The visit was conducted with you, Peggy Hunter, Operator. The most recent annual compliance visit was conducted on July 16, 2025. This program currently operates with a five-star rated license issued on March 22, 2019. The permit restrictions include maximum of 5 preschool children at any time, 1st shift (daytime care), 2nd shift, 3rd shift (overnight), and no more than 3 children less than 1 year. The program’s compliance history was 97% prior to today’s visit. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year -old children are enrolled. You are currently using Fireflies for your curriculum. You stated your business is not incorporated. A checklist was used to note the requirements monitored today. The FCCH and Centers Located in a Residence Operator’s Statements of Responsibility form and the Verification of Required Information for Operator and Additional Caregivers form were completed and signed during today’s visit. Staff records and children’s records were monitored during the visit. Three children, eleven months to three years of age, were present today. I observed children engaged in free play activities indoors, gross motor activities outdoors, personal care routines, and lunch. The caregiver was actively involved with the children. Four children, eleven months to three years of age, were currently enrolled. You stated no children were enrolled on second or third shifts. The most recent outdoor inspection checklist was completed on June 22, 2026. The most recent fire drill was conducted on June 22, 2026, at 9:30am. The most recent quarterly lockdown drill was conducted on April 20, 2026, at 1:40pm. Your EPR Plan was reviewed and updated on May 2, 2026. The analysis date for the most recent lead water test was April 13, 2024. You may review your facilities results by visiting https://www.cleanwaterforuskids.org/en/carolina/. This website also indicated no asbestos or lead-based paint hazards were identified at your facility. Criminal Background Check Qualifying letters for you and household members were reviewed and were in compliance during today’s visit. All staff and household members were listed on this facilities ABCMS Provider Portal roster. I suggested that you remove the expired qualification letter for a household member listed on the roster. This household members’ current qualification letter is showing on your facilities roster. QRIS Pathways to the Stars Rated License Assessment: You submitted your Application for Assessment for a Rated License for Centers on April 12, 2026, choosing Pathway #1, the Program Assessment pathway. I reviewed and verified your Family and Community Engagement Standards, Facility Continuous Quality Improvement plan, Self Study Verification form, and Rated License Assessment Request Review form. Your three-month self-study was completed from December 1, 2025, through April 11, 2026. The FCCERS-3 assessment was ordered on April 21, 2026. The FCCERS-3 assessment was conducted on June 9, 2026, and your facility scored 5.25 points. During today’s visit, I reviewed each FCCERS-3 item on the Facility Summary Report with you. Items that could have been cited as violations of Child Care Requirements if observed by a representative of the Division of Child Development and Early Education were also reviewed. I reviewed each indicator noted in the report and provided technical assistance. You had an opportunity to state corrections. All indicators with a score below 5 points listed on the Facility Summary Report were reviewed in an effort to enhance quality. I reviewed these items with you and made suggestions to improve your score for the next Environment Rating Scale assessment. I asked if you had any objections to the score and/or items that scored low. You stated that you did not want to submit a grievance. Your education was current in WORKS, and you meet five-star education standards for a Family Child Care Home operator. The Pathway 1 – Program Assessment Star Level Assessment form for Family Child Care Homes was completed and signed during the visit. Your facility has received a 5-star rated license effective June 25, 2026. Your new license will be issued and mailed to you within 4-6 weeks. You sent your Quality Initiatives Recognition Form to me via email on April 29, 2026. All selected quality initiative items were verified during today’s visit. The violation observed today was discussed with you and documented in the Visit Summary left with you at the conclusion of this visit. The following violation was observed: Violation Number Comment Rule 2023 Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. Previous health and safety trainings for P. Hunter, operator, were completed on September 13, 2020. All required health and safety trainings were not completed again, as required within five years, until May 2, 2026. .1703(d)(2) The violation observed was corrected during the visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical Assistance for the violation observed was provided on the following: Health and Safety Trainings- The operator completed all required health and safety trainings on September 13, 2020, and did not complete all required trainings again within five years. You completed all required health and safety trainings by May 2, 2026. I suggested that you utilize the Record of Health and Safety Training log available on the DCDEE website to assist you with tracking required health and safety training. You stated and I observed that you have created a file specifically for health and safety trainings and you are tracking these trainings on the Record of Health and Safety Training log. Health and Safety trainings may be completed through DCDEE Moodle or with your local Resource and Referral agency. The operator must complete health and safety training as part of on-going training so that every five years, all the topic areas are