The last annual compliance visit was conducted 4/18/24. Currently this program operates with a five (5) Star License, issued 12/18/19. The facility earned seven (7) points in staff education, seven (7) points in program standards, and one (1) quality point for education options met. Lead teacher Vickie Patrick was not present today. Assistant teachers Wendy Howard and Christy Neese assisted me today. I conducted an unassisted walk-through of the classroom. The last sanitation inspection was completed 9/12/24 with a ‘Superior’ classification and 0 demerits. A fire inspection was conducted on 3/19/24. The most recent inspection on file was not dated. A message was left with the fire marshal to obtain this date. Children were observed in large group activities, eating lunch, and routine care activities. Transportation is provided by children’s parents. The center has one classroom with fourteen NCPK enrolled. The NCPK requirements were not monitored during today’s visit. A partial assessment of childcare requirements was conducted today. The following violation(s) were observed today: Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). The qualification letter for W. Howard was due by 9/25/24 and was received 10/17/24. G.S. 110-90.2(b) & .2703(n)&(o) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Neese. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or 12/23/24. The letter must include the following: *Facility Name and ID number *List of each violation number *How each violation was corrected All items must be included, or the corrective action letter will be returned for corrections. All requested documentation must accompany the Corrective Action Letter. The letter can be emailed to lori.rierson@dhhs.nc.gov. CONSULTATION: All licensed childcare facilities are currently under the ‘Hold-Harmless’ state for their license from DCDEE. Once QRIS is implemented, you will receive information from DCDEE regarding the new pathways for licensure and your timeframe for completing your license renewal. The NCRLAP website, https://ncrlap.org, has trainings geared towards the new ECERS3 assessment scale. These will introduce you to the updated assessment scale and provide tips for completing the new scale. A checklist was used to note the requirements I monitored today, and a copy was left with you today. The last annual compliance visit was conducted 4/27/23. Currently this program operates with a five (5) Star License, issued 12/18/19. The facility earned seven (7) points in staff education, seven (7) points in program standards, and one (1) quality point for education options met. Lead teacher Vickie Patrick assisted me today. I conducted an unassisted walk-through of the classroom. The last sanitation inspection was completed 2/15/24 with a ‘Superior’ classification and 0 demerits. The last fire inspection was conducted on 3/19/24. Children were observed in large group activities, eating lunch, resting, and routine care activities. Transportation is provided by children’s parents. The center has one classroom with seven NCPK and three EC children enrolled. The NCPK requirements were monitored during today’s visit. The program operates for 6.5 hours a day. A monthly breakfast and lunch menu was posted in each classroom. Two NC Pre-K children's files were monitored and each contained a health assessment within 30 days, a DIAL screening within 90 days, and evidence of each child's on-going progress. The teachers use Tools of the Mind and activity plans were current and posted. Staff/child ratio of 1:9 or less is being maintained. The parents receive a bi-weekly newsletter, receive daily updates through DOJO,and they are able to volunteer throughout the school year. The teachers also have parent meetings and conferences three times a year. The staff noted on the Program Consultant Site Visit information screen was correct. The current ECERS-R score was received 11/26/19 and the score was 5.84. A full assessment of childcare requirements was conducted today. You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Patrick. Violation Number Comment Rule 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. One slide had a crack in the near the bottom posing a pinching/impingement hazard. One of the mulch bumpers was cracked open where the bolt was located. 10A NCAC 09 .0604(p) The violation(s) documented above must be corrected immediately.
