Upon my arrival at the Family Child Care Home, I was greeted by the provider, Carol Cone. I stated the purpose of my visit and conducted a walkthrough of the home and playground area. Today children were playing in the front living area of the home and getting ready for lunch. The living room continues to be the primary daycare space. It contains learning centers, a diaper change table, and information required to be posted. This room is set up with several appropriate age toys. The children play in an outdoor area that is fenced in. This area contains developmentally appropriate toys and materials for use by children. The operator's last annual compliance visit was conducted on 1/5/26. The facility currently operates with a Five- Star license that was issued on February 15, 2018. The last Fire Inspection was conducted on 7/20/25 and was approved for daytime care only. The following were monitored: Supervision, staff child ratio, CPR/First Aid training, specialized training, storage of medications, adequate and approved space, program records, and the license was posted with restrictions met. The center has an eighteen-month compliance history score of 99% prior to today’s visit. The following violation was cited during today’s visit and technical assistance was offered to maintain compliance. Violation Number Comment Rule 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. This facility is not registered in the ABCMS system. G.S. 110-90.2 & .2703(r) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received by my office on, or before 5:00 pm April 21, 2026. The letter should include each item’s number of noncompliance cited, describe how and when the violations were corrected. Please include accompanying documentation of compliance. The letter can be emailed to rena.mccaffrey@dhhs.nc.gov. If a compliance letter is not received in my office by 5:00 PM April 21, 2026, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violations may be documented if each violation has not been properly corrected or otherwise addressed. Technical Assistance: Family Child Care Homes: As stated in G.S. 110-90.2 & .2703(r) childcare operators are to notify the Division of any new childcare providers working who were hired or moved into the childcare facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new childcare providers working who were hired or moved into the childcare facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. RATED LICENCE ASSESSMENT Consultation: Today I met with Mrs. Carol to answer any questions she may have about the “Pathways to the Stars” process and to review the requirements to maintain all standards for 5-star license. Mrs. Carol has chosen pathway two. The programs current restrictions are as follows: serves no more than 2 infants less than 1 yr. old. This program serves 1st, 2nd, and 3rd shift. Max of 5 Preschool children. To meet the requirements for a 5-star standard you must adhere to the following restrictions. No more than 4 children under the age of 24 months of the children who are birth to 5 years of age when meeting NCGS110-91 (7)(b)(1). Educational Standards Mrs. Carol has completed Credentials and 15 semesters hours in EC/CD and she has 28 years’ experience. To maintain your 5-Star standards in education you must complete the following. Option 1- Is to complete 2.5 CEU’s which Smart Start is now offering or Southwestern Child Development Center and this is online and has a Competency Evaluation as soon as possible. This is good for three years. Option 2- To complete one more EC/CD course in college that is 3 semester hours. The FCCH has chosen to meet the 5-star requirement in Family and Community Engagement and Foundational Practices by committing to the following practices: Family and Community Engagement This facility communicates with their families through phone and text; this allows families to communicate any information necessary with their child’s caregiver. Communication Option FCCH will offer a family conference twice a year to discuss their children’s needs and progress towards individual goals and to receive feedback from families about their experiences at home and at the child care center. Parents are offered an opportunity to set goals for their child. These conferences can be conducted virtually, on the phone or in person. These parent/ caregiver conferences will need to be documented each time and parents will need to sign and date when the conference occurs. The child’s formal assessment should be attached to the Child Care Consultant to review. Engagement and Leadership The facility offers an enrollment orientation that includes an opportunity for families and children to visit their child’s caregiver prior to the first day of enrollment. Educational Opportunity The facility will offer an educational opportunity annually for parents and teachers to learn together. Today I reviewed your progress of the FCCH Operator CQI plan and Professional Development Plan. You have completed questions 1-4 to get started. This is an ongoing plan over the course of the year or until the goal has been achieved. Once this is achieved a new goal will be established. This FCCH is using an approved curriculum Creative Curriculum and will choose a formative assessment and get back to me as a formative assessment tool, and the operator will maintain 5 hours annually of coaching or training. Today I left an information sheet on requirements and how to meet these standards. It appears that this facility will meet all standards for a Five-Star license once the violation has been corrected and the completion of education standards are met. The license was issued on February 15, 2018. The last Annual Compliance visit was conducted on February 14, 2024. Upon arrival at the Family Child Care Home, I was greeted by the provider, Carol Cone, and she assisted me during the visit. The children were sleeping during their nap time. The back area is used for the primary area for play. The primary care area included a variety of age-appropriate toys and materials for the children to explore. The outdoor play area also contained a variety of toys such as a sand box, riding toys and a small play gym. The last Fire Inspection was conducted on August 15, 2024, and was approved for daytime and night care. A checklist was used today and a full assessment of child care requirements was conducted. The last Fire Drill was conducted on January 2, 2025. The last Shelter- Place was conducted on November 1, 2024. The last Playground inspection was conducted on January 2, 2025. Today I monitored two children’s files. FCCH AND CENTERS LOCATED IN A RESIDENCE OPERATOR’S STATEMENTS OF RESPONSIBILITY and the FCCH AND CENTERS LOCATED IN A RESIDENCE VERIFICATION OF REQUIRED INFORMATION FOR OPERATOR AND ADDITIONAL CAREGIVERS was completed by the owner and was checked for compliance. During today’s visit I monitored Space Requirements, Equipment and Furnishings, Capacity and Supervision, Medications, and Outdoor Area Equipment. The following violations were cited today, and technical assistance was offered. Violation Number Comment Rule 1603 The home was not in compliance with permit restrictions .Upon my arrival there were six children in the home today. GS 110-91 A letter of compliance must be received on or before 5:00 pm February 10, 2025. The violation(s) documented above must be corrected immediately. A signed and dated letter must include the following: *Facility Name and ID number. *List of each violation number. *How each violation was corrected. Please include accompanying documentation of compliance. The letter must be emailed to rena.mccaffrey@dhhs.nc.gov.
