Upon arrival, I entered the office area to sign in. Both owners, Elizabeth Garlick, Nicole Bell, and Tonya Silas were all present. I stated the purpose of today’s visit and asked who wanted to accompany me throughout the walk-through of the facility. Currently this center operates with a Temporary License, issued on August 7, 2025. The restrictions include daytime care only; Children under 0-12 years of age. Children under 2 ½ years old in rooms with direct exits only. The facility administrator stated that NCRLAP visited the program on 12/15/2025 for ECERS and SACERS and 12/16/2025 for ITERS classrooms. I stated that this is a great opportunity for the administrator to make sure all staff have a valid NC ID to create a NC WORKS account to get all education upload and sent for evaluations prior to the 3rd temp visit before the facility’s temporary license expires on February 7, 2026. Indoor Learning Environment: The facility has a total of 101 children enrolled. Out of the 101 children enrolled, 59 children were present. I reviewed all program records requirements. The last fire drill was conducted on 11/26/25. The last playground inspection took place on 12/15/2025. I stated to both owners that whoever is completing the inspection report should document areas that are concerns or present an issue to help correct any potential violations. Records: Staff: The Staff and Training Worksheet was not completed for two (2) new staff that have been hired since my last visit. I stated to Ms. Silas that she will have by Friday, December 19th, 2025, to send me a signed staff and training worksheet. Both records were verified and were in full compliance during today’s visit. Ten percent of children’s records were monitored during today’s visit. Weapons: Your facility reported that they were in compliance during today’s visit with Child Care Requirements regarding firearms. The following violations were cited during today's visit. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. In space 4, the posted activity plan did not have a date documented to verify the current date. GS 110-91(12); .0508(a) 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. In space #4, an area on the wall had peeling/chipping paint accessible to children (near the large group area). 15A NCAC 18A .2825(a) 721 All equipment and furnishings were not in good repair. On the preschool playground one fence wooden panel was observed not in good repair during today's visit. G.S. 110-91(6); .0601(b) 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. On the outdoor learning environment, I observed several trash cans with trash spilling out of the top area onto the ground. 15A NCAC 18A .2832(a) I spoke to both owners about QRIS PATHWAY #1 – PROGRAM ASSESSMENT FOR CHILD CARE CENTERS and we briefly went over the application that was sent to me prior to today’s visit and the request for the ERS assessment. I reiterated all three assessments have been completed and I am awaiting the results for each assessment. Upon receipt of the assessments, I will send an email to the email on file. The Education Excel Worksheets must be completed and sent to me by January 15, 2026. We spoke about the required/requested forms for the Program Assessment for Child Care Center: CQI for the facility and each staff member, Family Engagement, Education Worksheets and Self Study Verification. This is a link to the definitions of the different Pathways, to really let you understand what is entails. We spoke about raking the excessive mulch away from the borders and bring that raked mulch to fill in areas underneath the swings. I also stated that they will need to get the fire inspection completed on our fire inspection form. Please send to me as soon as possible upon completion. Reminder: The facility’s building inspection must be completed prior to February 7, 2026, to avoid a possible Administration Action, placing the facility on a Probationary Action. I met Elizabeth Garlick, Owner, and Tonya Silas, Administrator, about today’s temp time period visit. Ms. Silas indicated that she has to leave early and if I have questions, contact her. Ms. Silas and I conducted the walk-through of the facility. Currently this center operates with a Temporary License, issued on August 7, 2025. The restrictions include daytime care only; Restricted age 0-12 years of age and children less than 2 ½ years in age are in rooms with direct exits. The NC Secretary of State website was viewed before the visit and A Place To Grow Preschool and Daycare, LLC, active/current April 3, 2025. If any changes to the corporation need to be made or you decide to sell your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. Indoor Learning Environment: Upon arrival, staff/child ratio was observed in the rooms that were monitored. In space #1, there were four (4) infant children participating in free choice play on the outdoor learning environment. I discussed with the staff making sure all children have access and mobility when participating in the OLE. I monitored space #7-A room with three-and four-year-old children. These children were also engaged in playing on the OLE. There were nine (9) children presents aged ranging from 3- to 4-year-olds. Children played on the climbing structure, rode trikes and utilized the swings. . I also monitored space #8, a space caring for young two-year-olds. Children were observed sitting in cubbies and wandering around the classroom space. I spoke with the two (2) staff members about making sure materials were accessible to children and not placed on high shelving preventing access. We spoke about creating a plan to introduce materials to toddlers as well as making sure they are in full compliance