covered. The next series of health and safety trainings must be completed by May 2, 2031. Consultation: -I reminded you to renew your First Aid and Cardiopulmonary Resuscitation (CPR) certification by August 13, 2026. -I reminded you to complete ITS-SIDS training again prior to the expiration date of August 22, 2026. You stated you are registered to complete ITS-SIDS training on June 30, 2026. Reminders and Resources: -Providers can speak to a Behavior Support Advisor for advice and resources specific to the challenging behaviors in their classroom by calling 1-888-600-1685 Option 1. Providers may also submit questions to a Behavior Support Advisor online and receive a call or email in response. In addition, providers may post questions in the ‘Talk to the Expert’ Group on an online network, Social Emotional Connections, for early childhood educators. The visit was conducted with you, Peggy Hunter, Operator. You stated your FCCH is not incorporated. You indicated your facility was closed last week, June 30, 2025 through July 4, 2025. The following complaint allegation was received on June 24, 2025: There are allegations of violations of child care requirements. The allegations were regarding sanitation/health and criminal records. I discussed the allegation with you. You were given the opportunity to ask questions and respond to the allegations. I conducted a walkthrough of the indoor and outdoor child care space. Limited monitoring was conducted. Four children, one to four years of age, were present. Children and the caregiver were observed during diaper changing routines, handwashing routines, free play activities indoors, gross motor activities outdoors, lunch and nap. You stated five children, one to five years of age, are currently enrolled. I reviewed criminal background check qualifying letters for you, household members, and emergency caregivers. You stated your husband and your son, who live in the home, cover your planned and unplanned absences, which occur occasionally 1-2 times per month for less than 5 hours a week. Your husband and your son had a current criminal background check qualification letter on file for review. You stated a child in your care previously had hand, foot, and mouth. You stated the child kept putting her hands and toys in her mouth. After you observed the child continuously putting her hands in her mouth, you stated you "got scared" and sprayed diluted bleach solution on the child’s hands and rinsed them thoroughly with water and soap at the sink. You stated you have never sprayed diluted bleach solution on a child’s hands previously and you indicated the bleach solution was not strong enough to hurt the child. When asked, you indicated your bleach solution is prepared daily using one teaspoon of bleach with one quart of water and kept in a spray bottle. Based on interviews, information regarding the allegation concerning sanitation and health was confirmed, therefore the allegation was substantiated. Based on interviews and a review of documentation, information regarding the allegation concerning criminal records was unconfirmed, therefore the allegation was unsubstantiated. Violations observed today were discussed with you and documented in the Visit Summary left with you at the conclusion of this visit. The following violations were observed: Violation Number Comment Rule 1887 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser were not in their original container or in another labeled container, or used according to manufacturer's instructions. Bleach was not used in accordance with manufacturer's instructions. On June 23, 2025, the operator sprayed diluted bleach solution on a child's hands after the operator reported the child kept putting their hands in their mouth. The caregiver indicated the child had previously been diagnosed with hand, foot, and mouth disease. The diluted bleach solution contained one teaspoon of bleach to one quart of water. .1719(a)(7) 1954 Operator did not follow hand washing procedures. On June 23, 2025, the operator used diluted bleach solution to wash a child's hands and then rinsed the bleach solution from the child's hands with water and soap. .1725(a)(9)(A-F) The violations documented above must be corrected immediately. Send me a letter of compliance describing in detail specifically how each violation was corrected and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit and license ID number in the letter. I must receive your compliance statement by July 21, 2025. Please send your letter to Kristen.Mauney@dhhs.nc.gov. Failure to correct the violations and send the written statement by the due date may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Please be aware that any information submitted by you is considered legal documentation. If you state in your violation correction letter that corrections have been made when they have not, it will be considered falsification of information. Technical Assistance was provided on the following: Hazardous Products- The manufacturer’s instructions and intended use for bleach are for cleaning, sanitizing, and disinfecting. Bleach or bleach solutions should not be used for handwashing in a childcare setting. Washing hands with soap and water is the best and safest way to remove germs and dirt. Alcohol-based hand sanitizers are a good alternative when soap and water are not available, especially during outdoor play, as long as children’s hands are washed with soap and water when returning indoors. As a reminder, hazardous products, such as bleach, must be kept in locked storage inaccessible to children. Bleach solutions may be kept at a vertical distance of five feet above the finished floor, inaccessible to children. Storage of hazardous products was in compliance during today’s visit. Handwashing Procedures- Handwashing procedures shall include using liquid soap and water, rubbing