Upon my arrival, I was greeted by the Assistant Director, she assisted me throughout the visit. The Director was not present during today’s visit. I conducted a walkthrough of all classroom spaces, documented enrollment, attendance records, and completed observations. The children were observed participating in free play in their classrooms, listening to stories and interacting with the teachers. The last annual compliance visit was conducted on March 4, 2025. The center is currently operating with a 4-star license issued on June 28, 2018 . The most recent sanitation inspection was conducted on December 30, 2025, with the center receiving an Superior rating. The last fire inspection was documented, December 30, 2025. Fire drills and playground inspections were monitored. Staff and children’s files will be monitored at a later visit. Your compliance history is currently 97% this percentage may change based on the results of today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. I observed the following violations during today’s visit. Violation Number Comment Rule 319 Staff/child ratios applicable to a classroom, were not posted in each classroom. Ratios were not posted in space 1 and 2. .0713(a)(10), (c) & (f)(3); .2818(e) 871 Center staff did not comply with the safe sleep policy. The policy states notification will be placed near the crib when infants began to roll over, it was not posted in space 2, there are 5 infants enrolled. 10A NCAC 09 .0606(a) The above violations must be corrected immediately, and a letter must be sent to me no later than February 20, 2026 stating how each item was corrected and a plan to maintain compliance with the requirement. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation and your plan to keep each item in compliance. If I do not receive your letter by February 20, 2026 and/or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the violations have not been corrected or addressed. You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. QRIS The Director stated the center will be doing Pathway 2 for their Rated License. RESOURCES The Health and Safety Trainings are offered on the DCDEE Moodle format. On the Division’s website look under the “SERVICES” tab for “DCDEE Moodle”. These trainings are free, and participants will receive training certificates when completed. I conducted a walk-through of the facility unaccompanied. During the visit, I discussed the allegations with Christy McCawley, Administrator, and additional 3 staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A staff member hit a child on the head with an open hand. G.S. 110-91(10) Violations must be corrected immediately. A checklist was used to note the requirements I monitored today, and a copy was left with you. The last annual compliance visit was conducted 3/14/23. Currently this program operates with a four (4) star license, issued 6/28/18. The facility earned four (4) points in staff education, five (5) points in program standards, and one (1) quality point form programmatic options met. Christy McCawley, center director, assisted me today. I conducted an assisted walk-through of the center. The last sanitation inspection was completed 1/8/24 with a ‘Superior’ classification and 15 demerits. The last fire inspection was conducted on 12/9/23. Children were observed playing in activity areas, eating lunch, and routine care activities. Transportation is provided for school-age children in the afternoons and for field trips. Two minibus’ (EMA 5181 and FBS 7434) were available and monitored today. A partial assessment of childcare rules was conducted today. Staff records were not monitored today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. Two prescriptions, one in room 5 and one in room 7, were not in the original labeled container. .0803(2)(a) 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. The boards on a picnic table seat located on the playground were split and jagged. This posed and impingement or scratching hazard. 10A NCAC 09 .0604(p) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. A photograph was not included with the emergency information for three children transported on the butterfly bus. 10A NCAC 09 .1003(d) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. McCawley. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or 3/27/24. The letter must include the following: *Facility Name and ID number *List of each violation number *How each violation was corrected All items must be included, or the corrective action letter will be returned for corrections. All requested documentation must accompany the Corrective Action Letter. The letter can be emailed to lori.rierson@dhhs.nc.gov. CONSULTATION: A follow-up visit will be conducted within two weeks to complete a review of your staff files. Your childcare center is in cohort 1 for the rated license assessment. We discussed your options today for your preparation year that ends June 30, 2024. If you choose to complete an assessment during your preparation year, it must be requested and completed prior to June 30, 2024. During the visit, technical assistance was provided on the following items: ITERS: • Labeling shelves and reviewing the ITERS book for materials required in activity areas. • In rooms with one and two-year old children, more items and defined activity areas are required. • Add child-led artwork on the walls. • If the majority of children are older than thirty months on the day of the assessment, the ECERS will be used for that class. ECERS/SACERS: • A minimum of twenty books should be available for a group of fifteen children. One additional book per child over fifteen. Books should cover a variety of topics and reflect diversity. • Rooms assessed for ECERS should have a music player available for use by the children. Children under four-years of age can have use with assistance. A minimum of three different types of music should be available. • The quantity of materials for the art, science, math and block areas should be reviewed for all rooms. Teachers may create of list of the math categories and list where items are located throughout the classroom that meet the requirements. This information may be provided to the assessor. • Cozy areas should have a high level of ‘softness’. This can include lots of pillows, blankets, bean bags, and stuffed animals. Most rooms need to look at adding this area. • Five gross motor activities must be available for children to use indoors and outdoors. • The majority of items posted in each classroom should be children’s work. The majority of the children’s work must be child-led art. All classrooms need to post more children’s art and date it. It should not be older than two weeks. • We discussed spacing of cots. At the seven-point level, cots should be 36 inches apart. If cots are closer than 36 inches, a divider can be used. The divider should be the length of the cot and an easily cleanable surface. • All classrooms need to add dress-up clothes in the dramatic play area. • Activity areas should be clearly defined. These do not need to line the walls. Teacher should utilize all floor space. • Each room needs to add materials and develop the science areas. Lead teachers should familiarize themselves with the questions following the indicators. These will be asked during the interview portion. In addition, all staff may use the NCRLAP website, North Carolina Rated License Assessment Project (ncrlap.org), for additional notes and videos to help prepare. Under the training tab there are contact hour trainings and live training events. The resource tab has many documents on how to prepare for the assessments. These are great resources for staff who are not experienced with the assessment process.
