This facility currently operates with a Three Star license issued on January 27, 2025. The last sanitation inspection was completed on September 24, 2025, with a superior classification. The last satisfactory fire inspection was completed on July 17, 2025. T. Miles, Owner/Operator was present and available to assist. Eighteen (18) children, ages ranging from one (1) to nine (9) years of age was present. Children were observed in free choice play, teacher directed activities, and outdoor play. All staff interacted with children in a positive manner. You stated you do not provide transportation. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. Ten percent of staff records and ten percent of children’s records, and all program records were monitored. All children’s records were found to be in compliance. Four (4) violations were documented for staff recordkeeping. One (1) violation was documented for program record keeping. One (1) violation of safety and health for hazards in the outdoor play area and the kitchen being accessible to the children were documented as well. A computer-generated visit summary was reviewed and left with you at the conclusion of the visit. The following violations of childcare requirements were documented today. The last satisfactory fire inspection was completed on June 20, 2024. A fire inspection was completed on June 23, 2025 and yielded an “unsatisfactory” result. T. You stated you will treat the mounds as soon as possible. We discussed putting something around the mounds, so the children and staff don’t have access to the area. CHILDREN’S RECORD KEEPING An immunization record was not file for review for C. Smith. The immunization record should be on file within 30 days of the date of enrollment. PROGRAM RECORDS The facility’s emergency preparedness and response plan (EPR) was observed and was incomplete. REMINDERS The unsatisfactory fire inspection should corrected and submitted to the consultant no later than July 18, 2025. On or before July 29, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is accurate documentation. If any information provided in the letter is not accurate, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. COMPLIANCE HISTORY Prior to today's visit, this facility has maintained an 18-month compliance score of 88%. A copy of the most current compliance history score was printed and left with you at the conclusion of the visit. TECHNICAL ASSISTANCE/ CONSULTATION OUTDOOR PLAY AREA In the outdoor play area used by infants, one-year olds, and two-year olds, there were three (3) active fire ant mounds observed. You stated you will treat the mounds as soon as possible. We discussed putting something around the mounds, so the children and staff don’t have access to the area. CHILDREN’S RECORD KEEPING An immunization record was not file for review for C. Smith. The immunization record should be on file within 30 days of the date of enrollment. PROGRAM RECORDS The facility’s emergency preparedness and response plan (EPR) was observed and was incomplete. REMINDERS The unsatisfactory fire inspection should corrected and submitted to the consultant no later than July 18, 2025. RESOURCES The Division of Child Development and Early Education's (DCDEE) website is https://ncchildcare.ncdhhs.gov/. Visit this website for the most up-to-date child care forms, resources related to child care, and new child care rules/laws. STAR RATED LICENSE Hold harmless has been extended until the new QRIS is implemented (SB 425). Providers in Cohort 1 are not required to go any further with a rated license assessment unless they want to, and providers in Cohort 2 do not need to start their preparation year unless they want to. This should be the providers’ choice only. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. PATHWAYS TO THE PROFESSION The Division of Child Development and Early Education (DCDEE) prioritizes increasing career pathways for future ECE teachers. However, after lengthy careful consideration DCDEE has reluctantly decided it will not bring back the Early Childhood Equivalency Exam due to concerns about academic integrity. We make this decision to ensure fair and equitable opportunity for all aspiring early childhood teachers, uphold minimum educational standards, and keep staff and children in licensed child care environments safe. Note, other Pathways to the Profession of Early Childhood Teacher still include: • EDU 119 – Introduction to Early Childhood Education: This 12-week course introduces the foundations of culturally responsive, equitable and inclusive early childhood education. Available at many N.C. Community Colleges with online, evening, or in-person options. Upon successful completion of EDU-119, the student earns the North Carolina Early Childhood Credential and qualifies to be a lead teacher in a child care center. • Test-Out Option for EDU 119: An early childhood professional who has gained knowledge through work and lived experience and/or professional development may be able to receive Credit for Prior Learning by taking a challenge exam for EDU119. Community College for testing information. • EDU 3119 – Intro to Early Childhood Education: This new short-term (8-week) continuing education offering is designed to prepare students to work in a childcare facility. Students learn how to design developmentally appropriate and inclusive lesson plans and activities for young children in alignment with NC Foundations for Early Learning and Development and state regulations. At the end of the course, students will have obtained a qualifying