Lisa Oxendine, Owner, accompanied me on a walk-through of the premises. The program operated with a one-star license, issued July 15, 2025. License restrictions include first and second shift, and maximum of five preschool at any time. The program’s compliance history was ninety percent as of November 21, 2025. A walk-through of the family child care home was completed today, all indoor and outdoor areas were monitored. I observed children during teacher lead activities, personal care routines, and transition to outdoor free play. I found supervision and staff/child ratios to be in compliance. The caregiver was interacting and meeting the developmental needs of the child. Lunch consisted of chicken nuggets, broccoli, apples, sliced bread and milk. Violation Number Comment Rule 908 Health questionnaire was not completed annually. The last health questionnaire on file was dated July 18, 2024. The provider completed a health questionnaire during todays visit to be in compliance. .1703(a)(1) 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The AMBCMS roster was not completed before or during today's visit. The providers will continue to work on completing all necessary steps to complete a facility ABCMS roster. G.S. 110-90.2 & .2703(r) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The violation(s) documented must be corrected immediately. On or before December 8, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. This program currently operates with a temporary license effective January 14, 2025 to July 14, 2025. You Lisa Oxendine, Operator, was present during this visit. All indoor and outdoor areas approved for child care were monitored during the visit. One (1) two-year-old child was present during the visit. The child was observed during free play. The operator was observed helping the child complete a puzzle. Appropriate interactions were observed between the operator and the child. The child was adequately supervised. During the visit I reviewed one (1) child’s file, program records, and the operator’s file. For breakfast the child was served cereal, milk, and no more than 6 oz. of 100% apple juice. I discussed with you that you can only serve juice at one (1) meal, including snack, per day. At the completion of the visit, the Visit Summary was reviewed with you, and you were given a copy. Violation Number Comment Rule 543 Screen time was not prohibited for children under the age of three. The operator did not offer alternative activities for children under the age of three years. The two year old child was allowed to watch TV. This was corrected during the visit when the TV was turned off. .1718(c) 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Sanitizing wipes were accessible to a child. This was corrected when it was placed in locked storage. .1719 (a)(7) 716 Electrical outlets not in use were not covered. During the visit there was one outlet that was not covered. An application for a star rated license was given to you at the visit. If you decide to apply for a star rated license, you should submit your application to me by March 13, 2025. Education Standards: According to the most recent information in WORKS they were not able to process your education information until you submit the required information. During the visit I gave you a copy of the WORKS Return Letter dated January 24, 2025. It is imperative that your education information is processed by WORKS no later than June 1, 2025. Failure to do so could effect the points you receive for education standards of the rated license. Program Standards: In order to receive two (2) points or higher in program standards you will need to have the FCCERS-3 Assessment; however, to receive more than two (2) points in program standards, you will need to have the FCCERS-3 Assessment and score at least a 4.0. Be sure to visit www.ncrlap.org to familiarize yourself with the FCCERS-3 assessment. I recommend that you contact the Robeson County Partnership for Children to help you prepare for the assessment. Quality Bonus Point: While the facility may meet more than one possible quality point, the facility can only earn a maximum of one quality bonus point. Total points and Star Rated License Information: The total points received in both education and program standards and the quality bonus point will determine the star license issued to your FCCH. To receive a three-star license you must earn at least seven points. To receive a four-star license, you must earn at least ten points. To receive a five-star license you must earn at least thirteen points. In order to earn two or more points in Program Standards for the rated license a three-month self- study on the FCCERS-3 must be completed. If you choose to earn at least two points in Program Standards. You should begin your three-month self-study no later than March 1, 2025 to ensure that it is completed by June 1, 2025. The FCCERS-3 book may be purchased through various retail outlets or some resource and referral agencies. In addition, you may go to www.ncrlap.org for information on the FCCERS-3 process and helpful information. Also, if you meet the requirements for a 4-5 star rated license and plan to provide care to 4-year old’s you will be required to have a curriculum that has been approved by DCDEE. On going training: I cannot determine the on going training hours you will be required to complete until your education information is processed by WORKS. You should complete those hours within the first year of receiving your Temporary License. Documentation of training must be kept on file for review. Complete the health and safety training record as found on the DCDEE website.
