Monitoring included NC Pre-K requirements in the NC Child Care Rules Section .3000 which are applicable to Rooms 114, 111, and 109. Room 116 is also approved primary space but is not currently being used for Pre-K. Last Annual Compliance visit – 10/16/24 18-month compliance history from 9/15/23 – 3/14/25 = 97% Last Sanitation Inspection – 12/6/24 - Superior Last Fire Inspection – 11/1/24 – Passed with Comments *Sanitation and fire inspections are required at least annually by the end of the month of the previous inspection. Send fire inspections to me within seven (7) days of the inspection. Required Water, Lead Paint, and Asbestos Testing - The Clean Classrooms for Carolina Kids website reflects the following information about your center’s required three-year water testing and one-time lead paint testing, and asbestos testing: Three-year Water Testing – 8/20/24; due again 8/20/27 Lead Paint Testing – survey review by RTI pending Asbestos Testing – survey review by RTI pending Maintain any paperwork received by mail or email in your Program Records file. *Because program is part of a larger DPI school, water testing for the school may count as testing for the licensed Pre-K; however, the Pre-K program should enroll separately for the lead paint testing and asbestos testing to take advantage of the 100% reimbursement for mitigation if lead paint or asbestos is found. *For more information check out the FAQ page at https://www.cleanwaterforuskids.org/en/carolina/ C. Price, Administrator, was present, but T. Nicewonger, NCPre-K Coordinator, and the NCPre-K staff assisted me with the visit. The facility currently operates with a Five-Star Center License, issued 5/10/22, earning points in the following components: Program (7 PTS) + Education (7 PTS) + Quality Point (1 PT) = 15 Points = 5 Stars *Quality Point met and remains in effect: 75% of lead teachers have a BA/BS or higher in early childhood education or child development. The next rated license assessment is due by 5/10/25, but legislation (Senate Bill 425) has extended the “hold harmless” period until the new Quality Rating Improvement System (QRIS) is implemented. At this time, you may choose to complete a rated license assessment using the current rated license process or you may wait until the new QRIS is implemented. The proposed QRIS Modernization rules have been published and are open for public comments. I encourage you to review these and share any feedback you may have with the Child Care Commission. See Additional Comments for links for the proposed rules and contact information for the Commission. Note that the current program assessment tool is changing from the Early Childhood Environment Rating Scale – Revised (ECERS-R) to the Early Childhood Environment Rating Scale – 3 (ECERS-3). Information and training on the new program assessment tool is available on the NC Rating License Assessment Project website at https://ncrlap.org/. This facility is owned by Dare County Schools. Please contact me prior to any information change regarding the ownership of this facility. License information was current. Contact me to make any changes to address, phone, or email or to request changes to the license. The program recently increased staff/child ratios from 1:9 or 2:18 to 1:10 or 2:20. A new permit has been requested which will increase capacity from 54 to 60 children. I monitored the classrooms and spaces used for child care and outdoor play area for applicable child care center requirements using the Child Care Center Requirements, effective 11/1/2024 and Item Numbers Listing, effective 3/2024. I monitored two staff records and all program records. The facility meets DPI requirements for transportation. The NCPre-K requirements in Section .3000 of the child care rules were monitored for compliance in Rooms 109, 111, and 114. I reviewed the NCPre-K Site Monitoring Tool. Staff-child ratios were 1:10 or 2:20 or better, as required in Child Care Rule 10A NCAC 09 .3909. The current NCPre-K staff are NCPK Teachers: K. Flaugher, B. Davis, E. MacDonald and NCPK Teacher Assistants: N. Tarazona, N. Colon, and D. Hahn. I previously monitored other NCPre-K requirements at the Annual Compliance visit on 10/16/24. Center Observations: Parent information was posted outside each classroom. Current activity plans were posted in the classrooms. Children in all three classrooms participated in indoor and outdoor free play with access to defined activity centers where they could play with a variety of toys and materials to stimulate the five developmental domains. A science experiment was set up in Room 109 using colored water and paper towels to test what would happen with the colors from two different jars mixed. In room 111, a group of children cleaned up the block center, returning every block neatly to its assigned place on the block shelf. In Room 