hands vigorously with soap and water for 15 seconds, washing all surfaces of the hands, to include the backs of hands, palms, wrists, under fingernails and between fingers, rinsing well for 10 seconds, drying hands with a paper towel or other hand drying device, and turning off the faucet with a paper towel or other method without recontaminating the hands. Washing hands with soap and water is the best and safest way to remove germs and dirt. I provided you with a resource from Caring for Our Children, Chapter 3: Health Promotion and Protection, specifically pertaining to handwashing procedures. Consultation was provided on the following: -I suggested that you update your policies pertaining to exclusion and returning to childcare after an illness to include requiring a note from the doctor indicating the child is not contagious and is able to return to childcare. -An individual who provides care during unplanned absences of the operator, such as medical emergencies, shall be 18 years old and submit criminal records check forms as required in Rule .2703(j) of this Chapter. The children of an emergency caregiver shall not be counted in the licensed capacity for the first day of the emergency caregiver's service. -Per your request, we reviewed the following: 10A NCAC 09 .1729(a) states an individual who provides care for five hours or more in a week during planned absences of the family child care home operator shall: (1) be 21 years old; (2) have a high school diploma or GED; (3) have completed a First Aid and cardiopulmonary resuscitation (CPR) course within 12 months prior to caring for children; (4) have completed a health questionnaire; (5) have proof of negative results of a tuberculosis test or screening completed within 12 months prior to the first day of providing care; (6) have submitted criminal background check forms as required in Rule .2703 of this Chapter; (7) have documentation of annual on-going training as described in Rule .1703(d) of this Section after the first year of employment; (8) have completed ITS-SIDS training, if licensed to care for infants; (9) have completed Recognizing and Responding to Suspicions of Child Maltreatment training; and (10) have documentation that the operator has reviewed the requirements found in this Chapter, including the Emergency Preparedness and Response Plan, and in G.S. Chapter 110, Article 7. While the individual provides care at a family child care home, copies of information required by Subparagraphs (1) through (10) of this Paragraph shall be on file in the home and available for review by the Division. (b) An individual who provides care for less than five hours in a week during planned absences of the operator shall be literate and meet all requirements listed in Paragraph (a) of this Rule except the requirements for annual training and a high school diploma or GED. MAUNEY Operation Type: Family CC Home Case Number: Visit Date: 9/27/2023 Number Present: 4 Completed Date: 9/27/2023 Age: From 1 To 2 Total Minutes: 135 Time In: 09:40 AM Time Out: 11:55 AM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements, including health and safety training, for an annual compliance visit. You, Peggy Hunter, Operator, assisted me with the visit. Your program currently operates with a Five star rated license effective March 22, 2019. The program’s compliance history was 100 percent as of September 27, 2023. You stated your business is not incorporated. The FCCH and Centers Located in a Residence Operator’s Statements of Responsibility form and the Verification of Required Information for Operator and Additional Caregivers form was completed and signed during today’s visit. Your signature on these forms served as verification the information provided was accurate and complete. Your staff records were monitored during the visit. Four children were present today. I observed children engaged in free play activities indoors, art activities, and gross motor activities outdoors. The caregiver was actively involved with the children. Four children, one and two years of age, and one school age child were currently enrolled. The most recent outdoor inspection checklist was completed on September 19, 2023. The most recent fire drill was conducted on September 20, 2023. The most recent quarterly shelter-in-place drill was conducted on July 20, 2023. Criminal Background Check Qualifying letters for you and household members were reviewed and were in compliance during today’s visit. The violation and technical assistance were thoroughly reviewed with you and documented in the hand written Visit Summary left with you at the conclusion of the visit. The computer-generated Visit Summary was emailed to you after the visit was conducted due to connectivity issues on September 27, 2023. The following violation of child care requirements was observed during today’s visit. Violation Number Comment Rule 1893 Application did not include all required information including, but not limited to: child's full name, named to be called, child’s date of birth, any allergies, including symptoms and the type of response required, and any fears or behavior characteristics a child has. The application for one child enrolled on 10/17/2022 did not include any allergies or health needs nor any fears or behavior characteristics the child may have. 10A NCAC 09 .1721 (a)(3)(A)(B)(C)(E) The violation documented above must be corrected immediately. Send me a letter of compliance describing in detail specifically how the violation was corrected and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit and license ID number in the letter. I must receive your compliance statement by October 11, 2023. Please send your letter to Kristen.Mauney@dhhs.nc.gov. Failure to correct the violation and send the written statement by the due date may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period.
