill logs. This is also best practice with the “shelter-in-place/lock-down drills, which must be conducted at least quarterly. In-service Training: 10A NCAC 09 .1106 DOCUMENTATION OF ON-GOING TRAINING AND PROFESSIONAL DEVELOPMENT (a) Each center shall have a record of training activities in which each staff member participates, including copies of training certificates or official documentation provided by the trainer. That record shall include the subject matter, topic area in G.S. 110-91(11), training provider, date provided, hours, and name of staff who completed the training. This documentation shall be on file and current. We discussed that after your 2025 annual compliance rated license visit, you will be given the application for assessment. Please submit the application to me and I will request the assessment. AGENCY NEWS: Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in child care. Therefore, it is important that you remain knowledgeable of all licensing requirements. It is always your responsibility to maintain compliance with all applicable laws and rules.
