MEYER Operation Type: Center Case Number: Visit Date: 3/10/2025 Number Present: 43 Completed Date: 3/10/2025 Age: From 5 To 11 Total Minutes: 110 Time In: 01:40 PM Time Out: 03:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for an unannounced annual compliance visit. Upon arrival I was greeted by the front desk staff and I shared the reason for the visit. New Program Coordinator, Mr. J. Lamar accompanied me throughout the walk-through of the facility and the outdoor learning environments. Your program currently operates with a Five (5) Star Center License. The license was posted, with restrictions to: first (1st) shift care, meets enhanced space, meets enhanced ratios, serves school age children only, only second graders (2nd) and above on the second (2nd) floor, meets reduced ratios, and no children in the kitchen for any reason. Ownership: The facility is owned by the Union County School Board. If any changes to the ownership need to be made, you must notify your consultant at least thirty (30) days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. I used the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable childcare requirements pertinent to this facility and the Annual Compliance Monitoring Checklist for Child Care Centers. I monitored the following items today: supervision, staff/child ratio, staff, health, safety, and program records. The facility has the following spaces licensed, gymnasium, cafeteria, outdoor playground, Trailer #1, Media Center and two internal classrooms. Currently, the After School program is only able to utilize the two (2) internal classrooms every other quarter so they were not monitored as they have not used them after being licensed. Program Records: We reviewed all the required records and were in compliance. The following Department of Public Instruction (DPI) verification forms were monitored and found to be complete and upto-date: 1. Staff Records Verification Form 2. Staff Criminal Records Verification Form 3. Children’s Records Verification Form I did review one staff file, reminded Mr. Lamar to verify Human Resources has all applicable information and add that staff member to the off-site verification form. Inspections: All inspections were monitored, and inspection dates are listed below. •Fire drill was conducted on 3/7/25 •Emergency drill was conducted on 9/5/24. •Fire inspection was completed on 9/19/24. •Playground inspection was completed on 2/27/25.. •Sanitation inspection was conducted on 11/12/24 and received a Superior classification. Indoor Learning Environment: I observed children in the indoor learning environment and the following items were posted in the classrooms that were monitored: Daily schedules, activity plans, staff/child ratio worksheets, emergency numbers, emergency procedures, arrival/departure procedures, summary of law and menus. Children were observed arriving from end of school activities, transitioning to age based groups for snack, homework time, indoor gross motor activities and free choice center time. Outdoor Learning Environment: The outdoor learning environments were monitored and in compliance. While the safe protective surface remains in compliance, it would be best practice to add more mulch within the next few weeks, to maintain the appropriate depth and maintain safety. Please continue to check structures for nix in paint, and areas in disrepair. Staff Records: The staff-training worksheet was completed prior to the visit. Children’s Records: There are fifty-four (54)children enrolled in this center. There were forty-three (43) in attendance today. The Off-Site Verification forms were completed for each child enrolled and there were no discrepancies observed. Medication: The facility reported they did not have any emergency medication on site. Nutrition: The facility was in compliance during today’s visit with Child Care Meal Patterns Requirements. The menu and allergy listing were posted and for snack the children were having baked cheese curls and 100% (one-hundred) percent juice. Weapons: Your facility reported that they were in compliance during today’s visit with Child Care Requirements regarding firearms. Transportation: The facility reported they do not provide transportation. One (1) violation was observed during today’s visit. The violation is as follows: Violation Number Comment Rule 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last emergency drill was completed in 9/5/24. .0604(u);.0302(d)(8) The violations not corrected during the visit must be corrected immediately. On or before 3/23/25 I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent (75%). Any violation (s) documented may impact the compliance history score. The center's compliance history was reviewed with the operator. The program’s compliance history was percent ninety-nine (99%) percent prior to today’s visit. The compliance history could be impacted after today’s visit. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. Rated License: •This facility has completed an informal assessment of the SACERS-R and will inform the Child Care Consultant they are ready for the QRIS assessment. •Use this time for staff to update their information in the WORKS database. Technical Assistance: •All Department of Public Instruction (DPI) programs use the off-site verification forms, to verify compliance for staff, and children’s files. Please make sure when you add new staff or staff transfers in, this information is represented on the form. •Once you are able to use the internal classrooms, make sure that all documents to remain in compliance are posted in each room. •Each new employee has one (1) year to complete their health and safety training (with certain exceptions), and repeated in five (5) years. Please make sure each staff member is logging their training. At the completion of the visit, this visit summary was printed, reviewed, and a copy was left with you. Ebony Duncan, Licensing Supervisor, assisted me with today’s visit. Upon arrival we were greeted by the office staff and shortly after the Program Director met with us and assisted us on the walkthrough. The program currently operates with a five-star license with permit restrictions of Daytime care only, School Age only, Meets all enhanced standards, enhanced space, and reduced ratios. The center's compliance history was reviewed with the operator. The program’s compliance history was 89% percent as of 10/9/2023. Ownership: The facility owner is Union County Public School. If any changes to the ownership need to be made, you must notify your consultant at least thirty (30) days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. The following items were monitored today: Supervision, Capacity, Staff-Child Ratios, Emergency Medical Plan, Storage and Administration of Medication, Storage of Hazardous materials, General Safety, Adequate and Approved Space, Program Records, and License Posted and Permit Restrictions. The above items were observed to be in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. The spaces used currently are Gym, Cafeteria, Media Center, Center Classroom, bathrooms by the gym and the outdoor play area. The children were arriving from school, reviewing the rules for the day and engaging in teacher and peer interactions. There was no new staff since the last visit. The Program Director stated that there were no firearms on the premises. You do not currently provide transportation. One (1) violation was observed today. Violation Number Comment Rule 853 Incident logs were not completed and maintained as required. There were two (2) children had incident reports on file but was not listed in the incident log. .0802(g)(1-6) You are required to maintain compliance with all applicable child care rules and regulations at all times. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Failure to maintain compliance with all requirements may result in administrative action against your license, which could include civil penalties. Repeated violations may significantly affect your compliance history. Technical assistance As a reminder, all completed, fire inspections must be submitting to your Child Care Consultant within seven (7) days of a passing inspection.
