The facility currently operates with a one star rated license, issued November 9, 2018. The program’s 18-month compliance history before today’s visit was 98%. The last annual compliance visit was conducted on August 21, 2024. Upon arrival I was greeted by Paula Schuck, Owner. Ms. Schuck assisted me with today’s visit. One child was present. All items required to be posted were observed in compliance. The emergency drill log was reviewed today. The last fire drill was conducted August 1, 2025. A shelter-in-place and lockdown drills were conducted on June 2, 2025. Outdoor inspections were monitored today and occurred as required. A sanitation inspection was conducted April 8, 2025. The Emergency Preparedness plan was reviewed and in compliance. One child’s file and the provider’s file was monitored during the visit and found in compliance. The provider has completed all required training. One hour on-going training is due by August 21, 2025. I monitored all indoor areas today. The child present was cared for in a nurturing manner. The materials were age and developmentally appropriate and in good repair. I monitored hazardous materials and cited a violation. Please review violations section for details. The refrigerator temperature was forty-four degrees and in compliance. The outdoor area was not monitored due to inclement weather. Ms. Schuck shared the area is grass only and she takes materials outside for outdoor play. Ms. Schuck stated there are currently no emergency medications or topical ointments used in the facility. The following violation was cited today: Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Lysol wipes and Diaper rash spray were in an unlocked cabinet lower than five (5) feet accessible to children. .1719 (a)(7) Compliance Statement: Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The attendance including arrival and departure times had not been documented for August 19, 2024 to August 21, 2024. .1721(e)(6) 1704 Summary of the NC Child Care Law was not given to each child's parent, guardian, or full-time custodian before the child was enrolled in the home and/or signed statement was not on file. One child did not have verification on file that the Summary of the Law was given to the parent. The facility has a One Star License with an effective date of November 9, 2019. The facility license and NC Summary of the Law was prominently posted. The facility’s 18-month compliance history score before today’s visit was 97%. Upon arrival I was greeted by the provider, P. Schuck. I introduced myself and stated the reason for the visit. There were two preschool children present. The children were observed participating in personal care routines, free choice of indoor learning activities, lunch, and rest time. The children have a room in the home that is dedicated for their use. The space is equipped with a variety of materials and equipment accessible to them. The Division of Child Development and Early Education’s 2022 April Items Listing and the Annual Compliance Monitoring Checklist for Family Child Care Homes was used to conduct the monitoring visit. Three children’s files were monitored during the visit. The emergency drill log was reviewed today. The last fire drill was conducted September 1, 2023. A shelter-in-place and lockdown drill was also conducted on September 1, 2023. Outdoor inspections were monitored today and occurring as required. There were four violations cited today. Violation Number Comment Rule 925 Signed and dated statement which attests that a copy of the discipline policy was given to and discussed with a parent/guardian of each child prior to enrollment was not on file in the home. Three children did not have verification on file that the parents received a copy of the discipline policy. GS 110-102 2030 Operator did not provide a written statement to parents regarding the smoking and tobacco restriction. Three children did not have verification on file that the parents received the smoking and tobacco restriction. .1719(a)(11) 2031 Operator did not provide a copy of the shaken baby syndrome and abusive head trauma policy to parents at time of enrollment, and / or within fourteen days of a changes to the policy. Two children did not have verification on file that the parents received the shaken baby syndrome and abusive head trauma policy. .1726(b)&(c) Technical Assistance/General Information. A conversation was held with the Director regarding the violations cited. Below is the rule reference for each violation. 10A NCAC 09 .1719 REQUIREMENTS FOR A SAFE INDOOR/OUTDOOR ENVIRONMENT (11) notify the parent of each child enrolled in the facility, in writing, of the smoking and tobacco restriction; 10A NCAC 09 .1726 PREVENTION OF SHAKEN BABY SYNDROME AND ABUSIVE HEAD TRAUMA (b) A copy of the policy shall be given and explained to the parents of children up to five years of age on or before the first day the child receives care at the home. The operator shall obtain the parent's signature. The acknowledgement shall contain the following and be maintained in the child's file for review by the Division: (1) the child's name; (2) the date the child first attended the home; (3) the date the operator's policy was given and explained to the parent; (4) the parent's name; (5) the parent's signature; and (6) the date the parent signed the acknowledgment. 