114, children were studying shamrocks and rainbows which the teacher stated would transition in to their weather unit next week. I observed and documented the following violations during today’s visit. Violation Number Comment Rule 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. Documentation of outdoor inspections was not available from October 2024 - February 2025. .0605(q) 1041 Prior to employment a Criminal Background Check was not completed. A substitute working in Room 114 had not completed a DHHS Criminal Background Check before working in the NCPre-K classroom. G.S. 110-90.2(b) 1757 A valid qualification letter was not on file and available to review at the facility. A substitute working in Room 114 did not have a Qualifying Letter. G.S. 110-90.2(b) & (d) & .2703(e) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. A teacher assistant hired on August 15, 2024 was required to complete Recognizing and Responding to Suspicions of Child Maltreatment by November 15, 2024 but had not completed it. .1102(g) * Child care programs are expected to achieve and always maintain compliance and are required by NC GS 110-90(4)(d) to achieve and maintain an 18-month compliance history score of at least 75%. Any violations documented may impact the compliance history score. I completed an electronic copy of the visit summary and provided a printed copy of it to you, T. Nicewonger. We reviewed the visit summary and violation(s) documented during today’s visit. Correct these immediately. Send evidence of correction to me so that I receive it no later than 4/1/25. Your compliance verification letter needs to state your facility name and license number and list each violation by Item # and describe in detail when and how each violation has been corrected. Do not include names of children in the compliance letter. Use their initials. Also include the following documents/pictures to verify compliance: copy of Qualification Letter; copy of child maltreatment training certificate Send compliance verification letter in an email from your center’s official email address, nicewongertr@daretolearn.org to my email address, Marjorie.White@dhhs.nc.gov. Technical Assistance With Documented Violations: Outdoor Inspections and Playground Safety Training - A monthly safety check of all the equipment within the facility as a focused task provides an opportunity to notice wear and tear that require maintenance. Documentation of an outdoor inspection was not available for October 2024 – February 2025. When staff realized inspections had not been completed, they completed one for March 2025 and have scheduled them for the rest of the year. The NCPre-K Coordinator is going to have additional staff trained at each school to complete outdoor inspections. Criminal Background Check (CBC) – Criminal Background Checks ensure that staff have not committed any crimes which could potentially make them unsafe to be around children and other staff. Because Abuse and Neglect registries are also checked, it helps to ensure that children are protected from physical, mental, and sexual abuse. Performing diligent background screenings also protects the child care facility against future legal challenges. Bonita Hurdle worked as a substitute for part of the day today in Room 114 for the NCPreK Teacher Assistant. She did not have a Qualifying Letter available for review and when I checked ABCMS, she had not applied for a DHHS Criminal Background Check and Qualifying Letter. She stated that she had completed a CBC with the schools about three years ago. Dare County Schools are still working to get all substitutes and therapists qualified. Because Ms. Hurdle has not completed a DHHS Criminal Background Check and received a CBC Qualification Letter, she may not work in a licensed Pre-K classroom until she is qualified and has provided you with a Qualification Letter. (She may continue to work in the elementary classrooms.) T. Nicewonger, NCPre-K Coordinator, stated that she is helping Ms. Hurdle complete the on-line application today. For your compliance letter, state the date when Ms. Hurdle has completed a DHHS Criminal Background Check and received her Qualifying Letter. The letter may be stored in a file on-site, or off-site in the NCPre-K Coordinator’s office or central office. Include a copy of the Qualifying Letter with the compliance letter. Consider how you will ensure that staff working with the children enrolled in the licensed NCPre-K program who count in staff/child ratio or are alone with children have current DHHS Criminal Background Checks and qualifying letters. Provide a list of substitutes who have completed the DHHS Criminal Background Check and have received a qualifying letter to the principal/staff member at each school who assigns substitutes. *K. Dyer is the Assistant Principal. She assists the NCPre-K