The visit was conducted with you, C. Michelle Johnson, Owner/Operator. You provided me with applicable program, staff, and children’s records for review. This program currently operates with a Three star rated license effective January 23, 2023. The permit restrictions include 1st shift, max of 5 preschool children, and serves no more than 2 infants under 1. The program’s compliance history was 98 percent prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A checklist was used to note the requirements I monitored today. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios in compliance. Children and staff were participating in group letter activities, handwashing protocols, lunch, and rest time. Five children, one through four years of age, were enrolled. Three children were present today. The FCCH and Centers Located in a Residence Operator’s Statements of Responsibility form and the Verification of Required Information for Operator and Additional Caregivers form were completed and signed during today’s visit. Your signature on these forms served as verification the information provided was accurate and complete. Criminal Background Check Qualifying letters for you and household members were reviewed and were in compliance during today’s visit. Your staff records were monitored during the visit. Children’s files were reviewed. The last monthly fire drill was practiced on October 10, 2025. The last shelter-in-place drill was practiced on September 16, 2025. The last monthly outdoor inspection was completed on October 10, 2025. The Emergency Preparedness Response Plan was reviewed and published on June 15, 2025. The following violations were observed: Violation Number Comment Rule 801 Bacteriological analysis indicating the water is free of bacteria had not been performed on a sample of water from each well used for the children's water supply every two years. Results were not kept on file in the home.