with child care law/rules. Outdoor Learning Environment: The outdoor learning environment was monitored for compliance. The program has three (3) identified spaces for children to engage in outdoor learning experiences. I spoke with Ms. Garlick about making sure staff are positioned in certain areas on the OLE while children are engaging in play to support appropriate supervision practices, reduce safety concerns and to make sure needs are met when there are more than one class utilizing the areas. I also suggest that classrooms stick to their outdoor learning times on their daily schedules. A fire drill took place on July 14, 2025. A fire drill was not documented for the month of August 2025. The playground inspection was completed for August 8, 2025. I mentioned to Ms. Garlick that a fire drill and a playground inspection should take place next week to remain in compliance with child care rule regarding monthly safety inspections/drills. Transportation: This program does participate in Transportation. The transportation white van-license plate KHE 1815 was in full compliance with transportation policies and laws. Progressive Southeastern vehicle insurance-991828729-was current during today’s visit. The Transportation Binder was located on the van. I suggest making copies of driver driver’s license for an additional precaution. Records: Staff/Training Worksheets and Staff files were monitored during today’s visit. We discussed previous staff that were hired under the previous owners do not have to recreate a file or update any required first day documents to avoid any confusion. New staff members hired by the new owners will have to have all new documents to add to their files. We spoke about previous staff members working on training hours should continue to work on training hours and new staff members will have a year from their hire date to complete the required training. I also gave Ms. S. Philemon, Director, until Monday, September 22, 2025, by 5:00 p.m. to send me the last two (2) pages of the completed Staff/Training Worksheets. I spoke with Ms. Garlick about making sure new staff have documented two weeks of orientation for six (6) new staff members that have started 9/2/2025. Three (3) children’s records were reviewed during today’s visit, with no violations found. All staff records were reviewed during today’s visit, with no violations found. Nutrition: This program offers breakfast, lunch and afternoon snacks for families. The breakfast consisted of cheese English muffins, mangos, and whole milk/1% milk. The lunch is documented and prepared, fish sticks, lima beans, whole wheat slice bread, fruit cocktail, and whole milk/1% milk. Weapons: Your facility is in compliance with Child Care Requirements regarding firearms. The following violations were observed and corrected during today’s visit. A current schedule was not posted in space #1 during the walk-through during today's visit. GS 110-91(12);.0508(a) 445 For children under three years of age, developmentally appropriate toys and activities were not provided as outlined in Rule. In space #8 developmentally appropriate toys were observed on high shelving and not made accessible to children under the age of three during today's visit. .0510 (e ) (1)(A-G) 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. The ceiling in space #7 was not in good repair (water damage). 15A NCAC 18A .2825(a) 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. The stationary equipment on the preschool playground did not have more than 6 inches of protective surfacing (mulch) under the climbing structure. .0605(j) 718 Swings did not have surfacing that extends two times the length of the pivot point to the surface below beyond the external limits of the equipment. The swings on the preschool playground did not have enough protective surfacing that extended the length of the swings. .0605(m)(2) 805 Fire drills were not practiced monthly and/or the drill record was incomplete. A fire drill did not take place for August 2025. .0604(t); .0302(d)(5) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. I observed three (3) new staff members (N.B, N.C and Y.C) did not have a health questionnaire on file during today's visit. .0701(a) 1067 Each new employee did not complete, within the first two weeks of employment, six clock hours of training in required topic areas. Six (6) new staff members hired 9/2/2025 did not have documented six (s) clock hours of orientation received. .1101(a)(b) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. Several new staff did not have documentation of receiving The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy prior to caring for children in their file during today's visit. .0608(d)(1-4) Please me the building, fire, and sanitation inspections. Compliance History: The next two Temporary Time Period visits will be unannounced. During those visits a full assessment of the center's ability to maintain compliance with all applicable Child Care Requirements will be conducted. Please note that any violations cited today or during future visits will negatively impact your compliance history. North Carolina General Statute 110-90(4)(d) requires all child care facilities to maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action against the facility's license. Rated License: I discussed briefly the three pathways’ programs can choose. I recently encouraged the owners to participate in an in-person training opportunity to understand each pathway and for them to make an inform decision on which pathway will work best for their program. I encourage Ms. Garlick to attend the in-person training course located at Anson County Partnership for children: 115 East Morgan