classrooms. If she is ever alone with NCPre-K children or counts in staff/child ratios, she needs to also have a Criminal Background Check. *Federal requirements require all staff who work in licensed afterschool or pre-k programs operated by public schools complete a DCDEE Criminal Background Check (CBC) before hire and then every five years. This includes administrators, teachers, teacher assistants, substitutes, one-on-one assistants, therapists if therapy does not take place in the classroom, and bus drivers. Recognizing and Responding to Suspicions of Child Maltreatment – Recognizing and Responding to Suspicions of Child Maltreatment teaches citizens and professionals the signs and symptoms to look out for with child abuse and neglect, what to do when they suspect it is happening, NC’s reporting law, and what happens after a report is made. N. Tarazona was required to complete the training by 11/15/24 but had not completed it yet. You stated that she forgot about the deadline. I will email a list of required trainings/tasks that need to be completed at hire and throughout a new employee’s first year. General Visit Information: As a licensed child care facility, it is your responsibility to be knowledgeable of the child care laws and rules and to teach your staff the rules to ensure your center remains in compliance. The most recent versions of laws and rules regarding child care facilities in North Carolina are available on the DCDEE website, www.ncchildcare.ncdhhs.gov. You can also review the “What’s New” section and download a copy of the Items Number Listing which you can use as a checklist for your program. Check the designated facility email at least weekly to stay current with communication and email blasts from DCDEE. The Raise NC Newsletter, which is sent to your facility email almost weekly, provides the most current information and updates from the Division and available training opportunities. Consider creating an email folder for it so that you can refer back to it after you read it. Your local and regional Smart Start Partnerships and Child Care Resource and Referral Agencies and Child Care Health Consultants provide general training to meet on-going training hours, health and safety training requirements, Environment Rating Scale training and training specific to your center’s needs. Staff Worksheets – We reviewed your staff worksheet. I highlighted items that were delinquent and items that are coming due in the next year. *CPR/First Aid for K. Flaugher and N. Colon expire 8/31/25 *H&S Training – due for N. Tarazona by 8/15/25 On-going Training Hours – Review Row 18 (# hours to carry over) on the Staff Worksheets to determine how many on-going training hours returning staff will need by 10/16/25 (anniversary of last Annual Compliance visit). *After their first year and completing H&S Training, NCPre-K Teacher Assistants are required to complete 15 hours of on-going training. *ECE college coursework or coursework leading to an ECE degree counts towards on-going training hours. Each semester credit counts as 16 hours so a 3-credit classes also earns 48 hours of on-going training. *Use the On-going Training Log to document training annually. Attach copies of certificates. Staff Education and Works – N. Colon and N. Tarazona need to finish Works. Log into Works. Request to be approved as a DPI Teacher and DPI Teacher Assistant. Upload completed DPI Teacher/Teacher Assistant Education form and copy of original, official transcripts. *N. Colon is expecting to finish her AAS-ECE degree in May 2025. Finish Works registration when final transcripts are available so that the full AAS degree is finished. Water Bottles – All water bottles need to be labeled with the child’s name and sent home daily to be washed and refilled. Offices – If cleaning supplies are stored in the teacher offices between classrooms, store them in a locked cabinet. The next rated license assessment is due by 5/10/25, but legislation (Senate Bill 425) has extended the “hold harmless” period until the new Quality Rating Improvement System (QRIS) is implemented. At this time, you may choose to complete a rated license assessment using the current rated license process or you may wait until the new QRIS is implemented. The proposed QRIS Modernization rules have been published and are open for public comments. I encourage you to review these and share any feedback you may have with the Child Care Commission. I monitored the classrooms and spaces used for child care and outdoor play area for applicable child care center requirements using the Child Care Center Requirements, effective 11/1/2024 and Item Numbers Listing, effective 3/2024. I monitored two staff records and all program records. The facility meets DPI requirements for transportation. The NCPre-K requirements in Section .3000 of the child care rules were