This visit was conducted with Rebekah Daniels, Director. Pam Hauser, Licensing Supervisor was also present for today’s visit. Currently this center operates with a Four (4) Star license. The restrictions include daytime care only, children under 2 ½ years and younger in rooms with a direct exit and meets enhanced ratios. The sanitation inspection was completed on August 7, 2024. The facility received a superior rating with no demerits. The fire inspection was completed February 7, 2025. I received a copy prior to today’s visit. This facility is owned and operated by Locomotion Learning LLC. The Secretary of State website was checked prior to today’s visit and the corporation is listed as current and active. Forty-three (43) children were present during today’s visit. The children were observed playing indoors, completing routine tasks, and participating in group activities. The emergency drill log was monitored during today's visit. The last fire drill recorded was completed on June 6, 2025, with two (2) minutes and thirty (30) seconds to evacuate. The last emergency drill, a lockdown drill, was completed on April 24, 2025. The emergency drills must be completed every three (3) months. The staff files were not reviewed today. There are four (4) new staff present today. All staff have a current qualifying letter. Ms. Daniels stated that no children enrolled have emergency medication. They do not provide transportation. The following violations were cited today. Violation Number Comment Rule 815 Electrical cords were accessible to infants and toddlers. In space 1, there were two (2) cords accessible to the toddlers. 10A NCAC 09 .0604(f) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space 2, there was a container of baby powder that was in an unlocked cabinet. There were multiple warnings on the label. .2820(b) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. In space 5, there was a tube of Nystatin that did not have a prescription label or accompanied by a doctor's note. .0803(2)(a) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In space 1, there were two (2) sunscreens and one (1) diaper cream authorization that expired in 2024. .0803(12) Consultation: The new employee, hired June 13, 2025, must be added to the staff roster in the ABCMS system within five (5) business of hire. Health and Safety training topics, except for Recognizing and Responding to Suspicions of Child Maltreatment and CPR and First Aid trainings, must be completed within the first year of employment. The Recognizing and Responding, CPR, and First Aid trainings must be completed within ninety (90) days of employment. Then these trainings must be completed every five (5) years after the initial completion, except CPR and First Aid, which must be completed every two (2) years. There is a wooden slat that replaced a window pane in the bathroom off of space 4. The slat is loose and can be pulled away from the window frame. It needs to be more secure to the window frame. The fence is beginning to show signs of rust. Please make sure that you sand and paint those areas. The wooden railing at the steps outside space 5 coming into the outdoor play area is loose and is beginning to split. Please replace any split wood and make it studier. Rated License An email will be sent with information on when the rated license process will begin. Compliance: You must correct the violation found during today's visit immediately. Please send me a two (2) letters verifying compliance. The letters must be received by July 1, 2025. Please include in your letter each violation number and how you corrected each violation and what plan will be implemented to prevent these violations from occurring again. The staff and children’s files were monitored in accordance with the Division of Child Development and Early Education policies and procedures. Ms. Daniels stated that a child enrolled has emergency medication. The facility does not provide routine transportation. The following violations were observed during today's visit. The activity plan used in space 5 only included three of the required activities. .0508(g)(2) 608 Children did not wash their hands upon arrival at the center, after each visit to the toilet, before eating, before and after water activity play, after outside play, and after handling animals or animal cages. In space 2 no child washed their hands prior to lunch. They were observed touching books and toys around the classroom. 15A NCAC 18A .2803(c) 847 Parent's medication authorization did not include required information. In space 1 there was no parent signature for the authorization of an epipen. 10A NCAC 09 .0803(4)(6-9) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Staffing patterns can help to be prepared for when staff are absent and/or running late when determining how to maintain staff/child ratios and group sizes. Consultation: 1-The health and safety trainings are required to be completed within the first year of employment, with the exception of Recognizing and Responding to Suspicions of Child Maltreatment, CPR and First Aid training. These three (3) trainings must be completed within ninety (90) days of employment. Following the initial completion of the health and safety trainings they must be completed within the next five years of employment or prior to expiration (CPR and First Aid). There is a training log available on the Division’s website to document the completion of the trainings. All topics must be covered. You may access health and safety trainings for free on the Division’s website under moodle. 