Evan Moore, Site Director assisted me with the visit. The program operated with a three-star license, issued December 9, 2019, earning six points for education, two points for program standards, and one quality point for complying with a programmatic option. License restrictions include daytime care only, school age children only, playground area does not meet child care playground safety standards, meets enhanced ratios, and meets enhanced space. The last annual compliance visit was conducted on January 10, 2024. The sanitation inspection was completed November 5, 2024, with a “Superior” classification. The last fire inspection was conducted April 8, 2024, and your facility was approved for daytime care only. The program’s compliance history was one hundred percent as of November 13, 2024. Children were observed children transitioning to after school care, homework, and group activities. The Group Leaders were observed interacting with children while they played. Age-appropriate materials and equipment were accessible to the children. Children were adequately supervised. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). One staff did not complete and submit required forms for a criminal background check before the previous qualification letter expired. G.S. 110-90.2(b) & .2703(n)&(o) 1757 A valid qualification letter was not on file and available to review at the facility. L. Sammons did not have a current qualification letter on file or available for review. G.S. 110-90.2(b) & (d) & .2703(e) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. On or before November 29, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when was corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. Mail or email the information to: Nakita Bellamy, Child Care Consultant 3059 N. If no qualification letter on file by the end of the 15th day, L. Sammons will not be able to return to work until a qualification is on file and submitted. Health and Safety Trainings: Health and Safety training requirements were monitored. All staff have completed the required Health and Safety Trainings. I verified the following information: Mailing address, telephone number, email address, and ownership. There were not any changes. At the completion of the visit, this visit summary was reviewed, printed, and a copy left with you.
