The visit was conducted with you, Lead Teachers, Ms. Smith, Ms. Lytle, and Ms. Padgett. This program currently operates with a October 13, 2023. The permit restrictions were in compliance. The restrictions are as follows: daytime care only, meets enhanced ratios minus one for each group, meets enhanced space. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year -old children are enrolled. You said you use Creative Curriculum. The last annual compliance visit was conducted 8/4/25. The most recent sanitation inspection for your facility was conducted on 7/23/25. A superior sanitation classification was issued with 7 demerits noted on the grade card. The most recent approved fire inspection for your facility was conducted on 9/12/25. The facility is approved for daytime care only. The most recent monthly fire drill was conducted on 4/8/26 The most recent quarterly lockdown/shelter-in-place drill was conducted on 4/4/26. The program uses the school Emergency Plan. The facilities EPR Plan was reviewed on 7/7/25. The most recent monthly playground inspection was completed on 3/12/26 by Ms. Lytle. Three classrooms and the outdoor gross motor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children present today were observed during free play activities, lunch, handwashing/bathroom routines, outdoor gross motor activities, a teacher directed activity. One additional head start staff was persent today. A checklist was used to note the requirements I monitored today. The analysis date for the most recent lead water test was 2/26/24. This website also indicated complete for Lead Paint and Survey by RTI for asbestos. Check with school maintenance to ensure these requirements have been completed. Staff records were reviewed using DPI verification forms with no violations documented. There is no new staff. A sample of children’s files were reviewed using the DPI verification form with no violations. NC Pre-K requirements were monitored and in compliance. No violations were observed during today’s visit. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff, employed on 2/5/24, did not renew training with an approved agency. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff, employed on 2/5/24, did not renew training with an approved agency. .1102(d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The center's compliance history was reviewed with the operator. The program’s compliance history was 96% percent as of 4/1/26. The purpose of today’s visit was reviewed with, Lead Teachers, Ms. Hall, Ms. Lytle, and Ms. Smith. The Lead teachers assisted me with the visit. The school has a new principal. A checklist was used to note the requirements I monitored today. Your program currently operates with a five star license effective October 13, 2023. The permit restrictions are as follows: meets enhanced ratios, meets enhanced space, enhanced ratios reduced by one in each group. Three classrooms and the outdoor playground were monitored during today’s visit. Children present today were observed during free play activities, outdoor gross motor activities, transition to lunch, lunch and nap. Supervision and staff/child ratios in compliance. The caregivers interacted with the children and were responsive to the children’s needs. A sample of five children’s files were reviewed with no violations. Staff files were not reviewed since all staff was not listed on the Staff Training Worksheet. Another visit will be conducted to review staff files. The last monthly fire drill was practiced on July 20, 2024. The last shelter-in-place drill was practiced on May 30 2024. The last monthly playground inspection was completed on July 31, 2024. Medication was in compliance. The most recent lead water testing results were completed on April 29, 2024. Lead testing must be completed every three years. A new fire inspection is due by August 30, 2024. -The school has a new principal. The legal designee form was completed during the visit. Please send a Preservice for Administrator form to me. -I reminded you to have staff review incident reports to ensure that all required sections are completed before filing. -We reviewed the Emergency Care Plan. I reminded you to update the plan as staff changes are made. -Hold harmless has been extended until the new QRIS system is implemented. Programs still have the option to choose to voluntarily complete the star rated license reassessment if they so desire. -Child Care Rule Effective January 1, 2024 Lead Testing: Child Care Rule .0601(f)- Safe Environments Rule Amendment- This rule was amended to include the specific requirements that child care centers must be free of asbestos hazards. This is in addition to the existing requirement that they be free of lead poisoning hazards. In addition to the updates to this rule, 10A NCAC 41C .1001 through .1007 and 15A NCAC 18A .2816, require all child care facilities (homes and centers) to test for lead in drinking, water, identify lead-based paint hazard, and mitigate or restrict access to identified hazards. The Clean Classrooms for Carolina Kids program is designed to identify and eliminate exposure to lead and asbestos hazards in building infrastructure, while providing child care facilities with the support needed to meet all rule requirements. The purpose of today’s visit was reviewed with, Lead Teachers, Ms. Hall, Ms. Lytle, and Ms. Smith. The Lead teachers assisted me with the visit. The school has a new principal. A checklist was used to note the requirements I monitored today. Your program currently operates with a five star license effective October 13, 2023. The permit restrictions are as follows: meets enhanced ratios, meets enhanced space, enhanced ratios reduced by one in each group. Three classrooms and the outdoor playground were monitored during today’s visit. Children present today were observed during free play activities, outdoor gross motor activities, transition to lunch, lunch and nap. Supervision and staff/child ratios in compliance. The caregivers interacted with the children and were responsive to the children’s needs. A sample of five children’s files were reviewed with no violations. Staff files were not reviewed since all staff was not listed on the Staff Training Worksheet. Another visit will be conducted to review staff files. The last monthly fire drill was practiced on July 20, 2024. The last shelter-in-place drill was practiced on May 30 2024. The last monthly playground inspection was completed on July 31, 2024. Medication was in compliance. The most recent lead water testing results were completed on April 29, 2024. Lead testing must be completed every three years.