monitored for compliance in Rooms 109, 111, and 114. I reviewed the NCPre-K Site Monitoring Tool. Staff-child ratios were 1:10 or 2:20 or better, as required in Child Care Rule 10A NCAC 09 .3909. The current NCPre-K staff are NCPK Teachers: K. Flaugher, B. Davis, E. MacDonald and NCPK Teacher Assistants: N. Tarazona, N. Colon, and D. Hahn. I previously monitored other NCPre-K requirements at the Annual Compliance visit on 10/16/24. Center Observations: Parent information was posted outside each classroom. Current activity plans were posted in the classrooms. Children in all three classrooms participated in indoor and outdoor free play with access to defined activity centers where they could play with a variety of toys and materials to stimulate the five developmental domains. A science experiment was set up in Room 109 using colored water and paper towels to test what would happen with the colors from two different jars mixed. In room 111, a group of children cleaned up the block center, returning every block neatly to its assigned place on the block shelf. In Room 114, children were studying shamrocks and rainbows which the teacher stated would transition in to their weather unit next week. I observed and documented the following violations during today’s visit. A substitute working in Room 114 had not completed a DHHS Criminal Background Check before working in the NCPre-K classroom. G.S. 110-90.2(b) 1757 A valid qualification letter was not on file and available to review at the facility. A substitute working in Room 114 did not have a Qualifying Letter. G.S. 110-90.2(b) & (d) & .2703(e) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. A teacher assistant hired on August 15, 2024 was required to complete Recognizing and Responding to Suspicions of Child Maltreatment by November 15, 2024 but had not completed it. .1102(g) * Child care programs are expected to achieve and always maintain compliance and are required by NC GS 110-90(4)(d) to achieve and maintain an 18-month compliance history score of at least 75%. When staff realized inspections had not been completed, they completed one for March 2025 and have scheduled them for the rest of the year. The NCPre-K Coordinator is going to have additional staff trained at each school to complete outdoor inspections. Criminal Background Check (CBC) – Criminal Background Checks ensure that staff have not committed any crimes which could potentially make them unsafe to be around children and other staff. Because Abuse and Neglect registries are also checked, it helps to ensure that children are protected from physical, mental, and sexual abuse. Performing diligent background screenings also protects the child care facility against future legal challenges. Bonita Hurdle worked as a substitute for part of the day today in Room 114 for the NCPreK Teacher Assistant. She did not have a Qualifying Letter available for review and when I checked ABCMS, she had not applied for a DHHS Criminal Background Check and Qualifying Letter. She stated that she had completed a CBC with the schools about three years ago. Dare County Schools are still working to get all substitutes and therapists qualified. Because Ms. Hurdle has not completed a DHHS Criminal Background Check and received a CBC Qualification Letter, she may not work in a licensed Pre-K classroom until she is qualified and has provided you with a Qualification Letter. (She may continue to work in the elementary classrooms.) T. Nicewonger, NCPre-K Coordinator, stated that she is helping Ms. Hurdle complete the on-line application today. For your compliance letter, state the date when Ms. Hurdle has completed a DHHS Criminal Background Check and received her Qualifying Letter. The letter may be stored in a file on-site, or off-site in the NCPre-K Coordinator’s office or central office. Include a copy of the Qualifying Letter with the compliance letter. Consider how you will ensure that staff working with the children enrolled in the licensed NCPre-K program who count in staff/child ratio or are alone with children have current DHHS Criminal Background Checks and qualifying letters. Provide a list of substitutes who have completed the DHHS Criminal Background Check and have received a qualifying letter to the principal/staff member at each school who assigns substitutes. *K. Dyer is the Assistant Principal. She assists the NCPre-K classrooms. If she is ever alone with NCPre-K children or counts in staff/child ratios, she needs to also have a Criminal Background Check. *Federal requirements require all staff who work in licensed afterschool or pre-k programs operated by public schools complete a DCDEE Criminal Background Check (CBC) before hire and then every five years. Lunch provided by the school was chicken strips with waffles, peas, and diced peaches with milk. Some children bring their lunch