    Name of Operation: DEEP BRANCH EARLY CHILDHOOD PROGRAM Facility ID: 78000282 Consultant: NAKITA BELLAMY Operation Type: Center Case Number: Visit Date: 2/13/2026 Number Present: 15 Completed Date: 2/13/2016 Age: From 4 To 5 Total Minutes: 120 Time In: 09:00 AM Time Out: 11:00 AM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance with applicable child care requirements for an annual compliance a rated license visit. Stephanie Jacobs, Teacher assisted me with the visit. The program operated with a five-star license, issued February 12, 2018, earning seven points for education, seven points for program standards, one quality point for complying with a programmatic option, and education option. License restrictions include daytime care only, meets enhances ratios minus one, and meets enhances space. The last annual compliance visit was conducted February 26, 2025. The sanitation inspection was completed January 6, 2025, with a “Superior” classification. The last fire inspection was conducted on January 14, 2025, and your facility was approved for daytime care only. Children were observed during free play activities, personal care routines, and lunch. The Teachers were observed interacting with children while they played. Age-appropriate materials and equipment were accessible to children. Children were adequately supervised. Lunch consisted of popcorn chicken, grilled cheese blend sandwich, sweet potato, baked beans, pears, and milk. The following violation was observed during today’s visit Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection on file is dated January 14, 2025. The fire marshal has conducted the inspection and the Lead Teacher has requested a copy of the fire inspection from the Fire Marshal. 10A NCAC 09 .0304(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation documented must be corrected immediately. On or before February 27, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware that any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted.
