Name of Operation: BUILDING BLOCKS LEARNING WORLD Facility ID: 90000179 Consultant: ABIGAIL AVALOS Operation Type: Family CC Home Case Number: Visit Date: 3/19/2025 Number Present: 2 Completed Date: 3/19/2025 Age: From 3 To 4 Total Minutes: 120 Time In: 09:00 AM Time Out: 11:00 AM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor your program for compliance with applicable child care requirements for an annual compliance visit. Upon arrival I was greeted by Ms. Jennifer Smith/Owner, I shared the reason for the visit and you were able to assist me. Permit Information: The program currently operates with a Four-Star Family Child Care Home License effective 7/20/23 with the following restrictions: - 1st shift - Maximum of 5 preschool children at any time - Children in care on ground level only - Fuel burning stove not used during operating hours - Trampoline & pool not used by FCCH Children while in operator’s care. - Serves no more than two children under age one. I monitored the following items: supervision, ratios, capacity, staff qualifications and training, transportation, health, safety, program records, children’s records, indoor and outdoor learning environments. The following spaces are used by the children: childcare room, bathroom and outdoor learning environment. Ownership: This family child care home is owned by you, Ms. Smith, the individual owner, and you reside in the home. Inspections: The last playground inspection was conducted on 3/5/25. Indoor Learning Environment: I observed the indoor learning environment where plenty of materials and soft furniture items were available for the child children. I observed the children participating in free play outdoors and seasonal themed activities in the indoor learning environment. Outdoor Learning Environment: The outdoor learning environment was monitored. In addition to the fenced in playground, I observed the “nature area” on the other side of the home and the driveway that the children use. Program Records: The last fire drill was conducted on 3/5/25. The last emergency drill was conducted on 3/5/25. I suggest to complete the drills on different days in order for the children to distinguish between the types of drills and not anticipate them both in one (1) day. I reviewed that emergency drills (shelter in place or lockdown drills) are required every three (3) months. Staff Records: I reviewed your file, and you completed the statements of responsibility during the visit. Five (5) on-going training hours are required annually, two and a half (2.5) on-going training hours were carried over from last year’s annual compliance year. All certificates and remaining hours were on file. Two and a half (2.5) hours will carry over to your next annual compliance year which will be from 3/19/25 to 3/19/26. Children’s Records: All children’s records were reviewed, you were unable to locate one (1) child’s file. I reviewed that although the child is not attending every day, that a file is still required. Nutrition: The facility was in compliance with child care meal pattern requirements. Medication: It was reported that medication is not being administered. Transportation: Transportation is provided, please remember that all transportation requirements need to be in compliance when providing transportation: • Accessible transportation in the event of emergency • All children will be transported in a car seat/appropriate restraint device through age 8 or until they weigh more than 80 pounds • Identifying information to include child’s name, photograph, emergency contact information, and emergency medical care plan for each child transported on vehicle • Fire extinguisher (recommend it be secured) • First Aid Kit (recommend it be secured) • Proof of Liability Insurance • Expiration date of Vehicle Registration and Date of last inspection • Tire tread measures more than 2/32 of an inch • Vehicle free of hazards and in good condition – no torn upholstery, broken windows, holes in floor or roof • Cell phone or other two-way voice communication device It was reported that the children’s information from the files are carried out when transportation is taking place. During the visit, children’s photographs were added to the application, this was corrected during the visit. Weapons: The facility is in compliance with child care requirements regarding firearms. Violation Number Comment Rule 920 Records were not made available for review. One (1) child's information was not on site the following items were missing: Application for Enrollment, Immunization Records, Discipline Policy, Summary of Child Care Law, Operational Policies, Prevention of Shaken Baby Syndrome and Abusive Head Trauma Policy, Notification of Smoking and Tobacco Restriction, Animal (Pet) Acknowledgement Form and Written Plan of Care. G.S. 110-91(9) 1103 For each child being transported, the operator or other transportation provider did not have identifying information, including the child's name, photograph, emergency contact information, and a copy of the emergency medical care information form required by Rule .1721(a )(3) in the vehicle whenever children were transported. Two (2) files did not have a photograph attached to the identifying information. .1723(13) Corrective Action Plan – Compliance Letter: The violation not corrected during the visit must be corrected immediately. On or before 4/2/25, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. Please email the information to: abigail.avalos@dhhs.nc.gov a compliance letter template was shared with you please ensure that all these components are completed: • Name of your facility • Your facility ID number • Date the visit made • Date that you are submitting the compliance letter • Provide each cited violation number • Describe when and how each violation was corrected • Describe how you will prevent the reoccurrence of each violation (you need to specify how you will prevent the violation from re-occurring) • Sign with your signature Compliance History: The program’s compliance history was ninety-seven percent (97%) prior to today’s visit. Rated License: Hold harmless has been extended until the new QRIS (Quality Rating and Improvement System) is implemented, this is also known as SB 425 (Senate Bill 425). Providers in Cohort #1 are not required to go any further with a rated license assessment unless requested, and providers in Cohort #2 do not need to start their preparation year unless a rated license assessment is requested. Lead in Water, Paint and Asbestos: As of today, the “Clean Classrooms for Carolina Kids website indicates that your facility has completed all sections. Technical Assistance: Incident reports that require medical attention need to be sent to your consultant within seven (7) days. Please also remember to log the incident in your incident log and to file the incident report in the child’s file. During the visit, I sent over the website for the North Carolina Health and Safety Resource Center to obtain a new and updated copy of the first aid poster and safe sleep poster. Resources and Reminders: Exit Conference: At the completion of the visit, this visit summary was printed, reviewed, and a copy was left with you. We jointly reviewed the visit summary and documentation of my findings and violations with you, Ms. Smith.