letter which provides eligibility for employment in a N.C. child care facility. Contact a local N.C. Community College to see if this course is available near you. • Child Development Associate® Credential (CDA): DCDEE is still offering free memberships for a limited time to the ECE workforce to enroll in Teaching Strategies Quorum® online teacher education platform. Scholarships for CDA course materials are available through TEACH Early Childhood® North Carolina Scholarship Program. Remember lead teachers lacking the North Carolina Early Childhood Credential have six months to enroll in EDU 119 or a DCDEE-approved equivalent, plus up to 18 months to complete the course. This program currently operates with a Temporary License issued July 26, 2024. The temporary license is valid through January 26, 2025. This facility is owned and operated by Little Dreamers Academy, LLC. According to the NC Secretary of State, this LLC is active and current. You, T.Miles, are serving as this facility’s administrator. Upon arrival, you were not present. I asked that a staff member contact you to notify you of my visit. She did and I spoke with you via telephone. You stated you were not available to come to the facility today and when asked, you stated staff member, J.Hipps, could assist me with the visit. You also stated I could review the visit information with Ms. Hipps and she could sign the visit summary. When asked and based on today’s observations, three (3) rooms were being used today. Rooms 1A, 2, and 3 were in use today. Rooms 1B and 4 were not in use today. During our phone conversation, you stated you have two (2) school aged children currently enrolled but when asked, they continue to only attend your program when school is out such as during teacher workdays, holidays, and during the summer. You further stated when the school aged children are present, you serve at the group leader. The infant and toddler aged child in Room 2 were interacting with the assigned staff member, while she was providing routine care tasks with the children. Ms. Hipps was present caring for two children, both age 3, in Room 1A. She was assisting the children with coloring while interacting with them. The children present in Room 3 were playing with toys made available and interacting with the assigned staff member. When asked, no new staff have been hired since the last temporary time period visit was conducted. I reviewed all current staff records for the completion of CPR and First Aid, Criminal Background Check Qualification letters and current ITS-SIDS training today. As new staff are hired, ensure to include the staff on the worksheet as well. You are continuing to use the developed Staff Records Checklist to document when information is received and completed. Required center program records were also monitored today. Attendance, sign in/out information, activity plans, schedules, etc. were monitored. Overall attendance and enrollment were collected today. The attached space, capacity and enrollment worksheet reflects the enrollment and attendance information along with the staff that were present and caring for children today. The following violations were documented today: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The posted activity plan in Room 1A was dated 12/16/24-12/20/24. GS 110-91(12); .0508(a) 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. Three bottles were not labeled with the child's name and date in Room 2. 15A NCAC 18A .2804(d) 538 Baby bottles were not stored to protect from contamination. Two bottles did not have tops to cover the nipples of the bottles. 15A NCAC 18A .2804(d) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A can of Frebreze Air Mist was not locked and stored on the window sill in the hallway outside of Room 3. .2820(b) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. Documentation was not available for review for the last month verifying visual safe sleeps were conducted for the infants in care. Visual safe sleep checks were on file for January 7, 2025 only. .0606(g) The violations documented must be corrected immediately. Mail or email me a compliance letter addressing the correction of the violations. The compliance letter is required to be received on or before January 22, 2025. Two (2) of the violations are repeated from the last temporary time period visit. Also, one (1) of the violations was verified as corrected during the visit. Work to ensure violations are not repeated in the future. If violations are repeated, an administrative action could be warranted. COMPLIANCE HISTORY: The compliance history for this facility was 90% prior to today’s visit. The violations documented today may affect the facility’s compliance history score. We reviewed that all facilities must maintain at least a 75% compliance history score over an eighteen-month time frame to be in good standing with DCDEE. If a facility’s compliance history falls below 75% over an eighteen-month time frame, some type of administrative action may be issued. Technical Assistance/Consultation/Reminders Specific to Today’s Visit: SANITATION: Bottles must be labeled and dated for each child and tops for the bottles must be used. Tops were not provided for two (2) bottles observed in Room 2 today. I reminded the staff remind parents that bottles must be accompanied by tops to help prevent the spread of contamination. Three (3) bottles were not labeled with the child’s name and date in Room 2 today. Ensure the bottles labeled with the child’s name and dated each day. If a parent does not label and date the