Upon arrival I reviewed the purpose of the visit with Lead Teachers, Ms. Matthews and Ms. Padgett. The Lead teachers assisted with the visit. Your program operates with a license issued on May13, 2022. The follows restrictions were in compliance: daytime care, meets enhanced ratios, meets enhanced space and enhanced ratios minus 1 in each group. Two classrooms were monitored today. I observed children during indoor free play activities, bathroom/handwashing routines, teacher directed activities, and lunch. Supervision and staff/child ratios in compliance. Staff had positive interaction with the children. One new staff file and one existing staff file was reviewed using the DPI verification form with no violations. A sampling of four children’s files were reviewed using the DPI verification form with no violations. The last monthly fire drill was practiced on 12/10/24 A lockdown drill was conducted on 12/5/24. The last monthly playground inspection was completed on 12/6/24. The most recent lead water testing results were completed on 4/17/24. Lead testing must be completed every three years The most recent sanitation inspection for your facility was conducted on 12/19/24. A superior sanitation classification was issued with 0 demerits. The most recent fire inspection for your facility was conducted on 5/21/24. The facility was approved for day time care. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. NC Pre K requirements were in compliance. One violation was observed today: Violation Number Comment Rule 705 Equipment and furnishings were not sturdy, stable and free of hazards. The coating on the metal platform of the composite structure has deteriorated. The wire has rusted and has sharp edges. .0601(c) The above uncorrected violation must be brought into compliance immediately. You shall submit a written, signed and dated statement or email to me at the address below detailing specifically how the above violation(s) was corrected. The violations must be corrected no later than January 30, 2025. Failure to correct the violations and send the written statement by the due date listed above, noncompliance, may lead to an administrative action. Compliance History As discussed, violations cited today and/or during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. Your program must maintain a compliance score of at least 75%. The center's compliance history was at 99% prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical Assistance: #705- Equipment and furnishings were not sturdy, stable and free of hazards .0601(c) The coating on the metal platform of the composite structure has deteriorated. The wire has rusted and has sharp edges. 10A NCAC 09 .0601 SAFE ENVIRONMENT (c) Equipment and furnishings shall be sturdy, stable, and free of hazards that may injure children including sharp edges, lead based or peeling paint, rust, loose nails, splinters, protrusions (excluding nuts and bolts on sides of fences), and pinch and crush points. TA-I suggested that you use a rubberized spray paint to coat the areas of the composite structure where rust is present. The area had been noted on the playground inspection, be sure to continue to look out for areas containing rust and repair the areas as soon as possible. If the platform is can not be repairable the structure may need to be removed or replaced. Consultation: The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. Upon arrival, the purpose of today’s visit was reviewed with Ms. Matthews and Ms. Padgett, lead Teachers. A substitute was working in space #2 for the assistant teacher. Your program currently operates with a five star license effective May 12, 2022. The permit restrictions were in compliance. Children in care were observed during free play activities, handwashing , lunch, a teacher directed activity, outdoor gross motor play and nap. A sanitation inspection was conducted by Environmental Health on November 20, 2023, with 2 demerits noted. The most recent fire inspection for your facility was conducted on October 31, 2023. The facility was approved for daytime care. The latest fire drill was conducted on January 26, 2024. Children’s records were reviewed with no violations using the DPI verification form. Three staff files were reviewed with no violations Using the DPI verification form. Medication was in compliance. NC Pre-K requirements were in compliance. One violation was documented during today’s visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. A fire inspection was conducted on 10/31/23. The previous inspection was conducted on 10/27/22. The current inspection was not conducted within 12 months of the previous inspection. 10A NCAC 09 .0304(a) The above uncorrected violations must be brought into compliance immediately. Shaken Baby policy must be signed on the first day and a copy of the Qualifying letter must be on file on day one. -Training is available on the Moodle website. -I reminded you to keep soil and debris from building up at the bottom of the fence surrounding the playground.