Street, Wadesboro NC at 11:00 a.m. I also highly recommend that the administrator and owner visit the QRIS Modernization page on our website at https://ncchildcare.ncdhhs.gov/, for additional information regarding pathways to the stars. I informed the administrator and owner on my second unannounced visit to the facility; they will have to decide for me to move forward in the paperwork process for their desired pathway. As discussed, at the end of your temporary license period you must be at least a three (3) star facility in order to continue to provide care for children who receive subsidy. WORKS: If haven’t done so as of today, please make sure your staff establish an NCID for entering their information in the DCDEE WORKS system. Once all staff have completed entering their information in the DCDEE WORKS system, then mail the official transcripts to the Workforce and let me know when they are mailed. Refer to the information on the Division’s website for detailed instructions regarding how to use DCDEE WORKS. Emergency Preparedness and Response: The rules are designed to assist licensed child care programs to be adequately prepared to respond to the needs of children and others in the event of disasters and emergencies. As these rules will have an impact on your facility, please review the information below carefully and let me know if you have any questions. The new Head of School will have the opportunity to complete this required training and will have until June 2026 to complete and update the EPR Plan into the Portal. * Detailed information about the new emergency preparedness and response rules can be found at: http://ncchildcare.nc.gov/providers/pv_emergency.asp . * One person from your facility has taken the Emergency Preparedness and Response in Child Care training. A copy of the Emergency Preparedness and Response Training documentation must be on file for review. * Once the training is completed, the trained staff must complete an EPR Plan on a template at the following website https://rmp.nc.gov/portal/portal.aspx within four (4) months. o If you do not have a computer, you may access one at a public library. o To get on the template you will need an email address and an NCID. Detailed instructions for getting an NCID and for accessing the template are on the Division’s website on the Emergency Preparedness tab. * After you complete the Emergency Preparedness and Response in Child Care training, you must conduct either a shelter-in-place or a lockdown drill every three months. A new form has been created to document these additional drills as well as the fire drills. It is under the Provider Documents tab titled, “Emergency Drill Log.” Health and Safety Trainings: (Some staff are beginning to work on these required trainings. Please make sure you keep track of all received trainings. The Child Care Development Block Grant (CCDBG), also called the Child Care Development Fund (CCDF), is a federal grant which provides funding for child care subsidies for low income working families and improvements in child care quality. The recent reauthorization of CCDBG, Child Care and Development Block Grant Act of 2014, brought for the first time in 18 years new state requirements. These requirements promote the health and safety of children in child care, create continuity of access to subsidies for low-income families, better educate parents and the general public about the child care choices available to them, and improve the overall quality of early care and education programs. States must have pre-service or orientation and on-going minimum health and safety training requirements in place for child care providers. The trainings must cover the following content areas: * Administration of medication * Appropriate precautions in transporting children * Building and physical premises safety * Pediatric First Aid and CPR appropriate for the ages of children in care * Prevention and control of infectious diseases, including immunizations * Prevention of and response to emergencies due to food and allergic reactions * Prevention of shaken baby syndrome and abusive head trauma * Prevention of SIDS and use of safe sleeping practices * Recognizing and Responding to child abuse and neglect * Handling and storage of hazardous materials and the appropriate disposal of bio-contaminants * Emergency preparedness and response planning for emergencies To help child care providers meet the new training requirements, the Division of Child has free Moodle online training modules. Click on the Course Catalog tab at the top of the page on the website to login to your training. General Reminders (in compliance today): You are currently meeting minimum is required at this time. The following staff/child ratios and maximum group sizes apply to your facility: 0 to 12 Months ~ 1 staff / 5 children ~ 10 group 1 to 2 Years ~ 1 staff / 6 children ~ 12 group 2 to 3 Years ~ 1 staff / 10 children ~ 20 group 3 to 4 -Years ~ 1 staff / 10 children ~ 25 group 4 to 5 Years ~ 1 staff /13 children ~ 25 group 5 to 6 Years ~ 1 staff / 15 children ~ 25 group Please make sure you are marking minimum standards on the staff/ratio worksheet for all classrooms in operation. Please make sure teachers are adhering to their outdoor scheduled time. I reminded the new owner that children’s ages ranging from infants to two years of age can only be combined and remain in ratio for the first and last hour of the day. Please make sure you pay special attention to birth dates to avoid any confusion or misunderstanding. Please make sure you are reminding staff not to place materials out of reach of children during the day. Children should be taught how to engage/play with materials at all ages.
