Upon my arrival, I was greeted by Faith Smith-Johnson, Administrator. Ms, Tarry Johnson, owner, arrived shortly thereafter. The program operates with a 4-Star License issued December 12, 2024, earning 7 points Education Standards, 2 points in Program Standards, and 1 point in Quality by meeting a programmatic option. Restrictions are: 1st and 2nd shift, meets enhanced space, other: Building #1 limited to 30 children. I completed a general walk-through of the indoor and outdoor areas used by the children. Staff/child ratios, group size, capacity, approved space, and supervision were observed. I observed age's 0-4's in Building #1. Children were eating lunch and completing daily routines. I observed staff engaging with children as they ate. I observed thirteen children, age's 3-5 in Building #2 (space 4) eating lunch and preparing for rest time. Today's lunch consisted of hamburger meat with bun, french fries, peaches and milk. REQUIRED INSPECTIONS/DRILLS: A sanitation inspection was completed on October 8, 2024. A current inspection has not been obtained. Mrs. Johnson stated she has contacted Environmental Health regarding needing a current inspection. A fire inspection was completed October 2, 2025. The Emergency Drill Log was reviewed. One violation of child care requirements was observed and documented. Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. A sanitation inspection was completed on 10/8/2024. A current inspection has not been completed. 10A NCAC 09 .0304(b) The violation documented must be corrected immediately. On or before March 10, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. NC GS 110- 90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Technical assistance was provided with the new QRIS Pathways. Your current license was issued in 2024; therefore, you are not required to switch over to the new QRIS Pathways; however, you are interested in what the new requirements are to start putting practices in place. We reviewed the child care center requirements applicable to pathway 2. We reviewed the Family and Community Standards form. Keep documentation on how and when you meet the options. You were provided with a copy of the quality initiatives so you can review it. I provided you with a copy of the Facility CQI Plan, and I provided you with a copy of the Individual CQI and PD Plan. All staff should complete this annually. We discussed curriculum and formative assessments and I gave you a copy of the approved list. You are currently using Creative Curriculum with children who are four years of age. We reviewed the educational requirements for a center. I reminded you to make sure that all teaching staff have a Status letter from DCDEE WORKS. Make sure that the staff have been assessed for the position they are working in. The last annual compliance was conducted on August 10, 2023. Today's visit was conducted with caregiver, Faith Smith-Johnson. The owner, Tarry Johnson, arrived shortly during the visit. The Administrator, Stacy Wright, was not present. You currently operate with a 4-star rated license issued September 11, 2023 with 2 points earned in Program Standards, 7 points in Staff Education, and 1 point in Quality by meeting a programmatic option. Restrictions on the license: 1st and 2nd shift, meets enhanced space. The indoor and outdoor areas were monitored. Staff/child ratios, group size, capacity, use of space, and supervision were observed. I observed children playing in various activity centers, teacher lead activities, and free play. Developmentally age-appropriate materials were used. Positive and nurturing communication occurred as teachers interacted with children. A sanitation inspection was completed on December 19, 2023 with zero demerits and a Superior rating. The last fire inspection was completed on August 16, 2023. You need to contact the local fire marshal to request a fire inspection. Make sure to send me the original inspection within seven days of completion. The Emergency Drill log was reviewed during today’s visit. Prior to today’s visit the staff and training worksheet was obtained. A review of the information was completed. Three staff files were reviewed during today’s visit. During today’s visit, four children records were reviewed. The children’s records form was completed during the visit. Two violations of child care requirements were observed during the visit. Due to computer issues, today's visit could not be completed in Regulatory and a computer-generated report could not be printed. A hand written visit document was completed referencing two violations. Each violation was thoroughly reviewed with Ms. Johnson. She was informed the computer-generated report would be emailed to her within 24 hours. Violation Number Comment Rule 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. One staff's TB test results were older than 12 months. .0701(a) 1898 Staff did not complete the health and safety training within one year of employment. Two staff were missing one of the health and safety training topics. .1102(a) Child care programs must be in compliance with applicable child care requirements at all times. Technical assistance: Allow space and time for active exploration and vigorous exercise for infants everyday. With close supervision, infants should be placed on their tummies to play. Only use cribs for sleeping or preparing to go to sleep, not to keep children contained and confined. Children need opportunities for active physical play inside as well as outside, and this should be a normal part of everyday activities, included in the daily schedule. This gives young children the chance to strengthen muscles and practice skills for healthy development. The Health and Safety trainings for staff were monitored today. NC 10A NCAC 41C .1001-1007 requires lead-based paint and asbestos inspections at all licensed childcare facilities in NC. You can learn more at Clean Classrooms for Carolina Kids. Based on information obtained via Clean Water for Us Kids, the following has been completed: Lead in Water Analysis For Lead-Based Paint and Asbestos you have started the enrollment process. Rated License: The Governor signed Senate Bill 425 (now Session Law 2024-34), Health and Human Services Omnibus Bill, which includes the Quality Rating Improvement System (QRIS) Modifications recommended to the Legislature by the NC Child Care Commission to update QRIS and create additional pathways for licensed child care facilities to earn star rated licenses. The Child Care Commission is currently in the process of drafting proposed child care rule language to begin the rulemaking process for implementing the new requirements. The rulemaking process will include publishing of proposed revised rules as well as a public hearing and public comment period. Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. The purpose of this unannounced visit was to follow-up and monitor compliance with child care requirements observed/confirmed as violations on June 24, 2024. I conducted a walk-through of the facility unaccompanied. During the visit, I spoke with Tarry Johnson, Owner. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. Upon arrival children in the classroom for one/two-year-old children were watching an I-pad. .0510(f) 1041 Prior to employment a Criminal Background Check was not completed. A staff member that cares for school age children did not have a completed criminal background check. G.S. 110-90.2(b) 1949 The center did not report a suspected case of child abuse or neglect as required by the mandatory duty prescribed in G.S. 7B-301. Facility owner and one staff member stated and noted marks observed on a child but nothing was reported to the proper authorities. G.S. 110-91 Violations must be corrected immediately. Within one week July 1, 2024, you must submit documentation of the corrections you made and your plan to maintain compliance with the identified child care requirements to me at the information below.
