BENSON Operation Type: Center Case Number: Visit Date: 5/20/2026 Number Present: 2 Completed Date: 5/20/2026 Age: From 2 To 3 Total Minutes: 120 Time In: 09:00 AM Time Out: 11:00 AM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor compliance with applicable child care requirements pertinent to the second Temporary Time Period Visit. This program is currently operating under a Temporary License effective February 12, 2026 to August 12, 2026. Applicable childcare requirements were discussed with you, Cassandra Baker, Administrator. During this visit I monitored all indoor and outdoor areas approved for child care. I also observed children actively engaging with their assigned teacher and participating in various free-choice centers, including fine motor activities, books, blocks, and dramatic play. Other routine activities such as eating breakfast and handwashing were also monitored. Positive and developmentally appropriate interactions between staff and children, staff/child ratios, and supervision were monitored throughout the visit. The temporary license was posted and in compliance with all restrictions noted on the license. I observed children eating pancakes, sliced pears, and milk for breakfast. The center does participate in a food program. I monitored two (2) children files and applicable program files. No new staff have been hired since my last visit that was conducted on March 25, 2026, so, I didn’t rereview files, however they were available for review. I did review the Staff and Training Worksheet that was completed on March 25, 2026. According to the worksheet one (1) staff didn’t complete orientation within the required timeframe. Overall enrollment and attendance information were collected. The attached Space Capacity and Enrollment Worksheet reflect this information, along with the names of staff present and actively caring for children during this visit. At the conclusion of the visit, the visit summary was reviewed with you, and you were provided with an opportunity to ask questions, seek clarification, and request technical assistance. You were also provided with an electronic copy of this visit summary to keep for your records. Violation Number Comment Rule 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. This was not completed within the required timeframe for one staff. .1101(a) One (1) violation was cited during this visit. This violation must be corrected immediately. I must receive a signed and dated letter of compliance from you no later than June 3, 2026, stating that the violation has been corrected. You can email your compliance letter to: leisa.benson@dhhs.nc.gov. If you cannot meet the requirements by the above date, you must contact me with a proposed timeline of when the corrections will be completed. In some cases, this timeline may be extended. If I don’t receive your compliance letter or you haven’t contacted me by June 3, 2026, a Follow-up Visit could be conducted and violations cited. Please NOTE: If you state in the compliance letter that corrections have been made when they have not, this will be considered falsification of information. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen (18) months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during this visit may negatively affect your compliance score. As of today, your compliance history is 99%. Your compliance history was reviewed with you, and you were also given a copy to keep for your records. Rated License: During the visit conducted on March 25, 2026, I reviewed with you the Pathways to the Stars: The NC Quality Rating and Improvement System (QRIS). During this visit you stated that you will apply for pathway #2—Classroom and Instructional Quality for Child Care Centers. To access the links on the application you can visit: the DCDEE website at: www.ncchildcare.nc.gov and click on the provider tab and then scroll down and click on the provider documents and forms and then click on the Application for Assessment for a Rated License for Centers. All education information for staff should be processed by WORKS no later than July 1, 2026, because education information in WORKS at that time will be used to process your star rated license. Questions about QRIS Modernization (Reform) may be sent to DCDEE_QRIS@dhhs.nc.gov. Keep in mind that In order to be eligible for subsidy payments at the end of the temporary time period, the facility must transition to a three to five star rated license. Reminder: New employees are required to complete orientation within the required timeframe to ensure they understand child rules, health and safety requirements, emergency procedures, supervision expectations, and program policies. I provided technical assistance to help ensure that orientation is completed within the required timeframe going forward. Together we discussed creating an onboarding checklist to use as a reminder of when orientation should be completed.
