This visit was conducted with Rebecca Mensel, Lead Teacher. Currently this center operates with a Five Star license. Restrictions include daytime care only and meets all enhanced requirements and reduced ratio. This facility is owned and operated by Rockingham County Schools. The sanitation inspection was completed September 18, 2025. The facility received a superior rating with three (3) demerits. The last fire inspection on file is September 2024. Ms. Mensel stated that they inspected in October but could not locate the inspection form. She will need to send me a copy of the inspection form with the compliance letter. Upon arrival staff/child ratio was observed and in compliance as indicated on the attached enrollment worksheet. Children were adequately supervised during today’s visit. All indoor and outdoor areas were observed during today's visit. Nine (9) children were present during today’s visit. The children were observed playing indoors and outdoors, and completing routine tasks. Fire drill reports and playground inspections were monitored during today's visit. The last fire drill was completed March 4, 2026. It took the children one (1) minute to evacuate. The last emergency drill, a shelter in place drill, was completed on today, March 11, 2026. Emergency drills must be conducted every three (3) months. Children’s and staff files were reviewed according to the policy set by the Division of Child Development and Early Education. Ms. Mensel stated that no child enrolled has emergency medication. Ms. Mensel stated that they do not provide routine transportation. The NC Pre-K requirements were monitored today. The facility currently has seven (7) NC Pre-K children. The following violations were observed during today's visit. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. The net on the rock climber is torn. G.S. 110-91(6); .0601(b) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. There is an aerosol can in an unlocked cabinet in the bathroom alcove. Also the key was left in the lock. .2820(b) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The principal that began working at this facility in October 2025 is not listed in the ABCMS system on the staff roster. The teacher is also not listed on the staff roster in the ABCMS system. G.S. 110-90.2 & .2703(r) Rated License Information: This facility completed a rated license assessment on March 26, 2025. They will be due for a rated license reassessment in March 2028. Compliance: You must correct the violation found during today's visit immediately. Please send me two (2) letters verifying compliance. The letters must be received by March 25, 2026. Please include in your letter each violation number and how you corrected each violation and what plan will be implemented to prevent these violations from occurring again. Please include the name of your center and ID number, date, and the title of the person who signs the letter. Please send the letter to: Sally Allen, PO Box 1305, Eden, NC 27289. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. Michelle Casto, the Principal, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Casto. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 830 A First Aid information sheet was not posted in a place for referral and/or the information sheet did not include all the required information. A First Aid information sheet was posted but was not up to date with all of the required information such as First Aid/CPR information for children of all ages. .0802(h) 832 There was no written emergency medical care (EMC) plan. The facility did not have an emergency medical care plan posted and in a visible location for review, that would identify specific persons to respond to the medical care/needs of children as required in a timely manner. 10A NCAC 09 .0802(a) 1041 Prior to employment a Criminal Background Check was not completed. The administrator did not complete a criminal background check prior to employment. G.S. 110-90.2(b) Violations must be corrected immediately. Within one week, January 19, 2026 you must submit documentation of the corrections you made and your plan to maintain compliance with the identified childcare requirements to me, Rhonda Grady, Investigations Consultant, via email at rhonda.m.grady@dhhs.nc.gov.
