    Name of Operation: LONG BRANCH SCHOOL PRIME TIME Facility ID: 78000308 Consultant: NAKITA BELLAMY Operation Type: Center Case Number: Visit Date: 2/29/2024 Number Present: 2 Completed Date: 2/29/2024 Age: From 5 To 10 Total Minutes: 85 Time In: 02:35 PM Time Out: 04:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor compliance with applicable childcare requirements pertinent to an annual compliance visit. Martha Cushion, Program Coordinator assisted me with the visit. The program operated with a three-star license, issued October 26, 2017, earning six points for education, two points for program standards, and one quality point for complying with an education option. License restrictions include daytime care only, school age children only, playground area does not meet child care playground safety standards, meets enhanced ratios, and meets enhanced space. The last annual compliance visit was conducted March 21, 2023. The sanitation inspection was completed February 13, 2023, with a “Superior” classification. The last fire inspection was conducted July 25, 2023, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred percent as of February 29, 2024. Children observed were participating transition to after school care and group activities. Age-appropriate materials and equipment were accessible to the children. Children were adequately supervised. Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. The center last sanitation inspection was February 13, 2023. The facility has requested an inspection from Environmental Health. 10A NCAC 09 .0304(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation documented must be corrected immediately. On or before March 14, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted.
