Sandra Woriax, director assisted me with the visit. Your program currently operates with a Three- star license effective August 2, 2022. Your program is currently earning (5) points in the education component, (2) points in the program standards component (the program met enhanced space) and 1quality point (the child care administrator has at least 10 years of documented child care administrative work experience in a licensed program that can be verified by the Division). The center's compliance history was reviewed with the operator. The program’s compliance history was 93% as of today. The last sanitation inspection was completed November 1, 2024, with a “Superior” classification. The last fire inspection was completed on February 18, 2025, and your facility was approved for daytime and night care. The NC Secretary of State website was reviewed on today and KIDDIE WORLD NURSERY AND PLAYSCHOOL, INC. was listed as current and active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children playing in their indoor learning environments. Supervision and staff/child ratios were found to be in compliance. The caregivers were interacting and meeting the developmental needs for each of the children. There were no infants in care during today’s visit. Rated license requirements were reviewed to ensure compliance was maintained with current program and education components as well as the quality point. Breakfast consisted of cheese toast, apple sauce, and milk. The following violations were documented during the visit: Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. The last sanitation report is lasted 11/1/24. 10A NCAC 09 .0304(b) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. One staff member's heal questionnaire was not kept separately from their individual file. This was corrected by putting the health questionnaire in a separate file. .0701(d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violations documented must be corrected immediately. On or before February 17, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Sandra Woriax, director assisted me with the visit. Your program currently operates with a Three- star license effective August 2, 2022. Your program is currently earning (5) points in the education component, (2) points in the program standards component (the program met enhanced space) and 1 quality point (the child care administrator has at least 10 years of documented child care administrative work experience in a licensed program that can be verified by the Division). The center's compliance history was reviewed with the operator. The program’s compliance history was eighty eight percent as of today. The NC Secretary of State website was reviewed on today and KIDDIE WORLD NURSERY AND PLAYSCHOOL, INC. was listed as current- active. Today, I received your fire inspection. Your inspection is dated 1/16/24, and you are approved for daytime and night care. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children during rest time and found supervision and staff/child ratios to be in compliance. The caregivers were interacting and meeting the developmental needs for each of the children. Rated license requirements were reviewed to ensure compliance was maintained with current program and education components as well as the quality point. No new staff members had been hired. The following violations were documented during today’s visit: Violation Number Comment Rule 614 Bed, cribs, playpens, cots or mats were not placed at least 18" apart or separated by partitions when in use. While the children were resting the mats in space numbers 1a and 2, were placed less than 18" apart. 15A NCAC 18A .2821(e) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. In space #1b, a bottle of soda was sitting on a low shelf. .0901(i) 1867 The depth of the loose surfacing was not based on critical height of the equipment. On the playground, the large climbing structure is sitting on sand that is compacted by grass underneath the structure. .0605(k)(1-4) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violations documented must be corrected immediately. On or before 2/22/24, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. Make this a quarterly event on your calendar and playground check list. If there is a laps between the need of replenishment and receiving the surfacing, rope of the equipment until adequate surfacing is achieved.
