Sonya Morris, Administrator, accompanied me on a walk-through of the premises. The program operated with a four-star license, issued April 6, 2018, earning seven points for education, two points for program standards, and one quality point for complying with a programmatic option. License restrictions include daytime care, second shift, overnight, meets enhanced ratios, and meets enhanced space. The last annual compliance visit was conducted on February 11, 2025. The sanitation inspection was completed September 17, 2024, with a “Superior” classification. The last fire inspection was conducted on January 30, 2025, and your facility meets the satisfactory fire safety conditions. The program’s compliance history was ninety-five percent as of January 5, 2026. The NC Secretary of State website was reviewed on January 5, 2026, and Precious Angels’ Day Care, INC was listed as current- active. Upon my arrival, children were observed during free play activities. Children and teacher were observed during outdoor free play. Age-appropriate materials and equipment were accessible to children. Children were adequately supervised. Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. The last Annual was conducted on February 13, 2024. The Annual Compliance Monitoring Checklist for Child Care Centers was used during the visit to note items that were monitored. Upon my arrival I was greeted by the cook, Tonya Johnson. She informed me Mrs. Sonya Morris, owner/administrator, was on the way. You currently operate with a 4-star rated license issued April 16, 2018 with 2 points earned in Program Standards, 7 points in Education Standards and 1 point in Quality by meeting a programmatic option. Restrictions: 1st, 2nd, 3rd shift, meets enhanced ratios and enhanced space. Upon review of the fire inspection completed on 1-30-2025, it was noted on the inspection form you are approved for day time care only. Make sure to contact your Child Care Consultant, Nakita Bellamy, if you are no longer providing overnight care. All indoor areas used by the children were monitored. Due to inclement weather the outdoor playground areas were not monitored today. Monthly playground inspections were reviewed. I observed staff/child ratios, group size, capacity, use of space, and supervision. I observed children playing in various activity centers, teacher lead activities, free play, preparing and eating lunch. Today’s lunch consisted of chicken barbeque, corn, fruit cocktail, whole grain bread and milk. Developmentally age-appropriate materials were used. A sanitation inspection was completed 9-17-2024 with 7 demerits and a superior rating. A fire inspection was completed on 1-30-2025 with approval for day time care only. The Emergency Drill log was reviewed during today’s visit. Prior to today’s visit the staff and training worksheets were obtained. A review of the information was completed. Your signature on the worksheet verifies you are giving true, accurate and complete information for the files that were not reviewed. Two staff file were reviewed during today’s visit. During today’s visit, four children’s records were reviewed. The children’s records form was completed during the visit. One violation of child care requirements was observed and documented. Due to internet connectivity issues, the visit could not be keyed into the DCDEE Regulatory database. A WORD document was completed, and a copy was left with Mrs. Morris. Violation Number Comment Rule 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. One staff completed the Recognizing and Responding of Suspicions of Child Maltreatment training in 2017; this was not renewed within the 5 year time-frame of 2022. .1103(b) The violation must be corrected immediately. You may email verification addressing the individual violation and how it was corrected. The compliance letter must be received by February 25, 2025. NC GS 110- 90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Child care licensing requirements were established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. I explained that you have access to the on-line version of the Child Care Rule Book, as well as a hard copy of the rules. I recommend that you visit the Division of Child Development's website (www.ncchildcare.nc.gov) on a regular basis to find out what's new in childcare. You could also find any needed Child Care forms by going on the web site and clicking on the "Provider" tab and then clicking on the "Provider Documents" tab. Reminders: Based on information obtained via Clean Water for Us Kids, the Lead-in-Water analysis has been completed and RTI has completed the survey for Lead-Based Paint and Asbestos testing. A few reminders were shared with you regarding completing the Staff and Training worksheet: 1. Make sure to put the expiration date of CPR and First Aid training instead of the issuance date. 2. The training date for the EPR training is the actual training for the staff member(s) that completed the required EPR training course. The Review date is when the EPR Plan was reviewed on an annual basis to the other staff. This review is completed by the staff member who took the EPR training course. 3. Make sure all items on the staff and training worksheet are completed. If there are any blanks it appears the information is not on file and could result in a violation. If a certain item doesn’t apply, you can put ‘N/A’. The Child Care Commission is currently in the process of drafting proposed child care rule language to begin the rulemaking process for implementing the new requirements. Sonya Morris, Administrator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Morris, and three additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. On July 19, 2024, a one-year-old sustained a toddler fracture to the leg while in care at the facility. Staff members were unable to confirm when or how it happened. .1801(a)(1-5) 411 Every child was not provided an appropriate time and place to rest. A one-year-old fell asleep in a highchair and staff member failed to move them to an appropriate place for rest. GS 110-91(2)(i) Violations must be corrected immediately.