and have a nutrition opt-out form available. The children played more after lunch before resting on linen-covered cots. Both teachers reported that none of their students require medications at school this year. Interactions were warm and respectful. I observed and documented the following violations during today’s visit. The next rated license assessment is due by 5/10/25, but DCDEE has developed a two-year cohort system to return all facilities back to rated license reassessments. Your center has been assigned to Cohort 3. The Cohort 3 Rated License Preparation Year is from 7/1/2025 – 6/30/2026. You may complete your ECERS-R during the Preparation Year and choose to use your scores and complete the rated license reassessment if education is where you want it to be. Center Observations: Classroom information was posted in the hallways outside the classroom doors. Both classrooms were studying insects this week. Artwork from the morning, stories at circle time and activity centers reflected the theme. Children participated in large group and free play activities with small group time offered during free play. Two outdoor times provided opportunities for active play. Children played on complex climber and with the portable gross motor equipment. Lunch was served in the classrooms. Lunch provided by the school was chicken strips with waffles, peas, and diced peaches with milk. Some children bring their lunch and have a nutrition opt-out form available. The children played more after lunch before resting on linen-covered cots. Both teachers reported that none of their students require medications at school this year. Interactions were warm and respectful. I observed and documented the following violations during today’s visit. Nicewonger, NCPre-K Coordinator. She stated that Dare County Schools are still working to get all substitutes and therapists qualified. Federal requirements require all staff who work in licensed afterschool or pre-k programs operated by public schools complete a DCDEE Criminal Background Check (CBC) before hire and then every five years. This includes administrators, teachers, teacher assistants, program coordinators, group leaders, assistant group leaders, substitutes, one-on-one assistants, therapists if therapy does not take place in the classroom, and bus drivers. White and A. Parrish. I monitored two children’s files for completed health assessments (dental, hearing, and vision), developmental screenings, and on-going instructional assessments. The facility uses the Dial-4 developmental screening which were completed for children in June 2023 or as they entered the program throughout the year. The NCPreK program uses Creative Curriculum and Teaching Strategies Gold as the on-going instructional assessment tool to document evidence of each child’s progress three times throughout the school year: Fall, Mid-Winter, Spring and had completed Checkpoint 1. The NCPre-K operates daily from 9/5/23 – 5/24/24 from 7:55am – 2:35pm. Parent conferences are held twice a year. Parents are registered for Ready Rosie, but you also use Class DoJo app to communicate with families, along with daily notes that goes back and forth with reminders, and information for families and from families. The NCPreK program completed a modified Early Childhood Environment Rating Scale – Revised (ECERS-R) on 4/28/22 and scored 6.26. Center Observations: Parent information was posted on the bulletin boards outside each classroom. The children in Room B114 were learning about scarecrows, the number 9 and the letter “Cc”. After reading a story about a scarecrow that wasn’t too scary, the teacher reviewed the number and letter of the week. The circle time was very interactive and positive, with children dancing and singing, answering questions and counting together. One child was allowed to play quietly off to the side where he could still hear and participate but had some playdough and tools to keep his hands busy. The children in Room 111 were studying the five senses as observed on the posed activity plan and reflected in the classroom materials. They were on the playground climbing on the stationary climber and using other gross motor materials and indoor center materials. Teachers supervised the climber closely and reminded a group of children to go down the slide feet first instead of head first. `Lunch was hot dog on bun, French fries, fruit cup, and milk. Some children brought lunches from home. They were refrigerated before lunch. The children had Nutrition Opt-out forms in the files. After lunch, each group had more free play and outdoor time before resting on linen-covered cots. Teachers used appropriate guidance techniques to manage challenging behaviors. All children were treated with kindness and respect. I observed and documented the following violation during today’s visit. Monitoring