Facility ID: 5955024 Consultant: GAIL DULA Operation Type: Center Case Number: Visit Date: 10/26/2023 Number Present: 26 Completed Date: 10/26/2023 Age: From 0 To 5 Total Minutes: 150 Time In: 01:00 PM Time Out: 03:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced Annual Compliance visit was to monitor your program for compliance with all applicable child care requirements, including health and safety. The visit was conducted with you, Chelsie Arrowood, Director. A five star license was issued on July 14, 2022. Restrictions include daytime care only, meets enhanced ratios, meets enhanced ratios minus one pre age group, and meets enhanced space. A checklist was used to note requirements monitored during the visit. Three (3) indoor spaces and outdoor spaces were monitored today. Children and staff were observed during nap, free play activities, hand washing, and departure. Infants and toddlers were observed during free play, while being held, and floor play. Staff to child ratios were in compliance. Staff interacted in positive ways with the children. A fire inspection was conducted on September 13, 2023. You provided me with a copy of the inspection. Shelter in place drills and fire drills had been conducted and documented on the DCDEE form. A sanitation inspection was conducted on April 20, 2023, with nine (9) demerits noted on the grade card. Four (4) children’s files were reviewed with no violations. Staff files were reviewed with you with no violations including three new staff files One violation was observed during today’s visit. Violation Number Comment Rule 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. Two plastic riding toys, on the playground, had broken pieces with sharp edges. 10A NCAC 09 .0604(p) The violation documented must be corrected immediately. The violation was corrected during the visit. You removed two broken riding toys from the playground. Compliance History As discussed, violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. Your program must maintain a compliance score of at least 75% as required by G.S. 110-90 (4)(c). your compliance before today’s visit was 95%. Technical assistance on violations was reviewed with you. Areas discussed included: #856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. Two broken riding toys were on the playground. I reminded you to have staff check toys for broken pieces and sharp edges each day. Remove all broken items before children go to the playground.