bottle, a staff member can do this when the child arrives. These were documented as violations today and were repeated from the last temporary time period visit conducted November 5, 2024. This information was reviewed with both Ms. Evans and Ms. Hipps today. Per the compliance letter dated November 14, 2024, Ms. Miles stated if bottles are not accompanied with tops the parent will have to take the child with them or bring bottles back with caps on them. Ms. Miles also stated if a parent does not label their child’s bottle, then a teacher or the director would label each bottle. If you, Ms. Miles, are not present, ensure staff are aware of their responsibility to share this with the parents and also properly label and date the bottles. I explained to staff today that they could use masking tape to label bottles with the child’s name and date, if parents did not properly label their child’s bottles. STORAGE OF POTENTIALLY HAZARDOUS ITEMS: One can of Febreze Air Mist was observed on the window sill in the hallway outside of Room 3. When I made Ms. Hipps aware of this, she promptly removed the Febreze Air Mist and stored it in a locked cabinet. I reminded staff to ensure all aerosol containers are stored in locked storage along with any other potentially hazardous items. I further explained that if a sanitation inspection was completed and an aerosol spray was not in locked storage, a provisional sanitation inspection would be issued automatically. This has been reviewed with Ms. Miles before when a disapproved sanitation inspection was issued at this facility September 19, 2024. Staff should conduct a sweep of the facility each morning and throughout the day to ensure potentially hazardous items are properly stored. VISUAL SAFE SLEEP DOCUMENTATION: I reviewed with staff that the last 30 reporting days of visual safe sleep checks are required to be kept on file for review. Due to recent staff changes, staff stated they were not sure where the last visual safe sleep checks were kept or moved. Visual safe sleep checks were observed for January 7, 2025, and that was the only documentation of visual safe sleep checks available today. ACTIVITY PLANS: I reviewed with staff that activity plans must be current and dated for each week. When asked, staff stated they recently completed changes with the groups they were caring for and that is why the most current activity plan was not posted in Room 1A. Ms. Miles can review activity plans to ensure a current and dated activity plan is ready and posted for each week. You can post the next week’s activity plan at the end of the day on Friday to ensure it is posted and ready for Monday, the next week. OUTDOOR ENVIRONMENT: Continue to remove any toys that become broken and replace the toys as needed. Ensure children continue to play outdoors each day, weather permitting, for at least one (1) hour. STAFF RECORDS: J.Hipps, Z.Williams, and C.Evans completed an ITS-SIDS training December 3, 2024. Copies of their ITS-SIDS certificates were on file for review today. Ms. Williams was present caring for the infant aged children today. I verified that Ms. Evans completed CPR and First Aid training December 2, 2024, based on the card on file. Ensure staff continue to complete the health and safety trainings, including the Recognizing and Responding to Suspicions of Child Maltreatment training every five years and maintain copies of certificates on file. Ensure all staff have renewed CPR and First Aid training prior to their renewal dates. Continue to have staff complete training to count towards their on-going training and attach the certificates to the Record Of On-Going Training log. RATED LICENSE ASSESSMENT: The Application for Assessment for a Two Component Star Rated License, and the Application for the Rated License will be completed after today’s visit. I left copies of these forms with Ms. Hipps today to have Ms. Miles sign and return to me via email. I did not review this information with Ms. Hipps today, but Ms. Miles and I reviewed staff education information today and that if all lead teachers did not have the North Carolina Early Childhood Credentials or an equivalency, no more than 2 points could be earned for a lead teacher. Ms. Miles stated via phone call today that she was going to work on hiring some additional staff that have more education and experience to work alongside your current staff. We also discussed the possibility of combining some groups of children, but the ages of the children currently enrolled have to be considered based on applicable NC Child Care rules pertaining to grouping of children. Ms. Miles and I discussed what she needed to do and to let me know of staffing changes by January 17, 2025. Ms. Miles stated she would be in contact with me. The total points for the Child Care Star Rated License will be determined as soon as possible, before the end of the temporary time period. To earn a Three-star license and continue receiving subsidy funds, the facility must earn at least a total of seven (7) points. LICENSE INFORMATION: After a Rated License is issued for this facility, the facility working file will be transferred to the assigned Consultant. I will email you and “cc” the assigned Consultant so you both are aware of when the file is transferred. This facility was issued a temporary license in July, so this facility’s annual compliance month is July. You should receive an email regarding the upcoming annual compliance visit. A routine unannounced visit will also be conducted each year and possibly before the first annual compliance visit.
