The annual compliance visit was conducted on October 16, 2025. Upon my arrival, I was greeted by Michelle Prevatte, Administrator. This facility currently operates with a 4-star rated license issued December 4, 2024. You are applying for a 5-star license through the QRIS Pathways. You have chosen to complete Pathway 2: Classroom and Instructional Quality. The NC Pre-K classrooms will complete the ECERS-3 and final results will be used for NC Pre-K requirements only. I was accompanied by Mrs. Prevatte as I completed a general walk-through of the facility, which consisted of all classrooms, and the outdoor play area. Staff/child ratio, group size, capacity, use of space, license restrictions and supervision were observed. The license and emergency medical care plan were posted. Medication administration procedures were reviewed. REQUIRED INSPECTIONS/DRILLS: The most recent annual fire inspection was completed on October 22, 2025. The most recent annual sanitation inspection was completed January 13, 2026, and received a Provisional classification with 25 demerits. A re-inspection has not been completed at this time. The Emergency Drill log was monitored. One violation of child care requirements was documented. Mrs. Prevatte stated she has corrected all items noted on the Sanitation Inspection report and has informed the EHS specialist she is ready for a re-inspection. Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. The center received a Provisional sanitation inspection on 1-13-2026. 10A NCAC 09 .0304(b) Child care licensing requirements were established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. I recommend that you visit the Division of Child Development's website (www.ncchildcare.nc.gov) on a regular basis to find out what's new in childcare. You could also find any needed Child Care forms by going on the web site and clicking on the "Provider" tab and then clicking on the "Provider Documents" tab. You are required to maintain compliance with all applicable child care rules and regulations at all times. NC GS 110- 90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Compliance letter: In response to the violation cited, you must submit a signed compliance letter that explains how the violation was corrected. Send the compliance letter via email (Miriam.Byrd@dhhs.nc.gov) no later than February 11, 2026. If a re-inspection has been completed prior to this date, please email me the new inspection form. Your Compliance History score is 98% from 7/27/2024 to 1/26/2026. During the visit conducted on 10/16/2025, you stated you wanted to use an approved space to provide care for Infants and Toddlers. Space #4 will be used for children 0-1 year of age. There is a diaper changing area with sink as well as a separate handwashing sink. Space #5 will be used for children one year of age. A statement from Mercedes Locklear, Environmental Health Specialist, was obtained on January 15, 2026, that the rooms are approved for infants and toddlers. Your new license will reflect the age range change from 2-12 to 0-12. You requested information as to what needs to be posted for the new classrooms. The following must be posted in the space used for *Staff/Child Ratio for the classroom *Daily Schedules *Current Activity (lesson) Plan *Safe sleep policy *Current Menu *Individual written feeding plan (for children under 15 months of age) *Daily Attendance QRIS Pathways to the Stars- On November 11, 2025, I received your completed and signed Application for Assessment for a Rated License for Centers. You are applying via Pathway #2- Classroom and Instructional Quality. The program is meeting enhanced reduced staff/child ratios. Your program has two NC Pre-K classrooms. The ECERS-3 will only be used for Pre-K requirements and not licensure. Along with the Application, you submitted the following completed forms: • Family and Community Engagement Standards- you selected ten additional options; 4 are required to meet the Five Star Rated License • Facility Continuous Quality Improvement Plan (CQI) • Staff Information and Education Worksheet I reviewed one Individual CQI/PD staff plan during the visit. Your facility is currently using the approved curriculum, Experience Learning, for all ages served. You are currently using Experience Early Learning Authentic Assessment/Teaching Strategies as a formative assessment for children under four years of age. You use Dial-4/Teaching Strategies for the NC Pre-K program. Your program will share child assessment with families twice, annually. You and the lead teachers will participate in the annual completion of five on-going training hours, in additional to their required number of on-going training hours. Based on today’s conversation the facility has earned a 5-star license through Pathway #2- Classroom and Instructional Quality.     