Bernice Cromartie, Administrator, assisted me with the visit. You currently operate with a 4-star rated license issued May 16, 2022, with 7 points earned in Staff Education, 2 points earned in Program Standards and 1 point in Quality by meeting an educational option. Restrictions: First and Second shift, meets enhanced ratios. The program’s compliance history was ninety-eight percent as of July 23, 2025. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children were observed during teacher lead activities, free play activities, lunch, and personal care routines. Teachers were interacting and met the needs of each of the children. Children were adequately supervised. No staff files were reviewed due to there being no new employees. The following violation was observed during today's visit. Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The facility did have a completed roster in the ABCMS system. G.S. 110-90.2 & .2703(r) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. On or before August 7, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. Your last annual compliance was conducted on January 21, 2024. Ms. Bernice Cromartie, Owner/Operator, assisted me during today's visit. Based on the information obtained from the NC Secretary of State website, the corporation is current and active. You currently operate with a 4-star rated license issued May 16, 2022 with 7 points earned in Staff Education, 2 points earned in Program Standards and 1 point in Quality by meeting an educational option. Restrictions: First and Second shift, meets enhanced ratios. During today's visit, I conducted a walk-through of the indoor and outdoor areas. Staff/child ratios, group size, capacity, use of space, and supervision were observed. Two groups of children were observed, In space #3, five (5) children, ages 2-3, were present with one educator. Children were participating in story time and preparing for lunch. In space #5, eight (8) children, ages 3-5, were participating in story time as well with one educator. Developmentally appropriate toys and equipment were accessible. Activity plans were current and posted for easy reference. A sanitation inspection was conducted on September 18, 2024 with 0 demerits and a Superior rating. A fire inspection was conducted on December 31, 2024. The Emergency Drill log was reviewed during today’s visit. Staff Records: You submitted a completed staff and training worksheet prior to today's visit. A review of the information was completed. Children Records: I reviewed three children's files. One child was missing the Parent Signature acknowledgement of receipt of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was received. One item of non-compliance was observed and documented during today's visit. Computer-generated report was printed at the conclusion of today's visit. Violation Number Comment Rule 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. One child's file did not contain a parent signature acknowledging receipt of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy. .0608(b)(1-6) The violation must be corrected immediately. You are to submit a signed and dated compliance letter that explains how the violation was corrected. The compliance letter must be received by January 14, 2025. Your Compliance History Child care licensing requirements were established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. I explained that you have access to the on-line version of the Child Care Rule Book, as well as a hard copy of the rules. I recommended that you visit the Division of Child Development's website (www.ncchildcare.nc.gov) on a regular basis to find out what's new in childcare. You could also find any needed Child Care forms by going on the web site and clicking on the "Provider" tab and then clicking on the "Provider Documents" tab. During today's visit I verified through the Clean Classrooms for Carolina Kids website you completed the Lead in Water testing January 19, 2024. You have enrolled in the Lead-Based Paint and Asbestos testing. I reminded you of the CBC Provider Portal. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child providers working who were hired or moved into the child care facility within five business days. This is not captured in ABCMS. You will need to obtain a Business NCID and complete the Provider Portal training in MOODLE. Should you need assistance please contact the Criminal Background Check unit at 1-800-859-0829 and someone will assist you. Rated License: The Governor signed Senate Bill 425 (now Session Law 2024-34), Health and Human Services Omnibus Bill, which includes the Quality Rating Improvement System (QRIS) Modifications recommended to the Legislature by the NC Child Care Commission to update QRIS and create additional pathways for licensed child care facilities to earn star rated licenses. The Child Care Commission is currently in the process of drafting proposed child care rule language to begin the rulemaking process for implementing the new requirements. The rulemaking process will include publishing of proposed revised rules as well as a public hearing and public comment period. Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed.