included NC Pre-K requirements in the NC Child Care Rules Section .3000 which are applicable to Rooms B116 and B109 are approved primary spaces but is not currently being used for Pre-K. Last Annual Compliance visit – 1/4/23 Last Sanitation Inspection – 5/24/23 with a Superior Rating; expires 5/24/24 Last Fire Inspection – 4/11/23 with a Passed with Comments rating; expires 4/11/24 *Sanitation and fire inspections are required at least annually by the date of the previous inspection. I monitored the classrooms and spaces used for child care and outdoor play area for applicable child care center requirements using the Child Care Center Requirements, effective 7/1/2023 and Item Number Listing, effective 8/2023. I monitored four children’s records, two staff records and all program records. The facility meets DPI requirements for transportation. The NCPre-K requirements in Section .3000 of the child care rules were monitored for compliance in Rooms B114 and B111. The NC Pre-K Site Monitoring Tool was completed and available for review. Staff-child ratios were 1:9 or 2:18 or better, as required in Child Care Rule 10A NCAC 09 .3909. The current NCPre-K staff are NCPK Teachers: K. Flaugher and B. Davis and NCPK Teacher Assistant: A. White and A. Parrish. I monitored two children’s files for completed health assessments (dental, hearing, and vision), developmental screenings, and on-going instructional assessments. The facility uses the Dial-4 developmental screening which were completed for children in June 2023 or as they entered the program throughout the year. The NCPreK program uses Creative Curriculum and Teaching Strategies Gold as the on-going instructional assessment tool to document evidence of each child’s progress three times throughout the school year: Fall, Mid-Winter, Spring and had completed Checkpoint 1. The NCPre-K operates daily from 9/5/23 – 5/24/24 from 7:55am – 2:35pm. Parent conferences are held twice a year. Parents are registered for Ready Rosie, but you also use Class DoJo app to communicate with families, along with daily notes that goes back and forth with reminders, and information for families and from families. The NCPreK program completed a modified Early Childhood Environment Rating Scale – Revised (ECERS-R) on 4/28/22 and scored 6.26. Center Observations: Parent information was posted on the bulletin boards outside each classroom. The children in Room B114 were learning about scarecrows, the number 9 and the letter “Cc”. After reading a story about a scarecrow that wasn’t too scary, the teacher reviewed the number and letter of the week. The circle time was very interactive and positive, with children dancing and singing, answering questions and counting together. One child was allowed to play quietly off to the side where he could still hear and participate but had some playdough and tools to keep his hands busy. The children in Room 111 were studying the five senses as observed on the posed activity plan and reflected in the classroom materials. They were on the playground climbing on the stationary climber and using other gross motor materials and indoor center materials. Teachers supervised the climber closely and reminded a group of children to go down the slide feet first instead of head first. `Lunch was hot dog on bun, French fries, fruit cup, and milk. Some children brought lunches from home. They were refrigerated before lunch. The children had Nutrition Opt-out forms in the files. After lunch, each group had more free play and outdoor time before resting on linen-covered cots. Teachers used appropriate guidance techniques to manage challenging behaviors. All children were treated with kindness and respect. I observed and documented the following violation during today’s visit. Violation Number Comment Rule 1898 Staff did not complete the health and safety training within one year of employment. A staff person hired on September 6, 2022 did not complete the required Health and Safety Training modules by September 6, 2023. .1102(a) * Child care programs are expected to achieve and always maintain compliance and are required by NC GS 110-90(4)(d) to achieve and maintain an 18-month compliance history score of at least 75%. Any violations documented may impact the compliance history score. I completed an electronic copy of the visit summary and provided a printed copy of it to T. Nicewonger, NCPre-K Coordinator. We reviewed the visit summary and violation documented during today’s visit. Correct this immediately. Send evidence of correction to me so that I receive it no later than 11/20/2023. The compliance verification letter needs to state the facility name and license number and list each violation by Item # and describe in detail when and how each violation has been corrected. Do not use names of staff in the compliance letter. Use initials or position.
