Jennifer Bradshaw-Garrett, Child Care Consultant, accompanied me on the visit today. S. Huit, Administrator, assisted me with the visit. Your program currently operates with a three-star license, issued 5/27/23, earning 4 points in the education component, 2 points in the program standards component (meeting enhanced space) and 1 quality point for the child care administrator has 10 years of documented administrative work experience in a licensed child care center that can be verified by the Division. The last annual compliance visit was conducted 2/18/25. The sanitation inspection was completed 10/7/25 with a “Superior” classification. The last fire inspection was conducted 9/23/25 with a Satisfactory rating and approval for daytime care only. Three-year water testing due 7/19/26. The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred percent as of 1/23/26. This facility is owned by Manteo First Assembly of God, Incorporated was listed as current- active. License and contact information were current. Contact me to request any changes to your license or contact information. I monitored the classrooms and spaces used for child care and outdoor play areas for applicable child care center requirements using the Child Care Center Requirements, effective 7/1/2025 and Item Numbers Listing, effective 4/2025. I monitored one staff record, three children’s records and all program records. Children participated in indoor and outdoor activities even with the cold weather. In Space 4 & 5, the class is working on a personal Alphabet Book and were studying the Letter “Q”. The children in Space 1 took a walking field trip to the library for preschool story hour and made their own snow globes. In Space 6, the children engaged in free play while the teacher worked one-on-one with them on the smart table. Lunch was observed. Lunches are brought from home, but the center supplements lunches if required components are not brought from home. I observed and documented the following violations during today’s visit. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In Space 4 & 5, a medication permission form was not available for Libervant. The staff reported they had not administered the medication. 10A NCAC 09 .0803(1)(a & b) 847 Parent's medication authorization did not include required information. In Space 4 & 5, a medication permission form for acetaminophen was labeled as "Tylenol" when the medication available was "DG Pain and Fever Relief" and the authorization time frame was for twelve months instead of six months. A medication permission form for Banana Boat sunscreen did not include the twelve-month authorization dates. 10A NCAC 09 .0803(4)(6-9) * Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violations documented must be corrected immediately. On or before 2/11/26, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware that any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance With Documented Violations: Medication Permission Forms – To ensure the health and safety of children using medication, check medications consistently to verify that all information is included on the medication permission forms and medical action plans. In Space 4 & 5, a medication permission form was not available for Libervant, an emergency anti-seizure medication. The medical action plan was available. In addition, a medication permission form stated that a fever reducer available in the locked medication box called DG Children’s Fever and Pain Relief was Tylenol. The authorization period was for the OTC medication was 8/11/25 – 8/11/26 (12 months) instead of 8/11/25 – 2/11/26 (six months). A medication permission form for Banana Boat sunscreen did not include a 12-month authorization period. The administrator did not realize that there was no medication permission form for the Libervant and that the medication permission form for the acetaminophen and sunscreen did not include the required information. The medication permission forms provide additional information that could be critical to the administration of the medication and the care of the child. During the visit you printed a medication permission form to have the parent complete for the Libervant and stated that you will have parent update the medication permission forms for the acetaminophen and sunscreen. For your compliance letter, state the date the parents provided you with a current, completed medication permission forms. New staff need to complete Recognizing and Responding to Suspicions of Child Maltreatment and CPR/First Aid within three months of hire and the rest of the training modules within twelve months. Health and Safety Training hours can also count towards on-going training hours. * On-going Training Hours – Review Row 18 (# hours to carry over) on the Staff Worksheets to determine how many on-going training hours returning staff will need by 1/28/27, anniversary of today’s Annual Compliance visit). Center Observations: Parent information was posted on the bulletin board by the door that parents use to leave the child care. Children participated in indoor and outdoor activities including group activities and individual free play. Activity plans were posted. Children ate lunch in their classrooms. Lunches are brought from home. After lunch, children rested on linen-covered mats. Interactions were warm and nurturing. I observed and documented the following violations during today’s visit. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. In Space 4 &5, a medication permission form for Benadryl was dated April 4, 2023 - October 4, 2023 and was expired. 10A NCAC 09 .0803(4)(6-9) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. An inhaler belonging to a child who was no longer enrolled since September 5, 2023 was observed in Space 1 and was not returned to the parent. .0803(12) * Child care programs are expected to achieve and always maintain compliance and are required by NC GS 110-90(4)(d) to achieve and maintain an 18-month compliance history score of at least 75%.
