The last annual compliance visit was conducted 8/30/23. The program currently operates with a four (4) star rated license issued 1/23/19. The program earned six (6) points in program standards and four (4) points in staff education. Alexis Graves, teacher, assisted me during today’s visit. I conducted an unassisted walkthrough of the center. The last sanitation inspection was conducted on 2/20/24 and received a ‘Superior’ classification with 4 demerits. The last fire inspection was completed 11/9/23. Children were observed playing in activity areas and routine care activities. The files for two staff were reviewed. Transportation is not provided. A partial assessment of applicable childcare requirements was conducted today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). The qualification letter for R. Moore expired 11/8/23 and the letter for A. Graves expired 1/10/24. Technical assistance was provided for R. Moore. G.S. 110-90.2(b) & .2703(n)&(o) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Graves. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or 5/30/24. The letter must include the following: *Facility Name and ID number *List of each violation number *How each violation was corrected All items must be included, or the corrective action letter will be returned for corrections. All requested documentation must accompany the Corrective Action Letter. The letter can be emailed to lori.rierson@dhhs.nc.gov. CONSULTATION AND RESOURCES: As we resume the rated license assessments, this facility has been placed in Cohort 2. Your preparation year will be July 1, 2024-June 30, 2025 and your assessment year will be July 1, 2025-June 30, 2026. If a staff member leaves your employment and then returns to work, you must have the last day of employment recorded and the rehire date. If the length of separation is more than twelve months, the staff member must start a new file and complete orientation, medical, TB, and Health and Safety trainings if the trainings were completed more than twelve months prior to date of rehire. If staff members work at more than one of your locations, a copy of their complete staff file must be on site at each location for review. We discussed the enrollment in the infant room. If any of the infants are present and the one two-year old child is present, the two-year old must be moved to the toddler room. An infant cannot be in the same room with a two-year old child.