Name of Operation: FIRST BAPTIST EARLY CHILDHOOD MINISTRY Facility ID: 7855027 Consultant: MIRIAM BYRD Operation Type: Center Case Number: 0925-372L Visit Date: 10/2/2025 Number Present: 86 Completed Date: 10/2/2025 Age: From 2 To 5 Total Minutes: 120 Time In: 12:30 PM Time Out: 02:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Self Report Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to obtain information regarding alleged violations of child care requirements due to a self-report. The allegations are as follows: There are concerns that there are violations of child care requirements regarding discipline. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions were monitored. The license and emergency care plan were posted. Children were napping during the visit. Enrollment information was not gathered but all classrooms were in compliance with staff/child ratio, group size, and supervision. Mrs. Michelle Prevatte, Administrator, made a self-report on September 26, 2025, that she observed Ms. Jordan Chavis, teacher of the classroom for two-year-olds, on video footage popping/smacking children’s hands as part of a discipline to get them to stop. She was observed on at least three occasions (9/17/25, 9/18/25, and 9/19/25) with different children. Mrs. Prevatte stated she started was made aware of the incident on Wednesday, September 24, 2025. She stated observing video on Thursday, 9/25/2025 at 6:30 am. She finished viewing the videos around 8:45 am and Ms. Chavis was terminated at 9:08 am on 9/25/2025. I reviewed the information Mrs. Prevatte reported during today’s visit. Based on information received from Mrs. Prevatte, the finding regarding allegation of violation of discipline requirements is substantiated. A compliance letter is not required at this time. Violation Number Comment Rule 904 A child was handled in a rough way, including shaking, pushing, shoving, pinching, slapping, biting, kicking, or spanking. A teacher was observed via video popping two different children on the hand and handling another child in a rough way. This was corrected on 9/30/2025 when a staff meeting was conducted for all staff to review the discipline and behavior policies as well as 10A NCAC 09 .1803 and the NC General Statute 7b-301. .1803(a)(1) During today's visit I obtained documentation of a Staff Meeting, with staff signatures, that was conducted on September 30, 2025 with all staff that covered the following topics: 1. First Baptist ECM Discipline & Behavior Management Policy 2. First Baptist ECM Employee Discipline Policy 3. FB ECM Standards of Conduct 4. FB ECM Child Abuse/Neglect & Reporting Abuse/Neglect Policies 5. 10A NCAC 09 .1803 Prohibited Discipline in Child Care Centers 6. NC General Statute 7B-301-Reporting Suspected Child Abuse and Neglect Technical assistance regarding discipline was provided during today’s visit: A violation was documented for failure to maintain compliance with discipline requirements. Violations of this nature directly impact the safety of the children while in your care. Hitting, spanking, or any physical punishment is never acceptable in licensed child care settings. You want to make sure all staff are aware of written policies prohibiting corporal punishment. Ensure staff handbooks and training materials clearly outline expectations and consequences. You also want to remind all staff they are legally required to report suspected abuse or neglect. I encourage you to provide ongoing training in developmentally appropriate behavior management strategies, support staff in managing their own stress and frustration, encourage the use of breaks, team support, and wellness resources. I encourage you to reach out to Shakita Thompson, Healthy Social Behaviorist, Region 5 at (910) 627-2921 or sthompson@ccpfc.org. She can provide support to licensed childcare centers to address challenging behaviors in young children. She can also offer on-site consultation, training, and resources to teachers, helping them manage challenging behaviors and create positive learning environments. During the conclusion of today's visit, Mrs. Prevatte stated she would like to grievance the violation that was cited during today's visit. I informed her of the grievance procedures. She emailed Janet Edwards, Licensing Supervisor during the visit and attached a copy of her signed and written grievance statement. I informed Mrs. Prevatte she will be contacted either by the Supervisor or Regional Manager with a response within two (2) weeks of receiving the written grievance. At the conclusion of the visit, this visit summary was printed, reviewed, and a copy was left with Ms. Prevatte.