10A NCAC 09 .1727 DISCIPLINE POLICY (a) The family child care home operator shall provide a written copy of and explain the operator's discipline practices to each child’s parent at the time of enrollment. (b) The operator shall obtain a statement signed and dated by the parent that attests that a copy of the written discipline policy was given to, and discussed with him or her. (c) That statement shall include the following: (1) the child’s name; (2) the date of enrollment; and (3) if different, the date the parent signs the statement. The signed, dated statement must be in the child’s record and must remain on file as long as/ the child is enrolled. § 110-102. Information for parents. The Secretary shall provide to each operator of a child care facility a summary of this Article for the parents, guardian, or full-time custodian of each child receiving child care in the facility to be distributed by the operator. Operators of child care facilities shall provide a copy of the summary to each child's parent, guardian, or full-time custodian before the child is enrolled in the child care facility. The child's parent, guardian, or full-time custodian shall sign a statement attesting that he or she received a copy of the summary before the child's enrollment. The summary shall include the name and address of the Secretary and the address of the Commission. The summary shall explain how parents may obtain information on individual child care facilities maintained in public files by the Division of Child Development and Early Education. The summary shall also include a statement regarding the mandatory duty prescribed in G.S. 7B-301 of any person suspecting child abuse or neglect has taken place in child care, or elsewhere, to report to the county Department of Social Services. The statement shall include the definitions of child abuse and neglect described in the Juvenile Code in G.S. 7B-101 and of child abuse described in the Criminal Code in G.S. 14-318.2 and G.S. 14-318.4. The statement shall stress that this reporting law does not require that the person reporting reveal the person's identity. The summary of this Article shall be posted with the facility's license in accordance with G.S. 110-99. Religious-sponsored programs operating pursuant to G.S. 110-106 shall post the summary in a prominent place at all times so that it is easily reviewed by parents. The facility has a One Star License with an effective date of November 9, 2019. The facility license and NC Summary of the Law was prominently posted. The facility’s 18-month compliance history score before today’s visit was 97%. Upon arrival I was greeted by the provider, P. Schuck. I introduced myself and stated the reason for the visit. There were two preschool children present. The children were observed participating in personal care routines, free choice of indoor learning activities, lunch, and rest time. The children have a room in the home that is dedicated for their use. The space is equipped with a variety of materials and equipment accessible to them. The Division of Child Development and Early Education’s 2022 April Items Listing and the Annual Compliance Monitoring Checklist for Family Child Care Homes was used to conduct the monitoring visit. Three children’s files were monitored during the visit. The emergency drill log was reviewed today. The last fire drill was conducted September 1, 2023. A shelter-in-place and lockdown drill was also conducted on September 1, 2023. Outdoor inspections were monitored today and occurring as required. There were four violations cited today. Violation Number Comment Rule 925 Signed and dated statement which attests that a copy of the discipline policy was given to and discussed with a parent/guardian of each child prior to enrollment was not on file in the home. Three children did not have verification on file that the parents received a copy of the discipline policy. G.S.110-91(10); .1727(a)&(b) 1704 Summary of the NC Child Care Law was not given to each child's parent, guardian, or full-time custodian before the child was enrolled in the home and/or signed statement was not on file. A conversation was held with the Director regarding the violations cited. The acknowledgement shall contain the following and be maintained in the child's file for review by the Division: (1) the child's name; (2) the date the child first attended the home; (3) the date the operator's policy was given and explained to the parent; (4) the parent's name; (5) the parent's signature; and (6) the date the parent signed the acknowledgment. 10A NCAC 09 .1727 DISCIPLINE POLICY (a) The family child care home operator shall provide a written copy of and explain the operator's discipline practices to each child’s parent at the time of enrollment. (b) The operator shall obtain a statement signed and dated by the parent that attests that a copy of the written discipline policy was given to, and discussed with him or her. (c) That statement shall include the following: (1) the child’s name; (2) the date of enrollment; and (3) if different, the date the parent signs the statement. The signed, dated statement must be in the child’s record and must remain on file as long as/ the child is enrolled. § 110-102. Information for parents.
