NC-Pre-K requirements were also monitored. A checklist was used to note the requirements I monitored today. This program is operated by Guilford County Schools. This NC Pre-K currently operates with a Four Star license issued on March 6, 2018. This facility has one licensed space (room #205). Upon arrival, I was greeted by the classroom teachers Ms. Thompson (Teacher) & Ms. Robbins (Teacher Assistant). Ms. Thompson assisted me during my visit today. I made you aware of the purpose of today’s visit. A sanitation inspection was conducted on October 24, 2024. This facility received four (4) demerits and a superior rating. Monthly playground and fire drills have been conducted and documentation was available for review. Quarterly disaster drills have also been documented. The last fire inspection was conducted on February 29, 2024. Thirteen (13) children were present during today’s visit. All indoor spaces used by the children were observed today. During today’s visit, I observed the children, during story time, center time, gross motor time, and lunch time. I also observed the children completing bathroom and hand washing routines. The teachers interacted with the children in a pleasant manner. The following activity areas were available for the children, blocks, art, dramatic play, books/language, and manipulatives. There were enough materials available for at least three children and at least three related activities in each area. Along with these activity areas, there was a music, writing, science/math area and sand/water table available for the children. Staff and children’s records were reviewed today. The staff and training worksheet was updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. All required posted information was observed current and posted in the classrooms. NC Pre-K requirements were also monitored during today’s visit. The following violation of child care requirements was cited today. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. A newly hired teacher did not have a criminal record check completed. G.S. 110-90.2(b) Child Care facilities must maintain compliance with applicable childcare requirements at all times. In order to maintain compliance with North Carolina childcare laws and rules, the above violation must be corrected immediately. Please send a letter of compliance stating how the above violation was corrected and what you will do in the future to prevent this violation from being cited. Please email me this letter by November 29, 2024. ITEMS TO COMPLETE: --Both staff members need to complete Recognizing & Responding to Suspicion of Child Maltreatment training by the end of this month. Please send me a copy of this completed training by November 29, 2024. The training can be found on the website below: https://www.preventchildabusenc.org/recognizing-responding-online-course/ MEDICATION: Medicines can be crucial to the health and wellness of children. They can also be very dangerous if the wrong type or wrong amount is given to the wrong person or at the wrong time. Prevention is the key to prevent poisonings by making sure medications are inaccessible to children. Prescribed medications as well as over the counter (OTC) medicines, must be labeled properly when brought to the childcare facility by the parent/guardian. The medication must be in the original container (with a label that includes the child’s name, date filled, prescribing clinician’s name, pharmacy name and phone number, dosage/instructions, and relevant warnings). It is important that all medications properly contain the child’s first and last name on the medicine and the box (if applicable) to avoid a child receiving the wrong mediation (this also includes OTC medication). During today’s visit a child who had an action plan on file for an inhaler did not have the medicine located in the classroom. You stated that the medication was located in the office. I made you aware that it is imperative that all emergency medication be in the classroom and taken outside with you in your emergency bag. Emergency medication must always be accessible to you in the event of an emergency when the child may need the medication. If the medicine is stored in a location far away from the child, it can prevent the child from getting the care that is needed in an emergency situation. --Along with your corrective action letter please state that all emergency medication has be stored in the classroom. ANNUAL TRAINING: Each year I will monitor training from the last school year. Please have this information available for review. **ANNUAL REMINDERS: As discussed during today’s visit the following items must be completed/updated annually. --Staff Records: Annual training, Emergency/Health questionnaire, review of EPR Plan and Emergency Care Plan review. --These documents will be monitored for compliance during each annual compliance visit. Sample forms can be found on the DCDEE website. As a reminder, please make sure you update the Off-site verification forms for Staff and Children’s records each year, your Principal should sign these forms at the beginning of the school year. RATED LICENSE UPDATE: The Governor signed Senate Bill 425 (now Session Law 2024-34), Health and Human Services Omnibus Bill, which includes the Quality Rating Improvement System (QRIS) Modifications recommended to the Legislature by the NC Child Care Commission to update QRIS and create additional pathways for licensed child care facilities to earn star rated licenses. Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. UPDATED RULES: Effective January 1, 2024, we have new/updated rules posted on the DCDEE website. The changes relate to definitions; lead and asbestos for centers and family child care homes; building requirements for family child care homes; multi-unit child care centers; and criminal background checks. Upon my arrival, I was greeted by Ms. S. Terhune, the lead teacher. I informed them the purpose of today’s visit. Ms. Terhune gave me the necessary paperwork I needed to complete today’s visit. I observed and documented the attendance. Fifteen (15) children were present during today’s visit. They were observed eating lunch, using the bathroom and napping. Limited licensing requirements were monitored. The followings items were observed in compliance during today's visit: supervision, staff/child ratio: space capacity: proper storage of medications; use of approved space, permit posted and permit restrictions. Three (3) violations were cited during today’s visit. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. One staff did not have a criminal background completed prior to employment. G.S. 110-90.2(b) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff did not successfully complete certification in First Aid within ninety days of employment. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff did not successfully complete certification in CPR training within 90 days of employment. .1102(d) The above violations must be corrected immediately, and a letter must be sent to me no later than May 9,2024 stating how each item was corrected. Consultation: Today I discussed with Ms. Terhune and Ms. Vaka, the principal that Ms. Young would need to switch with the previous teacher assistant Ms. Person until Ms. Young received her criminal record letter. Ms. Terhune and Ms. Vada. stated they understood. This corrected the violation during the visit. Once she receives a her letter send me a copy of her letter. Rated License: I also discussed with Ms. Terhune the Rated license assessment process will begin July 1,2024. I informed Ms. Terhune this facility is in Cohort 1. I informed Ms. Terhune this facility's assessment year is July 1,2024 to June 30, 2025. Ms. Terhune informed me that she will be moving to first grade next year. Thank you for your time. A checklist was used to note the requirements I monitored today. This facility currently operates with a Four Star license issued on March 6.2018. Upon arrival to the facility, I was greeted by Ms. S. Terhune, the lead teacher. Ms. Terhune assisted me during my visit today. All indoor and outdoor spaces used use by the children was observed today. All required items were posted. All medication and hazardous items were out of the children's reach. During today’s visit I observed the children preparing to go outside playing outside, playing in centers., cleaning up, listening to a story washing their hands in preparation for lunch, eating lunch and napping. Today, I observed two children with unique and challenging behaviors. They were observed throwing toys, wooden blocks and furniture. During my monitoring visit, I had to duck to avoid being hit with the objects the children were throwing. The teachers are working with Bringing out the Best agency. They had provided. Sixteen (16) children were present during today’s visit. A sanitation inspection was conducted on February 21, 2023. This facility received a superior rating with two (2) demerits. A fire inspection was conducted on March 17,2023. Staff and children files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. Two violations were cited during today’s visit. Violation Number Comment Rule 402 Each child's diapers were not changed at areas designated exclusively for diapering, on a surface that was smooth, nonabsorbent, easily cleanable and of tight construction. One child was change in the block center. 15A NCAC 18A .2819(a) & (b) 807 A safe indoor and outdoor environment was not provided for the children. The indoor space was not safe. There were cots stacked upward around the room. This created a safety hazard for the children. 10A NCAC 09 .0601(a) The above violations must be corrected immediately, and a letter must be sent to me no later than December 11,2023 stating how each item was corrected and a plan to maintain compliance with the requirement. Consultation: I discussed with Ms. Terhune that the staff members need in-service training hours, health safety trainings, It-SIDS, CPR and First Aid Trainings by specific dates. Please refer to the staff/training worksheet. Rated License: Education: Today, I discussed with Ms. Terhune and the new principal, Ms. S. Vaka the education process for the upcoming rated license reassessment. I showed Ms. Vaka the NC Works page on our website. I explained to Ms. Vaka the steps she needs to complete in order to have her education evaluate in DCDEE's workforce database system. Based on the information in the workforce database system, the current lead teacher meets seven points in education and the current teacher meets seven points in education. I also discussed with Ms. Terhune the Rated license assessment process will begin July 1,2024 next year. There is an observation assessment that you can contact the rated assessment project and a preparation assessment you with me. Ms. Terhune stated she would let me know. I informed Ms. Terhune this facility is in Cohort 1. You must let me know if you ready to start the process before June 30,2024. Thank you for your time during today's visit.