2-Child Care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. Provider Access to ABCMS allows administrators to: •See the real-time background check status of staff members •Run a printable report of the staff roster to assist with compliance visits •See new background check applicants and add to staff roster To get started, complete the ABCMS Child Care Provider Portal Training in Moodle. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The children were observed playing indoors, playing outdoors and completing routine tasks. The emergency drill log was monitored during today's visit. The last fire drill recorded was completed on June 10, 2024, with two (2) minutes and eighteen (18) seconds to evacuate. The last emergency drill, a lockdown drill, was completed on April 30, 2024. The staff files were not reviewed today. There is one (1) new staff. All staff have a current criminal background check. Ms. Daniels stated that children enrolled have emergency medication. This is a choking hazard. 10A NCAC 09 .0601(a) Consultation: Health and Safety training topics, except for Recognizing and Responding to Suspicions of Child Maltreatment and CPR and First Aid trainings, must be completed within the first year of employment. The Recognizing and Responding, CPR, and First Aid trainings must be completed within ninety (90) days of employment. Then these trainings must be completed every five (5) years after the initial completion, except CPR and First Aid, which must be completed every two (2) years. As new staff are hired, and current staff are completing more class coursework make sure they are updating their WORKS accounts. If they do not have a WORKS account they need to create one, request for evaluation and send in all official transcripts. We reviewed the emergency medical care plan that was posted. We discussed what they would do if the person they had listed was not present and available to carry out the duties they were assigned. Ms. Daniels stated that they would look at the form and see if they need to update the information. If changes are made make sure that it is reviewed with all staff and the date of the review is recorded. Technical Assistance: While observing the children playing outdoors we observed a toddler climbing the fence several times. The teacher redirected him. I discussed with Ms. Daniels there are no climbing toys on the outdoor play area for toddlers. I also observed three (3) small patches of gravel on the outdoor play area for toddlers. Ms. Daniels stated that she could purchase an appropriate climbing piece of equipment and add mulch for the equipment to the area that would cover the patches of gravel. Compliance: You must correct the violations found during today's visit immediately. Please send me two (2) letters by mail verifying compliance, or you may email me a signed letter. It must be received by July 9, 2024. The allegation is as follows: There are concerns regarding nurture and care . During today’s visit, the allegations were discussed with you. You had the opportunity to respond. Information shared during this visit are documented in this visit summary. Your program currently operates with a Four (4) Star License. Restrictions include daytime care only, no cooking allowed in building, meets enhanced ratios, children under 2 1/2 years old in rooms with direct exit only. Today, the following items, in addition to the allegations, were monitored: • Supervision • Staff / Child Ratio • Adequate / Approved Space • License Posted • Permit Restrictions Allegation: There is a concern regarding nurture and care. Observation and/or Interviews: We monitored each child care indoor space. We also reviewed video from April 1, 2024, and video from today for space 2. We interviewed staff member from space 2 and Ms. Daniels. We asked Ms. Daniels if she had any concerns about what we observed of the videos. She stated that she did have concerns about the diaper changing that we observed on the video. I asked if she had concerns about how the teacher was handling the children and she stated “yes”. In the video from April 1, 2024, we observed the teacher in space 2, grab a child and put him on her lap to change a pull-up. We observed the teacher lifting the child by his leg and changing his pull up while still holding his leg up. The child’s upper body was hanging off the teacher’s lap and there was no support for his head or shoulders. After the pull up was changed neither the child or teacher washed their hands. Later in the April 1st video we observed two (2) children fighting over child-size chairs. We observed the teacher go over and grab the chair out of the child’s hands and put the chairs back at the tables. The teacher then was seen grabbing one of the children by the arm and placing him in a different activity area from the other child. Today, we observed space 2 and the teacher/child interactions. We observed a child biting another child. The director stopped the child from biting and told the child “biting hurts. We use our mouth for eating not biting.” We also heard the teacher repeating the “we use our mouths for eating not biting.” We observed another incident of a child hitting another child and again the director stepped in and redirected the children. The teacher did not interact with either child during this second incident. When we stepped out the classroom to observe outdoors. Upon returning to the classroom we observed the teacher sitting outside the bathroom while a child was in the bathroom crying. I asked the teacher why the child was crying. She stated that the child and another child had gotten into it. We asked Ms. Daniels to play the video of today around the time of us returning to the classroom. We observed one child biting another child. While the teacher was attending to the child that was bitten the biter ran around the room and hid from the teacher by the tables. The child that was hiding got up and went around the table to get away from the teacher. We saw the teacher turn around and walk toward the bathroom and the child that had bitten was seen walking toward the bathroom. At no time during the video did we see the child that had bitten crying or upset. We observed the child walk into the bathroom and then seen the teacher lean into the bathroom. We observed the teacher sit down in the chair outside the bathroom and that is when we entered the classroom and saw the child crying in the bathroom. We had Ms. Daniels to ask the teacher about the child crying in the bathroom. The teacher told Ms. Daniels that he did not want to sit on the potty and that is why he was crying. That is different then what she told us when we asked her why he was crying. Conclusion: Based on the observations the allegation is substantiated. The following violation was cited today: Violation Number Comment Rule 402 Each child's diapers were not changed at areas designated exclusively for diapering, on a surface that was smooth, nonabsorbent, easily cleanable and of tight construction. Restrictions include daytime care only, no cooking allowed in building, meets enhanced ratios, children under 2 ½ years old in rooms with direct exit only. This is the first annual compliance visit at this facility. The sanitation inspection was completed on October 31, 2023, with a “superior” classification with no demerits. The fire inspection was completed on November 4, 2022. The new fire inspection must be sent to me as soon as it is completed. The facility is owned and operated by Locomotion Learning, LLC. The Secretary of State website was checked and the corporation is listed as current and active. Thirty (30) children were present. The children were observed playing in activity areas, completing routine tasks, having diapers changed, eating lunch and napping. There are no infants enrolled. All indoor and outdoor areas were monitored. Fire drill reports and playground inspections were monitored during today's visit. The last fire drill was conducted on December 13, 2023. It took the children two (2) minutes and forty (40) seconds to evacuate. The last emergency drill, a shelter-in-place drill, was conducted on October 31, 2023. Restrictions include daytime care only, no cooking allowed in building, meets enhanced ratios, children under 2 ½ years old in rooms with direct exit only. This is the first annual compliance visit at this facility. The sanitation inspection was completed on October 31, 2023, with a “superior” classification with no demerits. The fire inspection was completed on November 4, 2022. The new fire inspection must be sent to me as soon as it is completed. The facility is owned and operated by Locomotion Learning, LLC. The Secretary of State website was checked and the corporation is listed as current and active. Thirty (30) children were present. The children were observed playing in activity areas, completing routine tasks, having diapers changed, eating lunch and napping. There are no infants enrolled. All indoor and outdoor areas were monitored. Fire drill reports and playground inspections were monitored during today's visit. The last fire drill was conducted on December 13, 2023. It took the children two (2) minutes and forty (40) seconds to evacuate. The last emergency drill, a shelter-in-place drill, was conducted on October 31, 2023. During today’s visit, the allegations were discussed with you. You self reported this allegation. You had the opportunity to respond. Information shared during this visit are documented in this visit summary. Your program currently operates with a Four Star License. Restrictions include daytime care only, children under 2 ½ years in rooms with direct exit only, no cooking allowed, and meets enhanced ratios. The program’s compliance history was 94% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • Adequate / Approved Space • License Posted • Permit Restrictions Allegation: There is a concern regarding supervision. Observation and/or Interviews: I observed each classroom space for supervision. We also reviewed video for August 2 and August 3 for space 1. There were instances observed on video for both days of the teachers sitting at the children’s table or in a camp outdoor chair and could not see the children playing in the dramatic area or the block area. On the video of August 3, 2023, the teacher was observed engaged in cleaning and other non-teaching activities throughout the video replay instead of interacting with the children. Conclusion #1: Based on the above information, this allegation was substantiated. The following violation was cited during today’s visit: Violation Number Comment Rule 303 Children were not adequately supervised at all times.
